Towards eliminating lymphatic filariasis in Papua New Guinea: impact of annual single-dose mass treatment on transmission of Wuchereria bancrofti in East Sepik Province.

The impact of annual single-dose community-wide treatment on the transmission of Wuchereria bancrofti was investigated in 5 villages in the East Sepik Province where pretreatment prevalence of microfilaraemia ranged from 34% to 73%. Anopheles punctulatus and An. koliensis were the only carriers of the parasite. 3 villages received diethylcarbamazine citrate (DEC) in combination with ivermectin (IVR) and 2 received DEC alone. The rate and intensity of microfilaraemia were both reduced in all 5 villages. Reduction in prevalence was between 43% and 67% in the DEC+IVR study villages and between 24

Common features of T cell reactivity in persistent helminth infections: lymphatic filariasis and schistosomiasis.

This review highlights striking similarities in the down-regulated inflammatory responses that are observed in chronic infectious diseases. It is clear that peripheral mononuclear cells show poor antigen-specific T cell proliferation in lymphatic filariasis as well as schistosomiasis. With respect to cytokines both Th1- and Th2-type products are down-regulated during chronic infection. Drug treatment restores responsiveness in both diseases indicating a link between infection and immune suppression. Interestingly, immunological responses in subjects carrying recent infections are characterized

Simple blood-spot sampling with nested polymerase chain reaction detection for epidemiology studies on Brugia malayi.

In the absence of a suitable Brugia malayi antigen detection assay, PCR remains one of the more sensitive alternatives to Giemsa-stained thick blood films for B. malayi detection. The need for refrigerated storage and transportation of blood has limited the use of PCR for large-scale epidemiology studies in remote endemic areas. Here we report simple finger-prick blood-spot collection, a one-tube DNA template extraction method and the development of a B. malayi-specific nested PCR assay. The assay was tested on 145 field samples and was positive for all 30 microscopy-positive samples and for a

Lymphatic filariasis in two distinct communities of upper Assam.

Current status of bancroftian filariasis among two distinct communities of Upper Assam was evaluated. High prevalence (10.3% mf rate) in tea workers descendents of tribals migrated from endemic states such as Orissa, Bengal, Uttar Pradesh and southern states of India, were recorded. Among non tea workers (indigenous population) significantly low mf rate was found in spite of the fact that both the communities are living in close vicinity and in similar ecological and mosquitogenic conditions. Culex quinquefasciatus was incriminated as vector in the areas of both the communities. A group of rec

[Views on the municipalization of lymphatic filariasis control in greater metropolitan Recife].

Lymphatic filariasis is caused by the nematodes Brugia malayi, Brugia timori, and Wuchereria bancrofti. The disease occurs in developing countries and is more frequent in urban areas. An estimated 4 billion people live in at-risk areas. In Brazil the endemic is caused by W. bancrofti and was first documented in 1878. It was first detected in Recife in 1952. Currently, Recife and Belém are the only cities in Brazil where the endemic is considered a public health problem. The objectives of this study are to discuss the epidemiology and control of lymphatic filariasis and review its control since

Permethrin-impregnated bednet effects on resting and feeding behaviour of lymphatic filariasis vector mosquitoes in Kenya.

The impact of permethrin-impregnated bednets on resting and feeding behaviour of mosquito vectors of Wuchereria bancrofti, causing human lymphatic filariasis was studied in six pairs of villages (treated and untreated) before and after intervention. The study villages were in Kwale District, near the coast of Kenya, where Bancroftian filariasis is highly endemic, transmitted by a combination of both anopheline and culicine mosquito vectors. Mosquitoes were collected weekly in each village, indoors (using pyrethrum spray catches) and outdoors (using pit traps) during 3-4 months following the lo

Lymphatic pathology in Wuchereria bancrofti microfilaraemic infections.

To determine the extent of lymphatic disease in persons infected with Wuchereria bancrofti who were microfilaraemic, we examined the superficial lymphatics of the legs by scintigraphy. In 4 endemic control subjects and in 10 of 14 clinically asymptomatic microfilaraemic individuals, lymphoscintigraphy revealed one major channel of lymphatic drainage in each leg. However, while trunk lymphatics were bilaterally symmetrical in the control, marked differences in the calibre of lymphatic vessels were observed in the microfilaraemic persons. Non-discrete lymphatics and a diffuse symmetrical distrib

[Experimental infection of periodic Brugia malayi in man].

We described the results of ten-year observations on the clinical manifestations, pathology, immunity to filariasis and aetiological biology of filariae in three volunteers (first author and his family members) who were inoculated experimentally with infective larvae of periodic Brugia malayi in 1981 and without use of any antifilariasis drugs. Microfilariae were first detected at 41 and 46 weeks after inoculation in two subjects and remained detectable in a small number until 8 and 8.5 years after infection. Eosinophilia occurred mainly before and at the initial stage of microfilaremia. The i

Correlation between the preferred biting site of Culex quinquefasciatus and the region of the body affected by clinical filariasis.

Entomological and filariasis surveys were conducted simultaneously in a rural and an urban environment in West Bengal from September 1986 to August 1987. The aim of the surveys was to see if any correlation existed between the preferred biting site of Culex quinquefasciatus, the filarial vector, and the region of the body affected by clinical filariasis. Significantly more mosquitoes (> 55%) attacked the thighs, legs and feet of human volunteers than other sites on their bodies, whether the volunteers were indoors or outdoors, in rural or urban environments. Significantly more patients had cli

Microfilariae in bronchial brushing cytology of symptomatic pulmonary lesions. A report of two cases.

Two cases of pulmonary filariasis with atypical clinical and radiologic features without significant eosinophilia are presented. A diagnosis of pulmonary filariasis could not have been made without bronchial brush cytology, which showed sheathed microfilariae of Wuchereria bancrofti. Both cases responded well to antifilarial treatment, indicating that the microfilariae were not an incidental finding but were responsible for the symptoms and radiologic lesions.

Brugia malayi in Mastomys natalensis: efficacy of mebendazole in combination with Freund's complete adjuvant.

The chemotherapeutic efficacy of mebendazole given in combination with Freund's complete adjuvant (FCA) against Brugia malayi in multimammate rat was evaluated. Animals treated with mebendazole, orally at 200 mg/kg x 5 consecutive days along with FCA given subcutaneously (s.c.) on day -10, day 0 and day +15 of the drug treatment killed 48.51% of the adult worms. This drug given alone at the same regimen and by the same route showed only 18.7% mortality rate on adults. Mebendazole given intraperitoneally along with FCA given s.c., however, was four times more efficacious as filaricide than mebe

Age-related changes of the susceptibility to infection with Brugia pahangi in male and female BALB/c mice.

Effects of host age and sex on murine Brugia pahangi infection were examined. In female BALB/c mice, partial resistance to B. pahangi developed rapidly with age. In animals inoculated at 5-6 wk of age, resistance was about 2 times higher than in the youngest group (2-3 wk) and remained at a similar level through the study period (12 wk). Male BALB/c mice showed a similar tendency until 5-6 wk of age, but afterward their susceptibility increased with age. Significant difference in the susceptibility was detected between sexes after sexual maturation. After macrophage blockade by an injection of

[Microsurgical therapy of lymphedema of the external female genitalia].

Lymphedema of female external genitalia is an uncommon disorder, for which ablative treatment procedures have been used in the past. Such an operation is most traumatic and achieved unsatisfactory results. On the basis of our experiences in treating lymphedema of the extremities by lymphaticovenous anastomosis, the author developed a microlymphaticovenous operation for treating this condition. From March 1980 to November 1984, a total of 6 patients with lymphedema of female external genitalia were operated and good results were achieved. The edema of the vulva, clitoris, mons veneris and of th

A quantitative analysis of the diagnostic value of diethylcarbamazine provocation in endemic Wuchereria bancrofti infection.

The efficiency (delta) of the day-time provocative effect of diethylcarbamazine (DEC) on the circulation of microfilariae (mf) of nocturnally periodic Wuchereria bancrofti compared with the night mf count was evaluated in terms of sensitivity and specificity. In one test, the mean mf count for finger-blood samples from the same person on 3 successive nights was compared with the day-time post-DEC count; another test compared the mf count in one night-time finger-blood sample with the following day-time post-DEC count. Both tests were efficient (delta = 0.74 and 0.77, S.E. = 0.07 and 0.07, resp

Whole body lymphangioscintigraphy in ferrets chronically infected with Brugia malayi.

Whole body lymphangioscintigraphy was performed after intradermal injection of 99mtechnetium human serum albumin or antimony colloid in the distal hindlimbs and forelimbs of ferrets chronically infected with Brugia malayi. The findings were compared with control ferrets and those with surgical interruption of the iliac lymphatics. While only one infected ferret manifested chronic hindlimb lymphedema, all exhibited delayed transport of radioisotope from the hindpaw with obstruction in the groin, poor or absent visualization of central lymphatic channels and regional lymph nodes, a picture simil

Immunodiagnosis of bancroftian filariasis.

The development of immunodiagnostic tests (IDTs) for bancroftian filariasis must be aimed at defined objectives, such as the determination of exposure rates, the detection of microfilaraemia and the diagnosis of clinical filariasis. Assays for both antibody and antigen detection are necessary. Antigen determination may be more appropriate in the detection of microfilaraemia, but for clinical filariasis, antibody determination may prove more useful. The microfilarial surface antigens are very good candidates for this purpose. Determination of antibody to larval antigens may be the best way of e

Development of Brugia malayi in Mongolian gerbils previously exposed to Wuchereria bancrofti.

Twelve Mongolian gerbils, Meriones unguiculatus, were infected with 100 third-stage larvae of Wuchereria bancrofti. One month later these animals, along with 4 control animals, were given 100 third-stage larvae of Brugia malayi. Eleven of the 12 experimental animals and the 4 controls survived, and 8 of the experimental animals and all of the controls demonstrated microfilaremia after 3 months. The animals were killed at 6-months post-infection and examined for parasites. One W. bancrofti larva was found in one of the experimental animals, and 15% of the B. malayi given were recovered as adult

The disappearance of bancroftian filariasis from Charleston, South Carolina.

I suggest the hypothesis that bancroftian filariasis, endemic since the early days of slavery in Charleston, South Carolina, disappeared around 1930 by virtue of the long-term effects of a municipal sewerage-water system begun in the 1890s or thereabouts. These public works, originally intended to abate typhoid and related diseases, helped to eliminate filariasis by reducing the availability of polluted domestic waters which are the preferred breeding sites for the urban vector, Culex quinquefasciatus (= fatigans). Cause and effect having been separated by some three decades, the epidemiologic
