BACKGROUND: Transcatheter closure of a patent foramen ovale (PFO) is an established strategy for secondary prevention of cryptogenic stroke in selected patients. During pregnancy, the timing and method of closure require individualized assessment of recurrent embolic risk and maternal-fetal procedural risk. In selected first-trimester patients, avoiding fluoroscopy, contrast, procedural transoesophageal echocardiography (TEE), and sedation may be desirable when adequate alternative imaging is available.
CASE SUMMARY: A 34-year-old woman at 7 weeks' gestation presented with acute ischaemic str

INTRODUCTION AND OBJECTIVES: Transesophageal echocardiography (TEE) has been the standard method for guiding left atrial appendage occlusion (LAAO) procedures. Recently, intracardiac echocardiography (ICE) has emerged as an alternative to TEE due to several advantages, particularly the avoidance of general anesthesia. This analysis aims to assess the safety, feasibility and efficacy of ICE-guided LAAO procedures.
METHODS: We performed a retrospective analysis of ICE-guided LAAO procedures, including a comparison of embolic and bleeding events with the predicted standard scores and a compariso

Ischaemic strokes resulting from atrial fibrillation (AF) constitute a devastating condition for patients and their carers with huge burden on health care systems. Prophylactic treatment against systemic embolization and ischaemic strokes is the cornerstone for the management of AF. Effective stroke prevention requires the use of the vitamin K antagonists or non-vitamin K oral anticoagulants (NOACs). This article summarises the latest developments in the field of stroke prevention in AF and aims to assist physicians with the choice of oral anticoagulant for patients with non-valvular AF with d

Ischemic stroke is a rare but serious complication of cardiac catheterization. We report a case in which systemic thrombolytic therapy was successfully utilized in treating a patient with a cerebellar stroke, leading to obtundation during elective cardiac catheterization. Underlying bilateral vertebrobasilar disease with thrombotic embolization to the basilar artery was postulated to be the pathophysiological basis for the stroke and subsequent success of thrombolytic treatment in this patient. As the consequences of this rare complication are severe, systemic thrombolytic therapy should be co

OBJECTIVES: We sought to assess the association between the presence of a septal pouch in the left atrium and ischemic stroke.
BACKGROUND: It has been suggested that the presence of a left septal pouch (LSP) may favor the stasis of blood and possibly result in thromboembolic complications. However, the embolic potential of an LSP is not known.
METHODS: The association between an LSP and risk of stroke was assessed using a population-based case-control study design. The presence of an LSP was assessed by transesophageal echocardiography in 187 patients >50 years of age with a first-ever ische

BACKGROUND: The 'Timing Hypothesis' states that the benefits and harms of hormone replacement therapy (HRT) are related to the proximity with which it is begun following the onset of menopause. The primary aim of this analysis was to test for heterogeneity of treatment effect for HRT using Chi2 and I2 tests for younger versus older initiators of HRT. The secondary aim was to perform a meta-regression with mean age at trial baseline as the covariate for various outcomes.
METHODS: Younger initiation trials were defined as those with mean age of participants <60 years and older initiation trials

BACKGROUND: Only few studies in selected cohorts have examined whether the CHA2 DS2 -VASc score can predict the risk of atrial fibrillation and thromboembolic events in patients without atrial fibrillation.
MATERIALS AND METHODS: Patients with coronary angiography performed between 2004 and 2012 were grouped according to CHA2 DS2 -VASc score. We excluded patients with atrial fibrillation, anticoagulant therapy and follow-up <30 days. The endpoints were atrial fibrillation and a composite of ischaemic stroke, transient ischaemic attack and systemic embolism. Event rates per 100 person-years we

Several reports describe a potential association between prolonged flights and stroke. However, causes of travel-related stroke due to paradoxal embolism other than patent foramen ovale have not been previously reported. We here describe the case of a 44-year-old woman who presented with an acute anterior circulation stroke after a transoceanic flight. The patient received intravenous thrombolytic therapy, with complete recanalization of the anterior and middle cerebral arteries. Contrast transthoracic echocardiography was suggestive of an extracardiac right-to-left shunt. Both cardiothoracic

BACKGROUND: Thrombus in transit through a patent foramen ovale (PFO) is a rare clinical entity that can be associated with concurrent pulmonary embolization and lead to cryptogenic stroke.
CASE SUMMARY: A 56-year-old patient presenting with shortness of breath and chest pain was diagnosed with large bilateral pulmonary emboli. Computed tomography demonstrated thrombus in the right atrium. Transthoracic echocardiogram demonstrated a large burden of thrombus in transit trapped in a PFO. After multidisciplinary discussion, emergency surgical thrombectomy and PFO closure were carried out successf

Carotid artery stenting (CAS) is an established revascularization option for patients with carotid disease at high surgical risk. Periprocedural cerebral embolization remains a clinical concern, and plaque prolapse, the extrusion of atherosclerotic material through the stent struts into the lumen, has emerged as an actionable mechanism directly linked to embolic events and adverse neurological outcomes. This narrative review provides a structured, practice-oriented framework addressing one specific question: how can plaque prolapse be prevented during CAS? Drawing on prospective registries, co

BACKGROUND: Transcatheter vacuum-assisted mass extraction is a safe alternative to surgical thrombectomy in high-risk patients, although experience with left-sided intracardiac material is limited.
CASE PRESENTATION: A 19-year-old man with newly diagnosed Hodgkin lymphoma developed airway compromise, cardiac tamponade, massive pulmonary embolism, and cardiogenic shock requiring venoarterial-venous extracorporeal membrane oxygenation. Echocardiography showed a mobile biatrial thrombus traversing a patent foramen ovale. Because of prohibitive surgical risk, he underwent AngioVac (AngioDynamics,

BACKGROUND: Atrial fibrillation significantly increases the risk of ischaemic stroke, primarily due to left atrial appendage (LAA) thrombosis. Percutaneous left atrial appendage occlusion (LAAO) offers an alternative approach to oral anticoagulation. However, the presence of LAA thrombosis has been considered a contraindication due to the risk of cerebral embolization.
CASE SUMMARY: An 82-year-old woman presented to our department with dyspnoea and history of non-valvular persistent atrial fibrillation as well as heart failure. Serial transoesophageal echocardiography revealed persistent LAA

Infective endocarditis (IE) is a rare but serious disease, and despite improvements in diagnostic and therapeutic tools, it remains associated with high mortality. It can develop in a healthy heart, but most commonly in underlying heart disease. The authors discuss a 34-year-old patient, who has presented for 4 months with generalized asthenia, fever, and chills, without any notion of chest pain, arthralgias. On admission, the patient was conscious, hemodynamically, and respiratory stable, the temperature was 38.5°C. The cardiac exam heart revealed a systolic murmur rated 4/6th at the mitral f

INTRODUCTION: Cerebrovascular events are one of the most serious consequences after transcatheter aortic valve replacement (TAVR). More than half of the cerebrovascular events following TAVR are due to procedure-related emboli. Embolic protection devices (EPDs) have the potential to decrease cerebrovascular events during TAVR procedure. However, randomized controlled trials (RCTs) have not conclusively determined if EPDs are effective, likely because of small number of patients enrolled. However, meta-analyses and propensity-matched analyses from large registries have shown efficacy and sugges

Approximately 750,000 US adults per year experience a stroke. On average, the annual risk for future ischemic stroke (secondary stroke) after an initial ischemic stroke or transient ischemic attack is approximately 3% to 4%. Cardioembolic strokes account for 20% to 25% of all strokes, with nonvalvular atrial fibrillation (NVAF) considered one of the main sources of embolism; this explains up to half of all cardioembolic strokes. We present the risk factors for stroke in NVAF, risk stratification, a diagnosis of NVAF, and treatment and prevention of stroke in NVAF. We reviewed the literature by…

Although evidence indicates that the incidence of stroke has declined over the past 30 years, stroke remains one of the leading causes of disability and death in the United States. Prevention appears to be the best treatment. According to the Straub Stroke Registry, 52% of strokes in its patient population can be attributed to a thrombotic mechanism. Besides thrombosis, the major causes of stroke are embolism, lacunar infarct, cerebellar infarction, and hemorrhage. Hypertension is a widely recognized risk factor for stroke, but the risks vary for each type of stroke. Transient ischemic attacks

An unusual case is presented of a tourist who developed fatal cerebral air embolism, pneumomediastinum and pneumopericardium while ascending from low altitude to Europe's highest railway station. Presumably the air embolism originated from rupture of the unsuspected bronchogenic cyst as a result of pressure changes during the ascent. Cerebral air embolism has been observed during surgery, in scuba diving accidents, submarine escapes and less frequently during exposure to very high altitude. People with known bronchogenic cysts should be informed about the risk of cerebral air embolism and surg
