Inducible nitric oxide synthase in the liver: regulation and function.

The inducible nitric oxide synthase (iNOS) gene is expressed by hepatocytes in a number of physiologic and pathophysiologic conditions affec … g sepsis and ischemia reperfusion. Its cytoprotective role is best exemplified in a rodent model of endotoxemia. Here the addition of the nonspecific NOS inhibitors significantly increased hepatic damage. NO exerts a protective effect through its ability to prevent intravascular thrombosis by inhibiting platelet adhesion and neutralizing toxic oxygen radicals …

Salt and water transport across alveolar and distal airway epithelia in the adult lung.

Substantial progress has been made in understanding the role of the distal airway and alveolar epithelial barriers in regulating lung fluid … ium, the alveolar epithelium is resistant to several clinically relevant types of injury, including endotoxemia and bacteremia as well as aspiration of hyperosmolar solutions. In addition, even when the alveolar barrier has been injured, its capacity to transport edema fluid from the distal air spaces of the lung recovers rapidly. Future studies need to integ …

Insulin potentiates lipopolysaccharide-induced IL-6 expression through epigenetic remodeling in adipocytes: in vitro and in vivo mechanistic study.

Interleukin-6 (IL-6) is a central mediator of chronic low-grade inflammation associated with metabolic disease. Because obesity is characterized by elevated circulating insulin and metabolic endotoxemia, we investigated whether insulin modulates lipopolysaccharide (LPS) induced IL-6 expression in adipocytes and examined the underlying epigenetic mechanisms. Insulin priming markedly enhanced LPS-induced Il6 mRNA expression (25.33 ± 0.833-fold) and protein levels (181.8 ± 2.754 pg/ml) in 3T3-L1 mouse adipocytes. Similar synergistic effects were observed in primary mouse (Il6 mRNA; 1.364 ± 0.287-…

Microbiota-miR-101 interactions in obesity-associated colorectal cancer: from barrier dysfunction to precision therapeutic strategies.

Colorectal cancer (CRC) remains a leading cause of cancer-related morbidity and mortality worldwide, with obesity recognized as a major modifiable risk factor. Obesity-associated CRC is characterized by systemic low-grade inflammation, altered lipid metabolism, and gut microbial dysbiosis, all of which converge to create a pro-inflammatory niche. Emerging evidence implicates murine miR-101a/b, an ortholog of the human miR-101 family, as a key molecular mediator linking metabolic dysfunction, promoting inflammation, endotoxemia, and affecting epithelial homeostasis. Traditionally, the miR-101 f…

Crosstalk between visceral adipose tissue secretome and tumoral microenvironment in obesity-induced colorectal cancer.

BACKGROUND: Currently, colorectal cancer (CRC) remains a major global health challenge, with a rising incidence linked to a sedentary lifest … cially for lipopolysaccharides (LPS), toxins, or short-chain fatty acids (SCFAs), trigger metabolic endotoxemia, further activating inflammatory pathways within VAT. These systemic changes remodel the colorectal TME by stimulating pathological angiogenesis, inducing epithelial-mesenchymal transition (EMT), and creating a permissive niche for immune evasion th …

The calcium-sensing receptor in sepsis and septic shock, mechanistic pathways and translational perspectives: a systematic review.

BACKGROUND: Sepsis and septic shock are major causes of mortality in critically ill patients. They are linked to widespread metabolic and im … mage, as well as vasoplegia related to endothelial dysfunction. Preclinical models of pneumonia and endotoxemia suggest that CaSR antagonists may effectively mitigate inflammation and organ injury. CONCLUSION: This systematic review identifies the CaSR as an amplifier of the host inflammatory response across both septic and non-septic preclinical models The …

Temporary Portocaval Shunts During Liver Transplantation: A Narrative Review of Technical Solutions and Post-Transplant Outcomes.

Background: Liver transplantation (LT) continues to evolve with techniques aimed at minimizing perioperative complications associated with caval and portal vein clamping. Caval-sparing approaches, such as the piggyback technique, preserve hemodynamic stability; however, portal clamping remains necessary and may trigger postreperfusion syndrome, endotoxemia, and hepatic microcirculatory disturbances. Temporary portocaval shunts (PCSs) have been developed to maintain portal flow during LT, mitigating these adverse effects and allowing for hemodynamic stability and a reduced intraoperative bleedi…
