Exercise performance and quality of life following surgical repair of anomalous aortic origin of a coronary artery in the pediatric population.

OBJECTIVE: We sought to evaluate exercise performance and quality of life in children after surgical repair of anomalous aortic origin of a coronary artery with an interarterial course. METHODS: Patients who had surgery from October 2001 to January 2007 were eligible for inclusion. Exercise performance and quality of life were prospectively assessed by maximal exercise tests and age-appropriate questionnaires, respectively. We used t tests to compare pre- and postoperative exercise data and quality-of-life scores to published normative data. We performed linear regression analyses to assess a

Exercise-induced hyperkalemia and concentration of Na,K-pumps in skeletal muscle in mitral stenosis: effect of balloon mitral valvotomy.

BACKGROUND AND AIMS OF THE STUDY: The study aim was to examine the effects of balloon mitral valvotomy (BMV) on exercise-induced hyperkalemia, and on changes in the concentration of Na,K-pumps in skeletal muscle, as an exaggerated exercise-induced rise in potassium concentration ([K+]) may contribute to exertional fatigue and breathlessness. METHODS: Eight subjects were evaluated with mitral stenosis (mean age 34 +/- 5.2 years) before, and at two weeks and four months after BMV. Subjects underwent incremental exercise to exhaustion for exercise-induced rise in [K+] and vastus lateralis biopsy

A Single Bout of Aerobic Exercise Increases Neuronal Extracellular Vesicle-Derived Insulin Signaling Biomarkers in Adults With Cardiometabolic Risk.

UNLABELLED: Exercise may lower Alzheimer's Disease and Related Dementia (ADRD) risk. While insulin has been proposed to benefit cognition, the effect of exercise on neuronal insulin signaling in humans is unclear. PURPOSE: We tested the hypothesis that a single bout of aerobic exercise would raise insulin signaling mediators from plasma-derived neuronal extracellular vesicles (nEVs). METHODS: Fifteen sedentary adults with obesity (12F; ~56y; ~31 kg/m2) completed an evening rest and acute exercise condition (70% maximal oxygen consumption (VO2max)) in a randomized, counterbalanced order. Foll

Pulmonary hypertension and home-based (PHAHB) exercise intervention: protocol for a feasibility study.

INTRODUCTION: Novel therapies for pulmonary hypertension (PH) have improved survival and slowed disease progression. However, patients still present with symptoms of exertional dyspnoea and fatigue, which impacts their ability to perform activities of daily living, reduces exercise tolerance and impairs their quality of life (QoL). Exercise training has shown to be safe and effective at enhancing QoL and physical function in PH patients, yet it remains an underused adjunct therapy. Most exercise training for PH patients has been offered through hospital-based programmes. Home-based exercise pr…

Exaggerated blood pressure response to dynamic exercise despite chronic refractory hypotension: results of a human case study.

BACKGROUND: Chronic refractory hypotension is a rare but significant mortality risk in renal failure patients. Such aberrant physiology usually deems patient unfit for renal transplant surgery. Exercise stimulates the mechano-chemoreceptors in the skeletal muscle thereby modulating the sympathetic effects on blood pressure regulation. The haemodynamic response to dynamic exercise in such patients has not been previously investigated. We present a case with severe chronic hypotension who underwent exercise testing before and after renal transplantation, with marked differences in blood pressure

Pulmonary rehabilitation and inspiratory muscle training in post-tuberculosis lung disease: A randomized controlled trial assessing functional capacity, respiratory muscle strength, and quality of life.

BACKGROUND AND OBJECTIVES: Even after microbiological cure is achieved, a considerable proportion of patients treated for tuberculosis continue to experience lasting respiratory impairment - a condition now termed post-tuberculosis lung disease (PTLD). This randomized controlled trial (RCT) tested whether a structured pulmonary rehabilitation (PR) program, augmented with inspiratory muscle training (IMT), could improve exercise tolerance, inspiratory muscle strength, and health-related quality of life (HRQoL) in adults with confirmed PTLD, relative to standard care. METHODS: We enrolled 120 a…

Determinants of submaximal exercise intolerance in patients with heart failure and preserved ejection fraction: Insights from the lactate threshold.

AIMS: Oxygen consumption at peak exercise is widely used to assess functional impairment in heart failure with preserved ejection fraction (HFpEF), but few patients exercise to this intensity in daily living. Alternative metrics that quantify submaximal fitness may provide more patient-centred evaluations, but the pathophysiology of submaximal exercise intolerance in HFpEF is unexplored. METHODS AND RESULTS: Patients with HFpEF underwent invasive haemodynamic cardiopulmonary exercise testing with blood lactate measurement during exercise to volitional fatigue. Lactate threshold (LT) was defin

Exceptional exercise capacity in a late bloomer octogenarian triathlete.

Maximal oxygen uptake (V̇O<inf>2max</inf>) gradually declines across the lifespan. However, little is known about the octogenarians. This is a knowledge gap given that individuals living >80 years old is the fastest expanding age-group world-wide. The increase in the elderly population has generated greater awareness in healthy ageing including psychosocial, physical, and cognitive domains, all of which benefit from engagement in physical activity or sport. In the present article are reported characteristics of an octogenarian man (GM), starting training for triathlon when he was 75 years old.
