Minimally invasive treatment of thoracolumbar flexion-distraction fracture.

INTRODUCTION: Flexion-distraction fractures represent around 15% of all thoracolumbar fractures, with neurological deficit in 25% of cases. Optimal surgical strategy remains controversial. In neurologically intact patients, percutaneous fixation can offer quick stabilization with good deformity correction. If necessary, an additional minimally invasive anterior approach can complete the surgical strategy. We report results in a series of 28 thoracolumbar flexion-distraction fractures without neurologic deficit, treated using a minimally invasive approach. METHOD: A single-center retrospective

Early healing with locked condylar plating of periprosthetic fractures around the knee.

Management of periprosthetic fractures around the knee is often difficult because of poor bone quality, comminution, and constraints imposed by the existing prosthesis. Locked condylar plates may provide more reliable fixation for these fractures than traditional methods of fixation. Eleven patients with periprosthetic fractures around the knee were treated with open reduction and internal fixation using a locked condylar plate. All 9 acute fractures and 1 of 2 periprosthetic nonunions healed at an average of 21 weeks, and no patient required additional surgery. Nine fractures healed in anatom

"Snowboarder's fracture": fracture of the lateral process of the talus.

BACKGROUND: As physicians caring for patients who sustain snowboarding ankle injuries, we have noted that fracture of the lateral process of the talus occurs frequently. The incidence of this fracture, however, is very low in other accounts of ankle injuries. We report a case of fracture of the lateral process of the talus and review the pertinent literature of this injury. METHODS: A literature review was performed using MEDLINE files from 1966 to the present. We found additional references from the bibliographies of available reports. RESULTS AND CONCLUSIONS: Physicians caring for snowboar

A Case of Closed Pilon Fracture Resulting in Soft-Tissue Necrosis and Treated with Reverse Sural Artery Flap and Circular External Fixation.

Tibial pilon fractures are difficult to treat due to articular comminution and soft-tissue injuries caused by high-energy trauma. Open reduction and internal fixation is a commonly used method of treatment. However, it has a high risk of infection and soft-tissue complications due to the extensive detachment of soft tissue. We report on a case with a tibial pilon fracture and soft-tissue necrosis that we treated using limited internal fixation combined with a circular external fixator (LIFCEF) and reverse sural artery flap (RSAF) as part of an orthoplastic approach within the orthopedic surger

Triceps-reflecting anconeus pedicle approach with double precontoured locking plate fixation is efficient in the treatment of orthopaedic trauma association type C distal humerus fracture.

BACKGROUND: The purpose of this study was to determine if the triceps-reflecting anconeus pedicle (TRAP) approach combined with the precontoured locking plate can provide acceptable clinical outcomes in the treatment of comminuted distal humerus fracture. METHODS: Between August 2008 and June 2014, 48 patients with Orthopaedic Trauma Association type C (21 with C2, 27 with C3) distal humerus fractures underwent open reduction and internal fixation with orthogonal precontoured locking plate fixation using the TRAP approach. Occurrences of bony union, implant failure, posttraumatic arthrosis, a

[Treatment of talus neck fracture with mini-plate internal fixation through dual-incision approaches].

OBJECTIVE: To explore clinical outcomes of talus neck fracture treated with mini-plate internal fixation through dual-incision approaches. METHODS: From August 2010 to February 2013,18 patients with closed talus neck fractures were treated (10 males and 8 females, aged from 31 to 66 years old with an average of 38.2 years old) with mini-plate internal fixation through dual-incision approaches. According to Hawkins classification, 12 cases were type II and 6 cases were type III. All cases were evaluated with X-ray and 3D CT scan preoperatively to define type and comminuted degree of fractures.

The effect of a coronoid prosthesis on restoring stability to the coronoid-deficient elbow: a biomechanical study.

PURPOSE: The coronoid process has been recognized as a critical component in maintaining elbow stability. In the case of comminuted coronoid fractures, where repair is not possible or has failed, a prosthesis may be beneficial in restoring the osseous integrity of the elbow joint. The hypothesis of this in vitro biomechanical study was that a coronoid prosthesis would restore stability to the coronoid-deficient elbow. METHODS: An anatomically shaped metallic coronoid prosthesis was designed and developed based on computed tomography-derived measurements and optimized to account for average ca

Minimally Invasive Plate Osteosynthesis (MIPO) technique for complex tibial shaft fracture.

TTo evaluate the clinical and radiological results of the treatment of complex tibial shaft fracture (AO/OTA type 42-C) with minimally invasive plate osteosynthesis(MIPO). Twenty patients diagnosed with complex tibial shaft fracture without extension to the articular surface and treated with MIPO, including 9 cases of AO/OTA type 42-C2 and 11 cases of AO/OTA type 42-C3, 6 of which were open fractures. External fixation was used for open fractures until the soft tissue damage had healed; then, 2nd stage operation with MIPO was performed to stabilize the fracture. Each patient was followed up fo

Locked intramedullary nailing of humeral shaft fractures. Implant design, surgical technique, and clinical results.

We report our experience with a modified implant and a new technique for locked intramedullary nailing of the humerus in 41 patients. Locking was by cross-screws placed from lateral to medial in the proximal humerus, and anteroposteriorly in the distal humerus. Early in the series, 11 nails were inserted at the shoulder, but we found that rehabilitation was faster after retrograde nailing through the olecranon fossa, which was used for the other 30. We used a closed technique for 29 of the nailings. Of the 41 patients treated, 21 had acute fractures, five had nonunion, and 15 had pathological

Lateral Humeral Condyle Fractures in Pediatric Patients.

Lateral humeral condyle fractures are frequently seen in pediatric patients and have a high risk of unfavorable outcomes. A fall on the outstretched arm with supination of the forearm is the most common trauma mechanism. A physical examination combined with additional imaging will confirm the diagnosis. Several classifications have been described to categorize these fractures based on location and comminution. Treatment options depend on the severity of the fracture and consist of immobilization in a cast, closed reduction with percutaneous fixation, and open reduction with fixation. These fra

An Atypical Presentation of Terrible Triad Injury in a Healthy 51-Year-Old Female Patient.

A left elbow fracture-dislocation in a 51-year-old female patient following a backward fall at home presented with a comminuted radial head fracture, coronoid fracture, and ruptured collateral ligaments. The patient underwent open reduction and internal fixation (ORIF) with a long-arm splint. Post surgery, aggressive rehabilitation was initiated to restore the range of motion (ROM). Compared to previous reports, this case demonstrates successful management of a severe elbow injury with excellent short-term outcomes. This case highlights the occurrence of the terrible triad in a patient without

Current Management of Intra-Articular Calcaneal Fractures.

Intra-articular calcaneal fractures are debilitating injuries that can result in a dramatic effect on quality of life post-injury. Surgical intervention is generally advised when significant displacement or comminution is present but can present a host of complications secondary to the limited blood supply and fragility to the soft tissues following injury. Surgical approaches to calcaneal fractures generally include the lateral extensile approach, minimal incision (sinus tarsi approach), or percutaneous approach. Each approach presents risks and benefits; therefore, determining the optimal in

A Novel Surgical Technique for Glenoid Reconstruction in the Trauma Setting With a Distal Tibia Allograft: A Case Report.

CASE: A 21-year-old woman presented with a chronic fracture dislocation of her left shoulder after striking a submerged tree during a watersport activity. She was initially treated at an outside hospital, where she underwent open reduction and internal fixation of her posterior scapula. She presented 8 weeks after index procedure with pain and severely decreased range of motion. Her comminuted and displaced fracture comprising 70% of the anterior superior glenoid was surgically reconstructed using a distal tibia allograft (DTA), resulting in a functional shoulder. CONCLUSION: A DTA is a feasi

Subacute Minimally Invasive Decompression of L5 and S1 Nerve Roots for Neurologic Deficit After Fixation of Unstable Pelvic Fracture: A Case Report and Review of the Literature.

CASE: A 73-year-old man experienced immediate neurological decline after percutaneous transsacral screw fixation for a pelvic ring injury sustained after a 25-foot fall. Workup revealed well-positioned screws and compression of the right L5 and S1 nerve roots at the fracture site. Symptoms improved after direct decompression without screw revision. CONCLUSION: The courses of the L5 and S1 nerve roots place them at risk of compression within the fracture during transsacral screw fixation. In highly comminuted fractures, avoidance of compression screws or use of intraoperative CT might prevent

Coronal shear fractures of the distal humerus: the capitellum and trochlea.

Fractures of the capitellum and trochlea are uncommon and multiple options have been advocated for the treatment of this injury. A single management technique has not emerged as the superior technique, and a complement of interventions is necessary to manage the continuum of injuries that can be observed. In general, open reduction and internal fixation is advocated for healthy and active patients with satisfactory bone quality to allow for the insertion of stable fixation. In the geriatric population, total elbow arthroplasty may emerge as the treatment of choice particularly for the more com

The influence of the mode of treatment of zygomatic bone fractures on the healing process of the infraorbital nerve.

Several authors have described the influence of the method of treatment of a fracture of the zygomatic complex on the recovery of the infraorbital nerve. In this study, the results of treatment of 106 patients with an isolated, non-comminuted, unstable fracture of the zygomatic bone are presented. Thirty-eight patients underwent fixation with interosseous wiring and 68 patients were treated with a miniplate osteosynthesis across the frontozygomatic suture. In the group with wire fixation, 50% suffered persistent reduced sensitivity in the infraorbital region at follow-up examination, whereas i
