[Management of stridor].

Pediatric stridor is usually due to benign laryngomalacia. This pathology of unknown origin usually heals spontaneously during the first 18 months of life. Mild laryngomalacia is characterized by normal weight growth, the absence of sleep apneas and of associated swallowing or neurological disorders. Medical treatment is sufficient to release symptoms. Severe clinical forms are more rare, representing about 10 % of cases. They require an endoscopic evaluation of the upper airway under general anesthesia, as well as an endoscopic suppraglottoplasty. Non-invasive ventilation is an interesting ad

Giardia: both a harmless commensal and a devastating pathogen.

The highly prevalent protozoan Giardia lamblia is an enteropathogen that can be asymptomatic in some individuals, while leading to persistent diarrhea and substantial morbidity in others. In this issue of the JCI, Bartelt et al. describe a mouse model of the disease and investigate the contribution of coincident malnutrition with the development of symptomatic infection. This work in part explains how Giardia infection can lead to growth retardation, and may offer insights that guide future therapeutic strategies.

Growth of prepubertal children on dialysis.

Growth failure is a common and significant clinical problem for children on dialysis and often remains a major impediment to their rehabilitation. Early referral to a paediatric nephrology centre and appropriate management before the initiation of dialysis may significantly prevent growth deterioration. Growth in children on dialysis can be affected by nutritional, metabolic, and hormonal changes. Early diagnosis of malnutrition and aggressive management should be a priority. Gastrostomy feeding should be used when adequate oral intake to maintain normal height and weight velocity cannot be ac

A child with coexistent hemihypotrophy and parapontine epidermoid.

This report describes a 10-year-old female who presented with poor growth and right hemihypotrophy. She had crossed sensory findings on neurologic examination consistent with a lateral medullary syndrome with no discernible cranial nerve findings, ataxia, or vertigo. Magnetic resonance imaging of the brainstem revealed a large right parapontine epidermoid. The possible relationship between the hemihypotrophy and the epidermoid is discussed.

[A case of growth retardation of rare occurrence].

The Authors describe an uncommon case of growth retardation; the clinical and radiological elements that the Authors to a diagnosis of spondyloepiphyseal dysplasia (SED) are noted; they compare this to other diagnosis of different types of growth retardation, they underline the necessity of taking these rare types into consideration before undergoing diagnostic procedures that would be costly both to the patient and society.

Endocrine aspects of the 22q11.2 deletion syndrome.

Hormonal disorders are common in patients with a 22q11.2 deletion. While hypoparathyroidism was the first endocrine disturbance documented in the DiGeorge syndrome, growth hormone deficiency, hypothyroidism, and hyperthyroidism are now known to occur in patients with a 22q11.2 deletion. This review briefly summarizes our current understanding of the spectrum of endocrinological manifestations of the 22q11.2 deletion and proposes guidelines for appropriate screening and management of endocrine disorders in patients with a 22q11.2 deletion.

From Preconception to Postnatal: Parental Risk Factors for Fetal-Infant Growth Faltering in LMICs-a Scoping Review (2020-2025).

PURPOSE: This review aimed to map the association between parental (both maternal and paternal) risk factors, from preconception through the … 17 eligible studies for this review. The findings showed that the determinants of fetal and infant growth disorders are multifaceted, falling into four key areas. These included individual factors including maternal age, education, nutritional status, health condition, reproductive history, mental health and psychological, marital and socioeconomic, occupation, su …

Preadipocyte factor-1 in maternal, umbilical cord serum and breast milk: The impact of fetal growth.

BACKGROUND/OBJECTIVE: To study the concentrations of preadipocyte factor-1 (Pref-1) -an inhibitor of adipocyte differentiation, implicated in adipose tissue metabolism, late metabolic disorders and fetal growth- in maternal and umbilical cord serum, as well as maternal milk and correlate above concentrations with intrauterine growth and other perinatal parameters. MATERIAL AND METHODS: Pref-1 concentrations were determined by ELISA in antepartum maternal and umbilical cord serum, as well as day 3 to 4 postpartum breast milk, deriving from 80 women, who delivered 40 appropriate (AGA), 20 large

Pretreatment Endocrine Disorders Due to Optic Pathway Gliomas in Pediatric Neurofibromatosis Type 1: Multicenter Study.

CONTEXT: Up to 20% of children with neurofibromatosis type 1 (NF1) develop low-grade optic pathway gliomas (OPGs) that can result in endocrine dysfunction. Data on prevalence and type of endocrine disorders in NF1-related OPGs are scarce. OBJECTIVES: The aim of the study was to determine the prevalence of endocrine dysfunctions in patients with NF1 and OPGs and to investigate predictive factors before oncological treatment. DESIGN: Multicenter retrospective study. SETTINGS AND PATIENTS: Records were reviewed for 116 children (64 females, 52 males) with NF1 and OPGs followed at 4 Italian cen

The Myelodysplastic Syndromes.

The myelodysplastic syndromes (MDS) are a heterogeneous group of disorders characterized by peripheral blood cytopenias with a hypercellular bone marrow exhibiting dyspoiesis. The MDS range from those with a relatively indolent course (e.g., refractory anemia with or without ringed sideroblasts) to more aggressive disorders (e.g., refractory anemia with excess blasts [RAEB], and RAEB in transformation [RAEB-T]), which may exhibit a clinical course indistinguishable from acute myeloid leukemia (AML). Supportive care is the standard treatment for most patients, particularly those who are elderly

Growth hormone (GH) dose-dependent IGF-I response relates to pubertal height gain.

BACKGROUND: Responsiveness to GH treatment can be estimated by both growth and ∆IGF-I. The primary aim of the present study was to investigate if mimicking the physiological increase during puberty in GH secretion, by using a higher GH dose could lead to pubertal IGFs in short children with low GH secretion. The secondary aim was to explore the relationship between IGF-I, IGFBP-3 and the IGF-I/IGFBP-3 ratio and gain in height. METHODS: A multicentre, randomized, clinical trial (TRN88-177) in 104 children (90 boys), who had received GH 33 μg/kg/day during at least 1 prepubertal year. They were
