Metacarpal synostosis: treatment with a longitudinal osteotomy and bone graft substitute interposition.

PURPOSE: To describe a case series of congenital metacarpal synostosis treated with longitudinal osteotomy and bone graft substitute interposition. METHODS: We retrospectively reviewed charts of all patients with metacarpal synostosis treated with a longitudinal osteotomy and bone graft substitute interposition at 2 institutions. Radiographic and clinical appearances were analyzed at initial diagnosis, intraoperatively, and at last follow-up. RESULTS: A total of 10 patients (14 hands) met the inclusion criteria. Six patients (8 hands) demonstrated ring-little finger metacarpal synostosis and

Finger Rescue Using the Induced Membrane Technique for Osteomyelitis of the Hand.

Background: The induced membrane technique is now commonly used for large diaphyseal bone defects. Recently, several papers reported using the induced membrane technique for hand surgery. We applied this technique with some modifications to treat osteomyelitis of the phalanges. Methods: This study included six men and one woman with a mean age of 56 years. The causes of osteomyelitis included animal bite (n = 3), trauma (n = 3), and an indwelling needle (n = 1). Two-staged surgeries were performed, including an initial stage with radical debridement of the infected tissue and placement of a ce

Methods of skeletal age assessment using hand and wrist radiographs in children and their applicability across different ethnicities: systematic review and meta-analysis protocol.

BACKGROUND: Skeletal age assessment using hand and wrist X-rays is commonly used to determine the biological maturity of children. It is invaluable when the chronological age is not known or in diagnosing and treating various endocrine, genetic, and developmental disorders as well as in a forensic context. Skeletal age is affected by both internal and external factors, leading to differences in skeletal maturity between children of different ethnicities. Regional studies have even questioned the applicability of current international skeletal age references across ethnicities. METHODS: A comp

Chat Generative Pretrained Transformer-4.0's accuracy in assessing cervical vertebrae and hand-wrist maturation stages: A retrospective study.

INTRODUCTION: This study aimed to evaluate the diagnostic accuracy of Chat Generative Pretrained Transformer version 4.0 (ChatGPT-4.0) in determining cervical vertebrae and hand-wrist maturation stages using cephalometric and hand-wrist radiographic films. METHODS: A retrospective analysis was conducted on 238 subjects who had cephalometric and hand-wrist radiographs taken on the same day. Each hand-wrist maturation stage was independently evaluated by 3 orthodontists using the method described by Björk and Helm, whereas cervical vertebrae maturation stages were assessed following the methodo

Imaging Traumatic Hand and Finger Injuries.

The hands and fingers play a vital role in everyday activities, which increases the risk for accidental injury. Imaging these potentially complex injuries is vital to successful diagnosis and treatment. Imaging techniques and best practices vary; multiple modalities might be needed to achieve an accurate diagnosis. This article discusses anatomy and common injuries of the hand and fingers. Methods for successful imaging also are described. A broad understanding of anatomy and imaging techniques enables radiologic technologists to better assist with hand trauma management.

[Peculiarities of wrist bone growth as a criterion for determining the biological age of children living under different environmental conditions].

The paper had the objective to study growth dynamics of short tubular wrist bones in children aged from 5 to 16 years and residing in the cities of Shelekhov and Irkutsk. The growth rate in the girls from Irkutsk was shown to be fairly uniform and proportional; in the boys, it substantially increased at the age of 10-11 and 15 years. In the boys living in Shelekhov, the bone growth became activated at the age of 9 and 15 years. In the girls, it was periodically uniform until the age of 9; thereafter, periodicity in the growth of middle and distal phalanxes was no longer apparent.

Ulnar dimelia.

Ulnar dimelia ("mirror hand") is a rare congenital anomaly of the upper extremity characterized by duplication of the ulna, absence of the radial ray, and polydactyly. A case report is presented in which the patient had seven triphalangeal digits arranged in a radial cluster of three and an ulnar cluster of four. Emphasis is placed on the technique of one-stage thumb reconstruction with the use of the principles described by Buck-Gramcko.

Oncocytic carcinoma arising in the submandibular gland with disseminated bone metastases.

Oncocytic carcinoma of the head and neck is a very rare neoplasm. It usually occurs in the parotid glands. Only 11 cases of oncocytic carcinoma of the submandibular gland have been reported, and no cases have shown distant bone metastasis. A 67-year-old man presented with a tingling sensation in both hands due to a herniated cervical disc. A whole body bone scan and PET-CT showed disseminated bone metastases. Neck CT revealed a 1.7 cm calcified left submandibular mass. The submandibular gland and bone marrow biopsies were consistent with oncocytic carcinoma. Our case is the first report of onc

BHLHA9 homozygous duplication in a consanguineous family: A challenge for genetic counseling.

Split-hand/foot malformation (SHFM) with long-bone deficiency (SHFLD) is a rare condition characterized by SHFM associated with long-bone malformation usually involving the tibia. It includes three different types; SHFLD1 (MIM % 119,100), SHFLD2 (MIM % 610,685) and SHFLD3 (MIM # 612576). The latter was shown to be the most commonly reported with a duplication in the 17p13.1p13.3 locus that was narrowed down to the BHLHA9 gene. Here, we report a consanguineous Lebanese family with three members presenting with limb abnormalities including tibial hemimelia. One of these patients presented with a

Vitamin K-Dependent Proteins in Skeletal Development and Disease.

Vitamin K and Vitamin K-dependent proteins (VKDPs) are best known for their pivotal role in blood coagulation. Of the 14 VKPDs identified in humans to date, 6 play also important roles in skeletal biology and disease. Thus, osteocalcin, also termed bone Gla-protein, is the most abundant non-collagenous protein in bone. Matrix Gla protein and Ucma/GRP on the other hand are highly abundant in cartilage. Furthermore, periostin, protein S, and growth arrest specific 6 protein (GAS 6) are expressed in skeletal tissues. The roles for these VKDPs are diverse but include the control of calcification a

Reconstruction of hand injuries with multiple metacarpal defects using free fibular osteoseptocutaneous flap.

Hand injuries with multiple metacarpal involvements often include midpalmar muscle, extensor tendon, and skin defects. Reconstruction method is decided according to the type and amount of structures to be restored. Bone reconstruction and resurfacing of the skin is regarded as priority, and restoration of tendon function and joint mobility can be left for further procedures. An ideal flap for such defects should provide bone for multiple metacarpal defects and a large enough skin paddle. Such flaps are few, and one of the most suitable of them all is the free fibular osteoseptocutaneous flap (

Flexor tendon ruptures in patients with rheumatoid arthritis.

One hundred fifteen flexor tendon ruptures were reviewed in 43 hands with rheumatoid arthritis, one hand with psoriatic arthritis, and one hand with lupus erythematosis. Ninety-one tendons were ruptured at the wrist, four ruptures occurred at the palm, and 20 ruptures occurred within the digits. At the wrist level, 61 ruptures were caused by attrition on a bone spur and 30 were caused by direct invasion of the tendon by tenosynovium. All ruptures distal to the wrist were caused by invasion of the tendon by tenosynovium. Patients whose ruptures were caused by attrition regained better motion th

Evanescent extraosseous calcifications in low turnover bone: management and outcomes: a case report.

INTRODUCTION: The prevalence of bone disease in peritoneal dialysis patients has been recently shown to exceed 54%, including patients with parathormone (PTH) levels within the theoretical adequate target, yet demonstrating low bone turnover on histomorphometry. Moreover, bone disease is often associated with abnormalities in calcium and phosphate metabolism, leading to tissular deposits such as extraosseous calcifications. CASE PRESENTATION: We present a 22-year-old female patient managed on peritoneal dialysis with persistent swelling of all four extremities, including the fingers, hands, a

SPECT/CT in Postoperative Hand and Wrist Pain.

In this review we summarize the current evidence and experience regarding the performance of SPECT/CT in the evaluation of patients with postoperative painful wrist or hand. There is a wide range of operative wrist and hand interventions due to congenital, traumatic, degenerative, or inflammatory diseases. A significant number of patients suffer from pain after operative procedures. Several imaging modalities have been used to evaluate the reason for painful postoperative wrists like standard conventional x-rays, ultrasound, CT, MRI, or bone scintigraphy. In the last decade, bone SPECT/CT has

[The hand: a reflection of arthritis].

Hand joint involvement in rheumatic diseases is often precocious and predominant as compared to other skeleton-muscular regions. Clinical examination not always allows for easy detection of fluid outpouring and synovial involvement, and undoubtedly does not allow to diagnose pathognomonic bone alterations of several rheumatic conditions. Articular ultrasonography is an innocuous methodology, easily reproducible and directly applied by a rheumatologist. The aim of this vignette is to present ultrasonography elementary lesions of the hand for a prompt diagnosis.

Changes in bone mass and bone turnover following distal forearm fracture.

Bone loss occurs close to a fracture and is associated with increased bone turnover. Fracture healing itself results in increased markers of bone turnover. But the exact patterns of these changes after different fractures are unclear. We aimed to investigate the changes in bone density and biochemical markers following distal forearm fracture. Twenty women (mean age 63 years) were recruited following fracture of the distal radius and ulna. Bone mineral density (BMD) of the hand and forearm were measured by dual-energy X-ray absorptiometry (DXA) and quantitative ultrasound (QUS) of the fingers
