Between 1950 and 1991 the authors have treated 45 adults with primary septic arthritis of the hip, not including tuberculosis. The mortality rate was 13%. There is an accompanying septicaemia, or, more commonly, a bacteraemia, and other general and local factors are often present. The diagnosis may be obvious, but in 50% of cases the presentation may be atypical and the clinical diagnosis uncertain. Hip synovitis plays a part in many rheumatic diseases. In the young adult acute arthritis may be due to reaction to trauma, infection or gout. Although transient synovitis of the hip is a well-known phenomena in children the condition is not well-documented in adults. We present 10 young adults with idiopathic transient synovitis of hip who had attended the Soroka Medical Centre between 1986 and 1990.

The authors report the data relative to 50 preterm infants with gestational age less than or equal to 34 weeks. The newborns were studied sequentially with clinical and US examination from February 28, 1989 to March 23, 1990 in order to exclude hip dysplasia or dislocation. US examination was carried out according to Graf's technique. All the newborns had another clinical examination at one year of age in order to evaluate their normal walking. Risk factors had poor significance in the studied group of infants.

Three cases of ganglia related to the hip containing gas are presented. Gas may be shown by conventional radiography, linear tomography or computed tomography. All three cases had advanced degenerative disease of the joint. In degenerative disease, ganglia form through a pathway which allows the transfer of intra-articular gas into the ganglion. It is important to differentiate this phenomenon from overlying bowel gas and other causes of gas within soft tissues. It is the most specific radiological feature of ganglia around the hip.

The authors examined the association of hip osteoarthritis with the presence of Heberden's nodes in a case-control study. Subjects were men aged 60-75 years who had undergone intravenous urography as out-patients. The relation between acetabular dysplasia and osteoarthritis of the hip was examined in a series of 1516 pelvic radiographs taken for non-skeletal indications. Osteoarthritis was assessed by measuring joint space, and dysplasia by the centre-edge angle and acetabular depth. In contrast with previous studies of patients with symptomatic osteoarthritis of the hip, no evidence that dysplasia predisposes to osteoarthritis was found. We report our use of linear sonography in evaluating painful and irritable hips. Images were correlated with the clinical course and outcome, and other methods of diagnostic imaging. Sonography demonstrated the anatomical structures of the hip, the joint space and capsule. A significant increase of joint space due to the progressive development of intra-articular fluid was found in patients with transient synovitis, septic arthritis and osteomyelitis of the femoral neck. The long-term outcome of treatment of posterior hip dislocations was studied in 60 patients. Thirty-two patients had concomitant fractures of the acetabular rim and seven had also femoral head fractures. Treatment consisted of immediate reduction, followed by gentle traction during one week and progressive weight-bearing over six weeks. The authors report on their experience with 2164 US exams of the hip performed over last year on newborns, according to Graf's technique. The babies were divided into 2 groups at first examination: risk/no risk. We considered as risk factors: a positive family history, malposition in utero, abnormalities at birth, and being one of twins. Over the period of one year, the hips of 92 preterm neonates were examined by ultrasound. Using the Graf classification only 7% showed an angle alpha between 50 and 60 degrees, which is characteristic of type IIa hips. In all other cases the angle alpha was above 60 degrees (type I). Sonographically there were no pathological cases (type IIg or worse). A reason for the relatively low number of type IIa hips could be that the short osseous acetabular rim and the broad cartilagenous Y-joint in this age group result in a "false" increase of the angle alpha.

The hereditary pattern of CDH is well known, but the results depend on time of diagnosis, either late or in the immediate postnatal period, the latter having been the basis of the more recent studies. Early screening shows that newborns with an unstable hip are 4 times more frequent than those with late CDH. The etiology includes constitutional and environment factors.
