Percutaneous biliary interventions are indicated for both benign or malignant biliary strictures to treat obstructive jaundice and/or biliary leak. Major vascular complications include active bleeding or more frequently pseudoaneurysm formation, bilio-portal, bilio-venous or arterio-biliary fistulae and intra- or peri-hepatic hematoma. Such adverse events can occur during the procedure itself or, more often, several days to weeks after the initial procedure. Major vascular complications frequently manifest with symptoms of hemobilia (jaundice, pain, melena or hemochezia and reduced serum hemog

BACKGROUND: Choledochal cysts are divided into 5 types. Physicians believe that Caroli disease (which refers to type V biliary cysts) is a special type of biliary cyst caused by a mutation in the PKHD1 gene and is associated with autosomal recessive polycystic kidney disease (ARPKD). There is currently no clear association between other types of choledochal cysts and polycystic kidney disease.
CASE PRESENTATION: We report a 65-year-old male patient with jaundice, decreased appetite, and itchy skin. His biochemistry test results indicated obstructive jaundice disease. Cross-sectional imaging s

BACKGROUND: The cystic dilatation of the biliary tract is a rare disease and uncertain origin. It is recognized more frequently in children; however, its incidence comes increasing in adults, representing 20% of the cases.
AIM: To evaluate morbimortality rates, evolution and handing of patients with cystic dilatation bile ducts in adults.
METHODS: Were evaluated, retrospectively, five adults who had the diagnosis of choledochal cyst and that had been submitted to some surgical procedure.
RESULTS: Abdominal pain was the commonest complain to all patients. Jaundice was present in 80%. Ultraso

Differential diagnosis between autoimmune pancreatitis (AIP) and pancreatic cancer can be very difficult. The main clinical symptoms in patients with autoimmune pancreatitis are jaundice, weight loss, abdominal pain and new onset of diabetes mellitus. Unfortunately, the same symptoms could be observed in patients with pancreatic carcinoma too. Imaging methods as computed tomography (CT) scan, magnetic resonance imaging (MRI) and endosonography (EUS); together with serological examination (IgG4 and Ca 19-9) play the important role in differentiation autoimmune pancreatitis from pancreatic cance

CONTEXT: Gastrointestinal (GI) involvement is present in about one quarter of cases of neurofibromatosis type 1 (NF1). Adenocarcinomas have been reported in several organs. Gastrointestinal stromal tumors are the most common GI lesion seen in NFI. GISTs in combination with ampullary neuroendocrine tumors in NF-1 have been reported rarely.
CASE REPORT: We present the case of a 44-year-old man who presented with a history of obstructive jaundice and weight loss. Investigations revealed a pancreatic tumor associated with a common bile duct (CBD) stricture. At operation, an ampullary adenocarcino

The mucin-hypersecreting biliary papillomatosis is a premalignant neoplasm characterized by intraductal papillary proliferation involving extensive areas of the intrahepatic and/or extrahepatic bile duct. We report a case of mucin-hypersecreting biliary papillomatosis manifested as obstructive jaundice and diagnosed only by microscopy, with a review of literatures. A 74-year-old female, who had a past history of cholecystectomy about 13 years ago, was admitted to our hospital with jaundice. A CT scan showed marked dilatation of intrahepatic and extrahepatic bile duct without intraductal fillin

Septic complications have been major problems in the management of patients with obstructive jaundice and neonatal jaundice. This study investigates effects bilirubin on human T lymphocyte responses against allogeneic mixed lymphocyte reaction. In vitro exposure of human peripheral blood mononuclear cells (PBMNC) with unconjugated bilirubin at pathological levels (6 to 12 mg/dl) did not alter the subsets of CD3, CD4, CD8, CD14, CD19 and CD56 positive populations, or expression of costimulatory surface molecules CD2, CD3, CD4 and CD8. Further incubation of bilirubin-treated PBMNC with irradiate

Occurrence of fever in a patient with liver cirrhosis should suggest the following: 1. Endotoxemia. Endotoxins are normally present in portal blood; in hepatic cirrhosis they are insufficiently cleared by the liver and their presence can be demonstrated in the systemic circulation by the "limulus test". Fever is one of the many consequences ascribed to the presence of endotoxins in the blood. 2. Infections. Cirrhosis and alcoholism (which often accompanies it) impair host defenses against bacteria and other organisms. Thus, infections are actually more frequent in hepatic cirrhosis as is shown

The most common presenting symptoms of Rapunzel syndrome include abdominal pain (37%), nausea and vomiting (33.3%), obstruction (25.9%), and peritonitis (18.3%). Less commonly, patients may present with weight loss (7.4%) or intussusception (7.4%). Exceedingly rare complications of Rapunzel syndrome include gastric ulceration, obstructive jaundice, and acute pancreatitis as well as other malabsorptive-related complications including protein-losing enteropathy, iron deficiency, and megaloblastic anemia. This report details the case of an 11-year-old female with Rapunzel syndrome complicated by

Pancreatic cancer commonly diagnosed at late stage due to subtle clinical manifestation and associated with low 5-year survival rate. Only 10-20% of patients were found in resectable or localized stage. Several complications may arise due to advanced pancreatic cancer such as obstructive jaundice, gastric outlet obstruction, pancreatic cancer cachexia, pruritus of cholestasis, and cancer pain. Palliative management should be optimized in order to improve patient's quality of life. A gastroentero-hepatologist should collaborate with other specialties to give comprehensive palliative care for ad

Mirizzi syndrome, a rare complication of chronic cholelithiasis, is caused by an impacted stone in the cystic duct or the neck of the gallbladder. Patients present with abdominal pain, fever, and obstructive jaundice. The cholangiographic finding is a smooth stricture caused by lateral compression of the common hepatic duct. A similar appearance on cholangiogram can result from carcinoma of the gallbladder, carcinoma of the cystic duct, or hilar adenopathy. Acute acalculous cholecystitis simulating Mirizzi syndrome is extremely rare. This is the report of such a case in which marked inflammato

MR cholangiopancreatography (MRCP) using a respiratory-triggered fast spin-echo technique was performed on intermediate MR imaging in 26 patients with suspected biliary disease. In almost all of 12 patients without dilated biliary tract, the hilum of the liver and extrahepatic bile duct were clearly visualized. All of 11 cases of cholelithiasis were demonstrated. In all 4 cases of obstructive jaundice, dilatation and obstruction of the bile ducts were clearly demonstrated. Respiratory-triggered fast spin-echo MRCP is a non-invasive technique for visualization of the biliary tract and biliary d

Clonorchiasis is a trematodiasis caused by chronic infestation of liver flukes, Clonorchis sinensis. The adult flukes reside in the medium-sized and small intrahepatic bile ducts and, occasionally, in the extrahepatic bile ducts, gallbladder, and pancreatic duct. The result is mechanical obstruction, inflammatory reaction, adenomatous hyperplasia, and periductal fibrosis. Signs and symptoms are usually mild and nonspecific, but heavy infestation results in obstructive jaundice. The disease has a close relationship with recurrent pyogenic cholangitis and cholangiocarcinoma. In this article, the

At the Tel-Aviv Medical Center, five of 720 (0.69%) patients undergoing endoscopic retrograde cholangiopancreatography (ERCP) subsequently developed septicaemia. One of these patients (0.14%) died as a direct result and three other septicaemic patients died from other complications. The most important risk factor associated with infectious complications was obstructive jaundice. Retrospective analysis of our data revealed that no cases of septicaemia developed in patients given prophylactic antibiotics, whereas 4.3% of patients who received no prophylactics developed infectious complications.

Ascariasis lumbricoides is one of the most common helminthic infections in developing countries. Biliary ascariasis presents with recurrent cholangitis and sometimes with liver abscesses. We report the case of a young Philippino worker in Saudi Arabia who presented with obstructive jaundice, cholangitis and liver abscesses secondary to biliary tract ascariasis. The diagnosis was made at endoscopic retrograde cholangio-pancreatography (ERCP) and the obstructing worms were removed with a Dormia basket after endoscopic sphincterotomy at this procedure. Obstruction of the biliary system due to intraductal echinococcal cysts is a rare but serious presentation of hepatic echinococcal disease. Four cases are described. There was a 50% mortality rate and morbidity was high. The clinical presentation is difficult to differentiate from other disease processes. Careful preoperative sonograms should be taken to detect echogenic cyst material in the biliary system in all patients with hepatic echinococcal cysts. If biliary pigments are seen in the cyst during surgical removal, or if the patient has a recent history of jaundice or cholangitis, intraope

17 patients with primary carcinoma of the gallbladder were studied. In 7 cases a palliative operation for biliary or gastrointestinal obstruction was performed, in 6 patients a biopsy only. None of the 4 patients undergoing a more "radical" procedure survived 2 years. The main symptoms were pain, jaundice and weight loss. Other manifestations were acute cholecystitis and pyloric obstruction. A correct preoperative diagnosis was established only in 3 cases. Most of the small tumors are discovered by the surgeon who performs a cholecystectomy for stones. Simple cholecystectomy, however, is a pal

We report the rare case of an 80-year-old man with hepatocellular carcinoma that ruptured in the gallbladder, causing a cystic artery pseudoaneurysm and hemobilia. Emergency transarterial embolization (TAE) successfully controlled the bleeding without causing ischemic cholecystitis. Cone-beam computed tomography angiography was useful in identifying the bleeding branch of the selectively embolized cystic artery. Although the patient had poor liver function (Child-Pugh class C) before TAE, it remarkably improved after embolization due to the resolution of coagulopathy and obstructive jaundice c
