[Management of postoperative ischemic jejunal stenosis using balloon dilatation and/or Wallstent implant].

Ischaemic stenosis of the jejunum is rare. For technical, anatomical, and pathological reasons ischemic stenosis of the jejunal segment used for the replacement of the stomach and oesophagus requires a special approach. The present study reports two cases of dilation of ischaemic strictures of the jejunal loop by balloon catheter, used for replacement after oesophagogastrectomy and gastrectomy. In the later case, in which the occlusion of the blood vessels supplying the affected segment was observed right at the level of the aorta, Wallstent was implanted. The advantages and disadvantages of m

Malignant Small Bowel Obstruction Unveiling Rare High-Grade Serous Ovarian Carcinoma of Mullerian Origin: A Case Report Detailing Diagnosis and Management.

Malignant small bowel obstruction in advanced ovarian carcinoma is uncommon but carries high morbidity; prompt diagnosis and targeted interv … mptom relief and maintain enteral nutrition. Operations performed included gastrojejunal bypass and jejunostomy tube placement. Postoperatively, the patient tolerated nasogastric decompression and total parenteral nutrition, successfully transitioning to jejunal feeding. Outcomes and adverse effects of teduglutide treatment with periodic withdrawal in pediatric short bowel patients.

OBJECTIVES: We evaluated the efficacy and safety of teduglutide in a real-world cohort in which national reimbursement policies required treatment interruptions. The primary outcomes were reduction in parenteral support (PS) and treatment-related adverse effects, and the secondary outcome was the impact of the mandated withdrawal periods. METHODS: In this retrospective registry-based nationwide study, we identified all pediatric short bowel-associated intestinal failure (IF) patients treated with teduglutide and reviewed clinical records to evaluate PS reduction and adverse effects during tre

Development of a gastrocutaneous fistula from a marginal ulcer after repair of duodenal injury with pyloric exclusion.

INTRODUCTION: Duodenal trauma is rare but can be associated with significant morbidity and mortality (Pandey et al., 2011). Adjunct procedures, such as pyloric exclusion, can be performed to assist in surgical repair of these injuries. However, pyloric exclusion can lead to severe long-term complications associated with significant morbidity that can be difficult to repair. CASE: A 35-year-old man with a history of duodenal trauma from a gunshot wound (GSW) status post pyloric exclusion and Roux-en-Y gastrojejunostomy presented to the Emergency Department (ED) with complaints of abdominal pai…

INTRODUCTION: Superior mesenteric artery syndrome is a rare entity, caused by compression of the duodenum between the superior mesenteric ar … erior mesenteric artery compression syndrome. He underwent a mini-laparotomy for the placement of a jejunostomy feeding tube aiming to promote weight gain and mesenteric fat restoration for preventing the SMA compression effects on the duodenum. DISCUSSION: The decrease in retroperitoneal fat owing to weight loss may result in aortomesenteric angle reduction …

Mucormycosis-induced upper gastrointestinal ulcer perforation in immunocompetent patients: a report of two cases.

BACKGROUND: Gastrointestinal mucormycosis (GIM) is a rare, opportunistic fungal infection with poor prognosis. Clinically, it is difficult t … s due to exfoliation and necrosis of the stomach wall. The patient underwent total gastrectomy plus jejunostomy. The pathology results revealed severe bacterial infection and fungal infection that was confirmed as GIM. The patient fully recovered after receiving anti-infective and antifungal treatments. CONCLUSIONS: Neither patient was immunosuppressed, and …

Successful preservation of the proximal stomach tube by evaluating blood flow using indocyanine green for gastric tube cancer: a case report.

BACKGROUND: There have been two reports on preserving the proximal gastric tube by using intraoperative indocyanine green (ICG)-based photod … ic tube approximately 4 cm from the esophagogastric anastomosis and then performed Roux-en-Y gastro-jejunostomy via the ante-sternum route. The postoperative course was stable, and the patient was discharged on the 14th postoperative day. CONCLUSIONS: ICG-based photodynamic diagnosis was found to be simple and less invasive. Therefore, even if the postoperat …

Nutrient deficiencies in tube-fed children.

The charts of children with gastrostomy or jejunostomy tubes followed in the Tube Feeding Clinic at the Children's Hospital of Wisconsin from January 1999 to June 2002 were reviewed. Prevalence of selected nutrient deficiencies were determined and their association with growth and caloric intake was measured. One hundred fifty patients (60% male) met the inclusion criteria. The mean age of children was 7 years 6 months (SD +/- 5 years 4 months). As a proportion of total caloric intake, the mean percentage of calories from formula was 92% (+/- 15). No deficiencies of vitamins D or E were found.…

Increasing complexity in children with life-limiting conditions in England: an analysis of national routine data.

INTRODUCTION: The number of children with life-limiting conditions has risen in recent decades, partly due to medical advances enabling increased survival. It is unclear how the medical complexity of this population has changed. AIM: To use quantitative analyses of routinely collected healthcare and administrative data from 2003 to 2022 to assess the complexity of children with a life-limiting condition (0-19 years) over time. METHODS: National hospital data in England (Hospital Episode Statistics) were used to identify children aged 0-19 years with a life-limiting condition from 2003/2004 t
