BACKGROUND: Antibiotic cement articulating spacers are recommended during 2-stage revision for prosthetic knee infection because of increased range of motion (ROM) and improved function; however, spacer mechanical complications have been reported. We aimed to determine the association between different constraints of articulating spacers and the rate of complications and infection eradication, functional outcomes, and ROM.
METHODS: A retrospective study of prosthetic knee infection using cruciate-retaining (CR) or posterior-stabilized (PS) spacers was conducted between 2011 and 2018. The rate

PURPOSE: There have been few large sample studies reporting the midterm outcome of Oxford phase 3 unicompartmental knee arthroplasty (UKA) in Asian patients.
METHODS: The study included 708 consecutive medial Oxford UKAs between February 2005 and May 2014 in Chinese patients. All cases were performed for the recommended indications with a minimally-invasive surgical technique. The functional and radiological outcomes were subsequently examined. In particular, we divided patients into the spontaneous osteonecrosis of the knee (SONK) group and the osteoarthritis (OA) group.
RESULTS: All patien

BACKGROUND: Medial UKA performed in England and Wales represents seven to 11% of all knee arthroplasty procedures, and is most commonly performed using mobile-bearing designs. Fixed bearing eliminates the risk of bearing dislocation, however some studies have shown higher revision rates for all-polyethylene tibial components compared to those that utilize metal-backed implants. The aim of the study is to analyse survivorship and maximum eight-year clinical outcome of medial fixed bearing, Uniglide unicompartmental knee arthroplasty performed using an all-polyethylene tibial component with a mi

In this report, 100 knees in 77 patients, with an average age of 65, were followed for a period between five and 10 years. Rheumatoid arthritis was the diagnosis in 43 patients and osteoarthritis in 34 patients. The majority had varus, valgus, and biplane deformities. Twenty-one patients underwent bilateral procedures; all but one had patellar replacement. Over 90% of the knees were rated good to excellent according to The Hospital for Special Surgery Knee Disability Score Sheet. Of the 34 osteoarthritic patients (40 knees), 24 or 71% could walk ten blocks and beyond. Ten patients, or 29%, cou

Hypermobility of joints and spine is a consequence of abnormal laxity of ligaments, joint capsules and intervertebral discs. Hypermobility per se is a state, not a disease. However, it may lead to generalized arthralgia or localized symptoms (frequent ankle sprains, knee effusions, dislocations of the shoulders of recurrent episodes of back pain). These events may occur even after minor strains. Young women are preferentially affected. The term "hypermobility syndrome" is defined as a combination of hypermobility with arthralgia, low back pain, frequent dislocation or sprains. It is often the

BACKGROUND: Recurrent patellar dislocation is common clinically, primarily in adolescents. However, the biomechanical properties of single- and double-bundle medial patellofemoral ligament (MPFL) reconstruction remain poorly understood.
METHODS: Six fresh frozen adult cadaveric knee specimens were obtained for this study. Each specimen was fixed at 0° to test the force needed when the patella was laterally shifted 10 mm at a speed of 0.5 mm/s, and the test was repeated three times. This test was repeated when knee flexion was at 0°, 15°, 30°, 45°, 60°, and 90°. All six specimens were tested i

BACKGROUND: Congenital knee dislocation (CKD), or genu recurvatum, is an uncommon congenital deformity marked by knee hyperextension and restricted flexion. It may occur either as an isolated finding or in association with other conditions. CKD can be diagnosed prenatally or postnatally. Most of the cases are treated during infancy by manipulation and serial casting or splinting without surgical intervention. This case report aims to enhance the understanding of this condition and its associations, improving the process of early diagnosis and management to prevent long-term functional impairme…

Osteochondral lesions (OCLs) that are frequently encountered in skeletally immature and adult patients are more common than once thought, and their incidence rate is rising. These lesions can appear in many synovial joints of the body, such as the shoulder, elbow, hip, and ankle, occurring most often in the knee. The term osteochondral lesion includes a vast spectrum of pathologies such as osteochondritis dissecans, osteochondral defects, osteochondral fractures, and osteonecrosis of the subchondral bone. When considering this, the term osteochondral fracture is preserved only for an osteochon

The multiple-ligament injured knee is a complex problem in orthopaedic surgery. Most dislocated knees involve tears of the anterior and posterior cruciate ligaments (ACL/PCL) and at least 1 collateral ligament complex. Careful assessment of the extremity vascular status is essential because of the possibility of arterial and/or venous compromise. These complex injuries require a systematic approach to evaluation and treatment. Physical examination and imaging studies enable the surgeon to make a correct diagnosis and to formulate a treatment plan. Arthroscopically assisted combined ACL/PCL rec

AIMS: Cementless fixation is an alternative to cemented unicompartmental knee arthroplasty (UKA), with several advantages over cementation. This study reports on the 16-year survival and ten-year clinical and radiological outcomes of the cementless Oxford Unicompartmental Knee Replacement (OUKR).
METHODS: This is a prospective study of the first 693 consecutive cementless medial OUKRs implanted in New Zealand.
RESULTS: The 16-year survival was 89.2%, with 46 knees being revised. The most common reason for revision was progression of arthritis, which occurred in 24 knees. The other reasons fo

BACKGROUND: The combination of a long-nosed patella and patella alta can lead to symptoms such as anterior knee pain and patellofemoral instability.
PURPOSE: Our objective was to address this uncommon, multifactorial cause of patellar pain and instability by a single-stage combined surgical approach.
CASE PRESENTATION: A 14-year-old German female presented to our hospital for recurrent patellar dislocations and exacerbated infrapatellar pain during kneeling. Following physical examination and imaging, the patient was diagnosed with patellar instability combined with the rare "Cyrano"-type pa

BACKGROUND: The gracilis tendon is commonly used as an autograft to reconstruct torn tendons or ligaments in many parts of the body. Little is known about the subjective and functional outcome after gracilis tendon harvest. The aim of this study was to evaluate the outcome of the donor leg in patients undergoing such surgery.
METHODS: Patients with chronic acromioclavicular joint dislocations undergoing coracoclavicular ligament reconstructions using autogenous gracilis tendon grafts were eligible for this study. The graft harvesting procedure was carried out in a standard fashion using a ten

An increased tibial tubercle-trochlear groove (TT-TG) distance is related to patellar maltracking and instability. Tibial tubercle transfer is a common treatment option for these patients with good short-term results, although the results can deteriorate over time owing to the progression of osteoarthritis. We present a ten-year follow-up study of a self-centring tibial tubercle osteotomy in 60 knees, 30 with maltracking and 30 with patellar instability. Inclusion criteria were a TT-TG ≥ 15 mm and symptoms for > one year. One patient (one knee) was lost to follow-up and one required total knee

UNLABELLED: The diagnosis of osteochondral fractures and medial patellar retinaculum injuries that originate as a result of Traumatic Patellar Dislocation (TPD) appears to be crucial for establishing the treatment strategy. Those injuries may be efficiently recognized by means of ultrasonography (USG).
MATERIAL AND METHODS: The analysis of the USG patterns in 22 knees after TPD in 21 patients treated between 2004 and 2007 was proceeded. In all children knee USG examination followed its clinical examination and X ray imaging. The standard USG knee assessment was supplemented by patellar articu

The lateral patellofemoral complex is an important stabilizer to medial and lateral displacement of the patella. Soft tissue abnormalities can range from pathologic tightness to laxity, presenting with symptoms related to patellar instability, anterior knee pain, or arthritis. Clinical evaluation should be performed to confirm patellar dislocation, assess the integrity of the lateral and medial soft tissues, and explore other pathoanatomic factors that may need to be addressed. Lateral retinacular lengthening is recommended over lateral release owing to the potential of iatrogenic medial insta

Complex two-level rotational malalignment of the lower extremity can cause maltracking of the patella with anterior knee pain. Double derotation osteotomy would correct the underlying pathology. However, it carries a high risk of complications such as nerve and vessel damage. We report a case of rotational malalignment in the femur and the tibia associated with trochlear dysplasia, which causes painful patellar instability. The patient was successfully treated with reconstruction of the medial patellofemoral ligament and lateral release. Although the malrotation was not addressed, the position

Obesity is an increasing burden on health-care globally. Significant obesity is presenting at a younger age, with pathology that has not been previously seen. This case report illustrates the catastrophic consequences which may occur when minor trauma occurs in a young person who suffers from morbid obesity. A 19-year-old woman with BMI 50 tripped over an uneven curb, and suffered complete dislocation of the knee with associated popliteal artery injury. She required femoro-popliteal bypass using vein. This case reports the youngest person to suffer from this injury and the first in the UK.

This study examines and compares three treatment options for patellofemoral arthritis resulting from patellofemoral malalignment: longitudinal semipatellectomy, total patellectomy, and a modified Trillat procedure. Patients returned for examination in a 3- to 12-year follow-up period. Results from longitudinal semipatellectomy were unsatisfactory, with a reoperation and failure rate of 70%. Good long-term effects were found with total patellectomy (87% good or excellent). The modified Trillat procedure was effective in preventing recurrent dislocation (94%); however, the procedure was less eff
