OBJECTIVE: To analyze the possibility and practicality of using free anterolateral thigh flap to repair defects of oromaxillo-facial region.
METHODS: One hundred and seventy-two patients underwent anterolateral thigh flap (ALP) reconstruction from Jan. 2007 to Dec. 2008. The size of the defect ranges from 4 cm x 4 cm to 10 cm x 25 cm. Two female patients presented nodular erythematous lesions overlying a permanent tattooed eyebrow and lip, respectively. Histologic examination showed in both cases epithelioid granulomas in close relation with scattered pigment. Complementary examinations and follow-up disclosed a sarcoidosis. The lesions resolved after treatment with topical steroids and also oral allopurinol in one of the cases. Allopurinol may be an effective treatment for granulomatous reactions to foreign body particles.

The facial artery musculomucosal flap, technically a combination of the nasolabial flap and the buccal mucosal flap, has been a reliable, versatile flap, either superiorly or inferiorly based for reconstruction of a wide variety of postcancer excision intraoral mucosal defects including defects of the palate, alveolus, lips and floor of mouth. We have used it 17 times in 16 patients with no failures and one flap with terminal necrosis. Almost all flaps developed venous congestion which settled on its own by conservative management.

By combining the principles of nasolabial and buccal mucosal flaps, we have designed a new axial musculomucosal flap based on the facial artery.
