PURPOSE: Gold standard recipient arteries in head and neck free flap microvascular reconstruction are currently branches of the external carotid. However, these arteries can be compromised by neck dissection or radiotherapy, resulting in 'vessel-depleted neck' and 'frozen neck' respectively. In such cases, the transverse cervical artery (TCA) may be a suitable recipient artery.
METHODS: The origin, course and diameter of the TCA were determined in 46 sides of neck from 23 cadavers. The distances from the origin of the TCA to the angle of the mandible, floor of the mouth and mandibular symphys

BACKGROUND/PURPOSE: Medication-related osteonecrosis of the jaw (MRONJ) is a severe complication associated with anti-resorptive therapy, and surgical intervention remains an effective treatment strategy regardless of disease stage. While extensive surgical resection is considered the standard approach for advanced-stage MRONJ, it may not be suitable for elderly patients or those with systemic comorbidities. This study evaluated the outcomes of conservative surgical management in patients with stage III MRONJ affecting the anterior mandible.
MATERIALS AND METHODS: A retrospective study was co

Our aim was to evaluate the effect of a digital template in the preservation of the inferior alveolar neurovascular bundle during osteotomy for benign lesions of the mandible in 6 patients who were treated with mandibular osteotomies during 2013. Computed tomographic (CT) data were imported into ProPlan CMF 1.4 software. The borders of the lesion and the inferior alveolar canal were marked, and a digital template designed to mark the borders, outline the canal, and guide the osteotomy. A mirror image of the unaffected mandible was used to make a stereolithographic model by a rapid prototyping

BACKGROUND: Knowledge about the anatomic variations of the root canal system and their prevalence is necessary for clinicians to ideally clean the root canal system. The anatomic complexity of the root canal system is one of the reasons for its inadequate debridement, resulting in residual microorganisms and root canal treatment failure. The present study aimed to evaluate the prevalence of middle mesial root canals in mandibular molars in an Iranian population.
MATERIALS AND METHODS: The samples in the present descriptive/cross-sectional study consisted of mandibular first and second molars

The rate of nerve regeneration is a critical determinant of the degree of functional recovery after injury. Here, we sought to determine whether treatment with the neuroprotective compound, agmatine, with or without nerve reconstruction utilizing a regional autogenous vein graft would accelerate the rate of facial nerve regeneration. Experiments compared the following seven groups of adult male rats: (A) Intact untreated controls. (B) Sham operation with interruption of the nerve blood supply (controls). (C) Transection of the mandibular branch of the facial nerve (generating a gap of 3 mm) fo

The purpose of this study was to assess the utility of a fascicular turnover flap for facial nerve repair and to investigate its possible application in the field of facial nerve repair using a rat model of facial paralysis. Twenty-four Wistar rats were used in this study. A left vibrissal muscle palsy model was established via excision of the buccal and marginal branches through a periauricular incision. In Group 1, the nerve gap was not reconstructed. In Group 2, the nerve gap of the marginal mandibular branch was reconstructed using an autograft, and in Group 3, the gap was reconstructed us

Titanium alloys are used in skeletal surgery. However, once bone union is complete, such fixation material becomes unnecessary or even harmful. Resorbable magnesium materials have been available for several years (WE43 alloy). The aim of this study was to clinically compare magnesium versus titanium open reduction and rigid fixations in mandible condylar heads. Ten patients were treated for fractures of the mandibular head with magnesium headless compression screws (2.3 mm in diameter), and 11 patients were included as a reference group with titanium screws (1.8 mm in diameter) with similar co

BACKGROUND: Pre-implant placement posterior mandibular bone grafts remain a challenge for maxillofacial surgeons. We present a coffering technique we prefer using in this localization.
SURGICAL PROCEDURE: This original technique involves creating a sagittal mandibular groove in which a graft harvested from the retromolar region is wedged vertically, after being sharpened in a bone mill. Reconstruction of the alveolar region itself is achieved by filling up the space between this vestibular coffer and the residual alveolar ridge with particulate bone graft from the bone mill.

BACKGROUND: Ameloblastoma is a benign neoplasm composed of epithelial tissue with invasive and infiltrative behavior at the local level and a high recurrence rate, with various histopathologic patterns and clinical forms. Approximately 85% of conventional ameloblastomas occur in the mandible, most often in the body, angle, and ascending ramus area. The treatment modalities include both conservative and radical treatments. Postoperative follow-up is most important in the treatment of ameloblastoma.
AIMS AND OBJECTIVES: To describe the clinicopathologic profile of mandibular ameloblastoma in pa

BACKGROUND: Malignant pediatric jaw tumors are rare but challenging entities. While unresectable tumors are typically managed with chemotherapy and supportive care, our institution advocates for aggressive surgery to reduce local disease progression.
METHODS: We retrospectively reviewed children treated for jaw tumors at a tertiary children's hospital (2006-2024). Patients with maxillary or mandibular cancers and distant metastases who underwent palliative resection followed by microsurgical reconstruction were included. Data on demographics, oncologic history, and outcomes were collected.

BACKGROUND: Ewing sarcoma (ES) is a rare, primary malignancy of bone that occurs in childhood and early adolescence. Improved methods of diagnosis and treatment have dramatically increased survival over the last 20 years. Treatment mainstays are chemotherapy and surgical tumor resection. ES usually occurs in long bones of the axial skeleton; however, it may rarely arise in facial structures, particularly the mandible. In these cases, resection presents a challenging postsurgical reconstruction.
METHODS AND RESULTS: We present the clinical findings and management of a case of ES that developed

Complex facial fractures, when managed without adhering to basic principles of reduction and fixation, can have disastrous consequences for facial soft tissues. This article presents the case of a patient with functional and aesthetic impairments secondary to panfacial fracture sequelae. The proposed treatment plan involved a total reoperation of the panfacial fracture, including bilateral orbital repositioning, orthognathic surgery to correct traumatic occlusal sequelae, and bilateral internal orbital reconstruction. Despite improvements in mandibular width, facial projection, orbital positio
