Fracture-dislocation of the thoracic spine without neurologic lesion.

An 18-year-old man with a fracture-dislocation of the thoracic spine showed remarkable displacement, but without neurologic complications. The halo-femoral traction allowed a reduction, which has been only partially maintained. Despite this, the clinical result is satisfactory. The lesion appears to be more common than the few cases published in the literature would suggest.

Transradial Perilunate Fracture-Dislocation With Lunotriquetral Coalition.

Perilunate fracture-dislocations are devastating injuries, and their occurrence in the presence of a congenital carpal coalition is exceedingly rare. This combination fundamentally alters normal wrist biomechanics and significantly complicates surgical decision-making. We present a case of a 52-year-old female with acute hand ischemia following a high-energy motorcycle collision. Radiographic evaluation revealed a transradial perilunate fracture-dislocation complicated by a unilateral Minnaar type 3 congenital lunotriquetral coalition. Emergent closed reduction was performed, which immediately

A public database on traumatic brachial plexus injury: data collection and sharing.

OBJECTIVES: To create a public digital database centered on adult traumatic brachial plexus injury (TBPI). TBPI mainly affects young males, usually involved in motorcycle accidents, often leading to severe motor and sensory impairment in the affected upper limb. This initiative aimed at encouraging data sharing and reuse. Managing data from TBPI may enable the identification of functional markers associated with clinical improvement in individuals with TBPI and foster the development of new investigative tools to elucidate its mechanisms. DATA DESCRIPTION: Detailed electronic questionnaires w

Labia Majora Symmetrization Using De-epithelialized Deep External Pudendal Artery Tubular Flap.

This case report presents a novel technique for reconstructing posttraumatic asymmetrical labia majora using a deep external pudendal artery tubular flap. A 20-year-old woman experienced deviation of the urinary stream during micturition as a result of vulvar sequelae following a motorcycle accident. To address atrophy of the unilateral labia majora, we designed a de-epithelialized deep external pudendal artery tubular flap measuring 3 cm in width and 10 cm in length. The flap was delayed for 2 weeks before transposition, during which the lateral borders were incised down to the crural fascia,

Trapezius and hamate body fractures. Case report of a rare association.

The trapezium is the most radial and mobile bone in the distal row of the carpus, establishing a proximal joint with the scaphoid and a distal one with the first metacarpal. Isolated trapezium fractures are uncommon, accounting for only 1 to 5% of all carpal fractures. Fractures are often associated with other injuries, including the rare hamate body fracture. At initial presentation, diagnosing those fractures can be challenging as clinical findings are often minimal, without obvious deformities. Crushing injuries are frequently associated with trapezium tuberosity fractures while hamate body

Updated fine-scale location data for Blantyre district collected using a low-cost approach.

Knowing up-to-date geographical location of residential areas is crucial for understanding health-related factors and formulating targeted interventions. However, such data are often unavailable or resource-intensive to collect. This study employed low-cost approaches to map residential areas in Blantyre district, Malawi, which was severely affected by a cholera outbreak that lasted over a year (March 2022 to August 2023). We trained five data collectors using KoBo Toolbox and engaged Health Surveillance Assistants (HSAs) and motorcycle operators for precise location identification. We validat

Achilles Tendon Tear Following an Unusual Pattern of Open Pure Ankle Dislocation: A Case Report.

We present a rare case of concurrent open Achilles tendon tear and pure subtalar dislocation in a 20-year-old male following a motorcycle accident, highlighting the complexity of managing such unique musculoskeletal injuries. The patient was initially admitted with an open and deformed left ankle, underwent prompt primary debridement, and received immediate medical intervention. Despite unsuccessful attempts to reduce the dislocation through external maneuvers, subsequent radiographic evaluation revealed a subtalar dislocation associated with a 90° rotation of the talus. A direct reduction was

The Macklin Effect: An Underestimated Cause of Pneumomediastinum.

The Macklin effect is a rare but potentially serious complication of pneumomediastinum, caused by the dissemination of air from the lungs into the subcutaneous tissue and mediastinum after severe chest trauma or invasive manipulation. Early recognition is crucial for proper management of the patient. A 33-year-old male skidded while riding a motorcycle, lost control of the vehicle, and crashed into a utility pole with a thoracic contusion. He was admitted to the hospital; a computed tomography (CT) of the chest and abdomen was requested, which ruled out the presence of fractures and showed air

Acute respiratory distress syndrome after treating ipsilateral femoral shaft and neck fracture with reamed intramedullary nailing in an asymptomatic COVID-19 patient.

Currently, it is quite difficult to determine the best timeframe of operative treatment, as well as conduct surgery on asymptomatic COVID-19 patients due to misunderstandings and unawareness of the patient's inflammation state. Caution needs to be taken in specific patient cohorts, particularly those with femoral shaft fractures, who are more likely to develop diseases like acute respiratory distress syndrome after a procedure like intramedullary nailing. In this case report, a 36-year-old patient suffered a motorcycle accident in which he suffered an ipsilateral femoral shaft and neck fractur

Case Report of a Post-Traumatic Hydrocephalus Patient: Ventriculo-Vesicle Shunt With a Review of the Literature.

Post-traumatic hydrocephalus (PTH) is treated through cerebrospinal fluid (CSF) diversion, typically through ventriculoperitoneal shunt (VPS) or other bypass techniques. As these shunts are associated with significant complications and high revision rates in certain populations, it is important to tailor a patient's shunt procedure according to their medical history and complications. Herein, we report the case of a 30-year-old man with PTH following a traffic accident on a motorcycle. VPS was chosen as the method of treatment but required multiple revisions and replacements due to persistent

Delayed Tension Hemothorax With Nondisplaced Rib Fractures After Blunt Thoracic Trauma.

Blunt thoracic trauma often causes rib fractures, hemothorax, and pneumothorax. Although there is no established definition regarding the duration and management of delayed hemothorax, it commonly occurs in a few days and exhibits at least one displaced rib fracture. Moreover, delayed hemothorax rarely develops tension hemothorax. A 58-year-old male who had a motorcycle accident received conservative treatment from his orthopedic doctor. He felt a sudden severe chest pain 19 days after the accident. Contrast-enhanced computed tomography (CT) of the chest revealed multiple left-sided rib fractu

Good outcome after an 8-year radial head resection on a Monteggia fracture in a young patient: A case report and literature review.

INTRODUCTION AND IMPORTANCE: Monteggia fractures are proximal ulna fractures that involve radial caput displacement. Radial caput excision or resection is only suggested in situations of Monteggia lesions with radial caput fractures. Radial head resection should be avoided because the radial head is the principal stabilizer of the elbow and forearm. In this case report, we present a case of radial head resection at a young age after Monteggia reconstruction. CASE PRESENTATION: A 26-year-old female came to the orthopaedic clinic for removal implant control after eight years postoperatively, pa

Rupture of finger distal interphalangeal collateral ligament treated with a mini soft anchor.

BACKGROUND: Ruptured finger distal interphalangeal (DIP) collateral ligament is a rare injury, with controversial treatment until the present. Our case series aimed to demonstrate feasible surgical intervention using a mini anchor. METHODS: The present study includes four patients with ruptured finger DIP collateral ligaments who received primary repair in a single institute. They have suffered from joint instability due to ligament loss caused by infection, motorcycle accidents, and work-related accidents. All patients were operated on similarly using a 1.0 mm mini anchor for ligament reatta

Functional outcome of modified Jones procedure in patient with non-union Humerus and high radial nerve palsy: A case report.

BACKGROUND: Middle to distal humeral fractures can cause high radial nerve palsy. A tendon transfer surgery can be performed to treat irreversible and longstanding radial nerve palsy in order to improve the lost hand function. CASE REPORT: A 39-year-old right-handed female office worker presented with deformity in her left arm and drop hand. She had a motorcycle accident a year before and was treated by a traditional bonesetter. The extension movements in her left wrist and fingers were restricted. We performed an ORIF with bone graft at her left nonunion humerus and also tendon transfer proc

An Eye Capturing Clutch - An Orbital Foreign Body.

Foreign bodies inside the orbital cavity are rare. They may lead to serious complications, depending on their nature, size and mechanism of injury. A 29-year-old male presented with a motorcycle handle embedded in his left orbit, with the vision unaffected. Active wound bleeding, increasing hematoma, a low haemoglobin level, signs of hypovolemic shock, ocular acuity and mobility were investigated. A computed tomography scan revealed a long bent metallic object lodged between the globe and floor of the left orbit with fracture of the medial orbital wall and ethmoidal hemosinus with an intact cr

Leg Impalement - A Rare Injury Pattern Case Report.

INTRODUCTION: Impalement injuries are defined as rare, high-energy lesions caused by foreign bodies, usually steel bars or wooden objects, which pierce body cavities or extremities and remain interposed in the perforated body region. They usually occur with road accident or civil construction falls. CASE REPORT: A 24-year-old male patient was admitted at the emergency department after a motorcycle accident, resulting a left leg impalement with a wooden object. A partial deep peroneal nerve palsy and a proximal third fibula fracture were observed. The foreign body removal and wound debridement

Mind the gap: A novel endovascular technique to repair a transected vessel.

Level one trauma centers experience horrific injuries on a regular basis. Blunt or penetrating trauma causing vascular injuries are treated by surgeons and interventional radiologists. When a blood vessel is completely transected, the ends of the vessel retract and vasospasm occurs as a normal survival response. When this phenomenon occurs, it is sometimes impossible to reattach the two ends of the injured vessel by surgical means and a bypass graft is often required. However, from an endovascular perspective, covered stents can serve as a vascular bypass as well. The limiting factor with an e

The surgical history of head injury in motor vehicle collision.

Motor vehicle collisions (MVCs) are a significant cause of head injuries today, but efforts to manage and prevent these injuries extend as far back as the beginning of modern neurosurgery itself. Head trauma in MVCs occurred as far back as 1899, and the surgical literature of the time mentions several cases of children being struck by passing automobiles. By the 1930s, Dr. Claire L. Straith, a Detroit plastic surgeon, recommended changes to automobile design after seeing facial injuries and depressed skull fractures that resulted from automobile accidents. During World War II, Sir Hugh Cairns,

Alcohol Misuse and Illicit Drug Use Among Occupational Groups at High Risk of HIV in Sub-Saharan Africa: A Systematic Review.

Key occupational groups in sub-Saharan Africa (SSA) are at increased risk of HIV, and may be at increased risk of substance use. In January 2018, we systematically searched for studies reporting prevalence of, and risk factors for alcohol misuse or illicit drug use and their association with HIV incidence or prevalence among fisherfolk, uniformed personnel, truckers, miners, motorcycle taxi riders and sex workers in SSA. Seventy-one studies published between 1983 and 2017 were included: 35 reported on alcohol misuse (19 using AUDIT, 5 using CAGE) and 44 on illicit drug use (eight reported both

[Management of Rib Fractures].

The therapy of rib fractures is controversially discussed. Neither does an osteosynthesis have to be performed for each individual rib fracture, nor is the internal splinting by ventilation or respiratory therapy sufficient for each patient. Rib fractures are common in polytrauma patients after car and motorcycle accidents or falls from great heights. However, rib fractures are also increasingly occurring in older patients with falls from low heights. Anamnesis and clinical examination are groundbreaking for the diagnosis and therapy decision of rib fractures. In radiological diagnostics, proj
