Spastic movement disorder is characterized by reduced ability to selectively activate muscles with significant co-activation of antagonist muscles. It has traditionally been thought that hyperexcitable stretch reflexes have a central role in the pathophysiology and the clinical manifestations of the disorder. Here we argue that the main functional challenges for persons with spastic movement disorder are related to contractures, paresis, weak muscles and inappropriate central motor commands, whereas hyperexcitable reflexes play no or only an insignificant functional role. Co-activation of anta

Restless legs syndrome, periodic limb movements in sleep, and periodic limb movement disorder are a group of conditions that merit awareness from the medical community. These disorders are commonly encountered yet are often confused and misdiagnosed by health care professionals. It is imperative that health care providers are able to recognize these conditions to accurately diagnose, manage, and appropriately refer patients.

Abnormalities of eye movements have been reported in patients with Parkinson's disease (PD). However, it is unclear if they occur in the prodromal stage of synucleinopathy represented by idiopathic rapid eye movement sleep behaviour disorder (iRBD). We thus aimed to study eye movements in subjects with iRBD and in de novo PD, to assess if their abnormalities may serve as a clinical biomarker of neurodegeneration. Fifty subjects with polysomnography-confirmed iRBD (46 male, age 40-79 years), 18 newly diagnosed, untreated PD patients (13 male, age 43-75 years) and 25 healthy controls (20 male, a

BACKGROUND: Patients with Huntington disease (HD) and their spouses often complain of agitation during sleep, but the causes are mostly unknown.
OBJECTIVE: To evaluate sleep and nocturnal movements in patients with various HD stages and CAG repeats length.
METHODS: The clinical features and sleep studies of 29 patients with HD were retrospectively collected (11 referred for genotype-phenotype correlations and 18 for agitation during sleep) and compared with those of 29 age- and sex-matched healthy controls. All patients had videopolysomnography, but the movements during arousals were re-anal

Periodic limb movement disorder (PLMD) is a condition in which patients experience frequent periodic limb movements of sleep (PLMS). Synchronized arousal responses cause sleep fragmentation, resulting in insomnia, daytime sleepiness, and fatigue. A 59-year-old man was identified as having intense sleep-talking and body movements, suggesting rapid eye movement (REM) sleep behavior disorder (RBD). Attended video-polysomnography (PSG) revealed that sleep-talking and body movements occurred only during non-REM sleep and were associated with PLMS-induced arousals (periodic leg movement arousal inde

Multiple studies in patients with obsessive-compulsive disorder (OCD) became the basis for revealing selective attention, inhibitory control, and working memory impairments, which correlates with an imbalance in the activity of the cortico-striatal-thalamic-cortical circuit associated with maintenance of cognitive control functions. Patients with OCD often demonstrate changes in the parameters of target-oriented eye movement reactions being a consequence of a possible impairment of the cognitive control neurophysiological framework. This review summarizes and analyzes data on cognitive control

Repetitive movements are the hallmark of chronic tic disorders such as Tourette disorder (TD), whereas repetitive behaviors are characteristic of obsessive-compulsive disorder (OCD). Both disorders tend to frequently co-occur, as a substantial number of patients with TD also suffer from OCD and vice versa. While the clinical distinction between tics and OCD is usually easy, there are borderline forms, and correct symptom recognition is vital for choosing the appropriate treatment modalities for each disorder, be they pharmacological, psychotherapeutic (behavioral therapy) or brain stimulation

Cholinergic system degeneration is a hallmark of Lewy body disease, including Parkinson's disease (PD) and dementia with Lewy bodies (DLB), but its involvement in prodromal stages, particularly regarding pedunculopontine-thalamic projections, remains unclear. This study investigated cholinergic pathway integrity in isolated REM sleep behaviour disorder (iRBD) and its relation to phenoconversion risk. We analyzed data from 146 iRBD patients and 102 controls from the Parkinson's Progression Markers Initiative using T1-weighted MRI for basal forebrain volume and diffusion-weighted imaging for thr

PURPOSE OF REVIEW: Functional movement disorder (FMD) is a biopsychosocially-complex condition with diverse neurologic, physical, and psychiatric manifestations. Converging neuroimaging evidence demonstrates abnormalities in networks supporting self-agency, attention, sensorimotor processing, multimodal integration, salience/threat detection, and interoception. This review summarizes recent work on the prognostic and therapeutic relevance of these findings. RECENT FINDINGS: Baseline functional neuroimaging predictors of outcome implicate cingulo-insular, frontolimbic, and subcortical circuits

The PRRT2 gene located at 16p11 encodes proline-rich transmembrane protein with the heterozygous PRRT2 mutation being commonly reported. The most common variant found was the c.649dup.(Arg217Profs*8). Various case reviews documenting pathogenic PRRT2 variants reported an association with paroxysmal movement disorders, including paroxysmal kinesigenic dyskinesia (PKD), benign familial infantile epilepsy, paroxysmal kinesigenic dyskinesia associated with infantile convulsions (PKD/IC), also known as infantile convulsions with choreoathetosis syndrome paroxysmal non-kinesigenic dyskinesia (PNKD),…

OBJECTIVE: This study aimed to determine if there are deficits in force variability, force increase, force decrease and force errors in rapid eye movement behavior disorder (RBD) using established force control paradigms.
METHODS: A cohort of 27 controls, 37 RBD and 37 early-stage Parkinson's disease (PD) were investigated. Individuals completed constant force and ballistic force control for the finger and ankle.
RESULTS: There was greater force variability in RBD compared with controls and PD during the constant force tasks (p < 0.05). Additionally, we split the RBD group into those with mi

BACKGROUND: Hereditary ataxias are genetically diverse, yet up to 75% remain undiagnosed due to technological and financial barriers. A pathogenic ZFHX3 GGC repeat expansion was recently linked to spinocerebellar ataxia type 4 (SCA4), characterized by progressive ataxia and sensory neuropathy, with all reported cases in individuals of Northern European ancestry.
METHODS: We performed Oxford Nanopore Technologies (ONT) genome long-read sequencing (>115 GB per sample) on a total of 15 individuals from Chile; 14 patients with suspected hereditary movement disorders and one unrelated family membe…

Patients with sustained shoulder elevation postures were observed over time in our movement disorders clinic and occasionally reported in literature as variants of dystonia or post-traumatic movement disorders. We retrospectively assessed the clinical records of patients in our movement disorders registry with sustained or fixed shoulder elevation. Their clinical phenomenology, response to treatment and precipitants were investigated. The patients underwent neurophysiologic, genetic and neuropsychologic tests. A PubMed search of cases with similar presentation was performed. Six patients fulfi…

OBJECTIVES: Periodic limb movement disorder (PLMD) involves periodic, stereotypic movements of upper/lower extremities during sleep. Propriospinal myoclonus (PSM) involves repetitive, irregular, myoclonic jerks starting in the midthoracic region and propagating to adjacent segments through propriospinal pathways. Limited evidence exists for interventional modalities. This report discusses the utility of stellate ganglion block (SBG) for PLMD.
METHODS: A review of one patient record who underwent SGB for PLMD.
RESULTS: We present a case of successful treatment of refractory PLMD with SGB.

An under-recognized group with rapid eye movement sleep behavior disorder (RBD) includes children, adolescents, and young adults, who generally have more mild and less injurious RBD compared to middle-aged and older adults (particularly in males). Also, in contrast to older adults, the younger population with RBD rarely has idiopathic RBD but rather has symptomatic RBD from various neurological disorders, such as narcolepsy, brainstem tumors, neurodevelopmental/neuropsychiatric disorders, and also from antidepressant medication-induced RBD. Younger populations also can have parasomnia overlap

Involuntary movements are an uncommon manifestation of transient ischemic attacks (TIAs) and are often associated with carotid occlusion or stenosis. These symptoms are frequently position-dependent, typically occurring when the patient stands or becomes hypotensive, and can be misdiagnosed as focal motor seizures. Patients presenting with this phenomenon may be at a high risk of recurrent stroke due to underlying carotid disease. We present the case of a 79-year-old woman who exhibited hyperkinetic movements on her left side. Her symptoms appeared upon transitioning from sitting to standing a

Paraneoplastic movement disorders, though rare, can be the initial symptoms of malignancies like leiomyosarcoma, as in our case. Clinicians should keep malignancies in their differential diagnosis in cases of unexplained movement abnormalities.
