Outcomes of Total Hip Arthroplasty After Acetabular Open Reduction and Internal Fixation in the Elderly-Acute vs Delayed Total Hip Arthroplasty.

BACKGROUND: Optimum management for the elderly acetabular fracture remains undefined. Open reduction and internal fixation (ORIF) in this population does not allow early weight-bearing and has an increased risk of failure. This study aimed to define outcomes of total hip arthroplasty (THA) in the setting of an acetabular fracture and compared delayed THA after acetabular ORIF (ORIF delayed THA) and acute fixation and THA (ORIF acute THA). METHODS: All acetabular fractures in patients older than 60 years who underwent ORIF between 2007 and 2018 were reviewed (n = 85). Of those, 14 underwent OR

Carpal Fractures Other than Scaphoid in the Athlete.

Carpal fractures of bones other than the scaphoid occur at a much lower rate than scaphoid fractures. The close relationship between the carpus, intrinsic and extrinsic wrist ligaments, and wrist kinematics makes a thorough history, clinical examination, and interpretation of imaging for carpal malalignment essential. Carpal malalignment should be addressed with reduction and fixation. Nondisplaced fractures are often treated nonoperatively and displaced intraarticular fractures are almost always treatment operatively. The physician should keep in mind the athlete's specific goals and needs. T

Lift-Off Screw Results in Accurate Sagittal Tilt Correction in a Distal Radius Fracture Model.

PURPOSE: To determine the accuracy of a distal-first open reduction internal plate fixation technique in achieving correction of volar tilt in dorsally angulated distal radius fractures. METHODS: Twenty foam radius models were divided evenly into groups N (normal) and O (osteoporotic). Dorsally angulated extra-articular distal radius fractures were then created. Group O underwent further modification to simulate an osteoporotic model. After static pinning in various degrees of dorsal angulation, opaque fiducial markers were placed and fluoroscopy was used to measure prereduction volar tilt. A

Functional Outcomes of Unstable Ankle Fractures in Adolescents.

BACKGROUND: The purpose of this study was to describe the functional outcomes and complications of unstable supination external rotation (SER) and pronation external rotation (PER) ankle fractures in adolescents. A secondary outcome was to compare functional outcomes of SER stage IV injuries that were treated definitively with closed reduction and cast application to a similar group of minimally displaced fractures treated with open reduction and internal fixation (ORIF). METHODS: A retrospective review of adolescents aged 10 to 18 years with unstable ankle fractures treated at a single insti

Effect of Surgery vs Functional Bracing on Functional Outcome Among Patients With Closed Displaced Humeral Shaft Fractures: The FISH Randomized Clinical Trial.

IMPORTANCE: Humeral shaft fractures traditionally have been treated nonsurgically, but there has been a steady increase in the rate of surgery over the past 2 decades without high-quality evidence to justify the trend. OBJECTIVE: To compare the effectiveness of surgical treatment with open reduction and internal plate fixation to nonsurgical treatment with functional bracing in the treatment of closed humeral shaft fractures. DESIGN, SETTING, AND PARTICIPANTS: Randomized clinical trial conducted at 2 university hospital trauma centers in Finland, enrollment between November 2012 and January

Can Gartland II and III supracondylar humerus fractures be treated using Blount's method in the emergency room?

BACKGROUND: Studies have established that Blount's method is reliable for treating extension supracondylar fractures (SCFs) in paediatric patients. Reduction in the emergency room (ER) under procedural sedation followed by orthopaedic treatment is increasingly used for many fracture types. The primary objective of this study was to determine whether SCF reduction in the ER was feasible, by determining the failure rate. The secondary objective was to identify causes of failure with the goal of improving patient selection to reduction in the ER. HYPOTHESIS: Gartland II and III SCFs (Lagrange-Ri

Treatment of proximal humerus fractures in children and young adolescents.

BACKGROUND: Fractures of the proximal humerus in children are rare and represent approximately 0.45% of all paediatric fractures. These injuries are common in patients up to an age of 16 years. The treatment of displaced subcapital fractures is still controversially discussed in literature. Therefore the aim of this study was to evaluate the short-term outcome and to provide guidelines for surgical treatment of these fractures in children and adolescents. METHODS: Clinical and radiological results of 231 patients between 0 and 17 years with subcapital humerus fractures were evaluated. Patient

Triplane fracture of distal radius treated surgically: case report and review of the literature.

Only four cases of triplane distal radius fracture have been described in the literature. We report the first that has been treated with open reduction and internal fixation. A 15-year-old boy sustained a triplane fracture of the distal radius. After an unsuccessful attempt at closed reduction, open reduction and internal fixation were performed. Six months after injury, the patient is asymptomatic, but radiographs show a partial growth arrest. The goal of treatment is to achieve anatomic reduction of the articular surface. In this case interposed periosteum prevented anatomic reduction, requi

Ankle fractures malreduction rate and its causes in two Brazilian tertiary training hospitals.

Ankle fractures are one of the most frequent injuries managed by any trauma surgeon. Literature has shown that adequate reduction is of utmost importance to achieve satisfactory functional outcomes. However, malreduction rates remain high worldwide. The objective of the present study is to evaluate the quality of ankle fracture reduction in two major trauma centers in a large Brazilian city and to analyze the factors associated with malreduction METHODS: Epidemiologic data of 382 patients (189 men; 193 women) aged between 7 and 87 years who underwent osteosynthesis of the ankle in two trauma c

A Novel Technique for a Successful Closed Reduction of a Bosworth Fracture-Dislocation of the Ankle.

The Bosworth fracture is defined as a bimalleolar fracture-dislocation of the ankle, with entrapment of the fibula behind the posterior tubercle of the distal tibia. In the current orthopedic literature, not only is this fracture pattern rare, but this type of fracture-dislocation has also been reported to be near impossible to close reduce, with the majority requiring early open reduction and internal fixation to prevent complications and poor clinical outcomes. Reported early complications include compartment syndrome and soft tissue complications from repeated closed reduction attempts. Com

Stabilisation of the fractured fibula plays an important role in the treatment of pilon fractures: a retrospective comparison of fibular fixation methods.

Ninety-eight pilon fractures associated with ipsilateral distal fibular fracture were included in this study. The pilon fractures were treated by open reduction and plating. The 98 fractures were divided into three groups based on the treatment method of fractured fibula. Group A was composed of 50 fibular fractures treated by open reduction and plate fixation. Group B was composed of 23 fibular fractures treated by open reduction and pin fixation. Group C was composed of 25 fibular fractures treated conservatively by closed reduction. The radiographs were reviewed for adequacy of fracture red

Treatment of Displaced Femoral Neck Fractures in Children: Mid-Term Outcomes and Predictors of Failure.

BACKGROUND: Displaced femoral neck fractures in children are rare but carry significant risk for complications such as avascular necrosis (AVN), nonunion, and malunion. Although adult literature increasingly supports closed reduction when adequate alignment is achieved, the pediatric literature lacks consensus on optimal management. This study evaluates whether the quality of reduction, regardless of open or closed technique, predicts adverse outcomes in pediatric displaced femoral neck fractures. METHODS: We retrospectively reviewed 56 patients (ages 2 to 18) with displaced Delbet type II or

Outcome of open comminuted tibial plateau fractures treated using an external fixator.

BACKGROUND: The purpose of this study was to evaluate the outcome of open severe comminuted tibial plateau fractures treated with minimal internal fixation and circular external fixation frames. METHODS: Fifteen open comminuted tibial plateau fractures were involved in this retrospective study. In eight knees, the fracture was reduced through an extended open traumatic wound. In seven patients, another limited incision over the major fracture line or area of comminution was performed to assist in reducing the articular reduction. Simultaneous bone grafting, soft tissue reconstruction, and sta

Beyond conventional methods: The trans-trochanteric approach and the role of preoperative CT in optimal management of femoral head fracture-dislocation.

Hip fracture-dislocations are uncommon injuries that require substantial force and can be complicated by residual cartilaginous or bony fragments, making reduction challenging. Although immediate closed reduction is typically the initial treatment, open reduction becomes necessary when closed methods fail or in the presence of associated fractures or femoral head fractures. The long-term prognosis is influenced by the direction of dislocation and the severity of the injury, with young adults, particularly those in road traffic accidents, being at higher risk for complications such as osteoarth

Cementless acetabular reconstruction after acetabular fracture.

BACKGROUND: Total hip arthroplasty in patients with posttraumatic arthritis has produced results inferior to those in patients with nontraumatic arthritis. The use of cementless acetabular reconstruction, however, has not been extensively studied in this clinical context. Our purpose was to compare the intermediate-term results of total hip arthroplasty with a cementless acetabular component in patients with posttraumatic arthritis with those of the same procedure in patients with nontraumatic arthritis. We also compared the results of arthroplasty in patients who had had prior operative treat

Single-surgeon perspective: is there ever a need to open extension-type supracondylar fractures?

BACKGROUND: Supracondylar humerus fractures (SCHFs) represent the most common pediatric elbow fracture, constituting approximately 12-17% of all pediatric fractures. The vast majority of operative supracondylar humerus fractures are treated with closed reduction and percutaneous pinning (CRPP); however, the estimated rate of SCHFs requiring open reduction is approximately 12.7%. AIM: This study aims to analyze the likelihood of open reduction in pediatric extension-type SCHFs and to reaffirm the traditional teaching of reduction techniques described by Smith and Rang. METHODS: A single-surge

Nonunion of Hoffa Fracture Successfully Treated by Total Knee Arthroplasty: A Case Report.

A Hoffa fracture is a rare type of femoral fracture that is difficult to treat. Nonoperative treatments usually result in failure; hence, in most cases, surgical treatments are essential. Nonunion following Hoffa fracture appears to be relatively uncommon, and there are limited reports in the literature about this type of nonunion. These reports suggest that open reduction and rigid internal fixation is the standard treatment for this type of nonunion. This study reports the case of a 61-year-old male patient who suffered from left lateral Hoffa fracture after falling from a truck bed. At the
