The facial nerve (CN VII) emerges from the facial nerve nucleus in the pons. It is accompanied by CN VIII along its cisternal pathway, as well as at the internal auditory meatus. Its petrous pathway includes a labyrinthine segment, a horizontal tympanic segment and a vertical mastoid segment until the stylomastoid foramen. It then continues to the parotid gland. Pontine impairment is usually associated with other neurological symptoms. Lesions of the cerebellopontine angle (most often meningioma and schwannoma) initially result in impairment of CN VIII. The impairment of CN VII takes second pl…

A 47-year-old woman presented with an extremely rare case of trochlear nerve neurinoma manifesting as left hemiparesis. Magnetic resonance imaging demonstrated a cystic tumor, 4 cm in diameter, in the right ambient and cerebellopontine cisterns, compressing the midbrain and upper pons. The preoperative diagnosis was trigeminal neurinoma. However, the tumor was identified arising from the trochlear nerve, and was totally removed via the posterior transpetrosal approach. The histological diagnosis was neurinoma. The patient's hemiparesis improved postoperatively. Preoperative diagnosis of trochl…

We report the case of a 64-year-old man with sudden onset of numbness in the right hand and foot. Neurological examinations were normal except for hypersthesia, and hyperalgesia of the right hand and foot. Brain MRI demonstrated a high signal intensity on T2-weighted image and a low signal intensity on T1-weighted image in the left tegmetum of the pons. He was diagnosed with pontine infarction presenting with cheiro-pedal syndrome (CPS). Damage in the sensory pathways can cause CPS. Difference in the threshold may explain the specific sensory pattern in this syndrome. Further examination of th…

Listeria neuromeningitis presents as febrile rhombencephalitis in half the patients, often with persistent sequelae such as cranial nerve palsy. We report the case of a man who developed central apnea with simultaneous consciousness impairment subsequent to an infectious disease. Magnetic resonance imaging showed low signal intensities in T1 and T2, particularly in the medulla. Neither acidosis, hypoxia nor hypercapnea stimulated the respiratory centers during episodes of apnea; ventilation started again only with strong voluntary stimulation. Pons and medulla lesions of the reticular formatio…

In 58 cases with central pontine myelinolysis (CPM) and/or extrapontine myelinolysis, systematic examination of the central nervous system was performed. The demyelinating disease occurred in three subtypes: (1) CPM, in which the lesion was confined to the pons, (2) CPM combined with extrapontine myelinolysis and (3) exclusively extrapontine myelinolysis. Type (1) was found in 27 cases, (2) in 18 cases and (3) in 13 cases. Cerebellum and lateral geniculate body were the most frequently affected extrapontine regions. One case with an extreme extension of the lesions is described in detail. Extr…

The brains of rats infected with Trypanosoma brucei gambiense were analyzed for their content of dopamine, 3,4-dihydroxyphenylacetic acid (DOPAC), serotonin, and 5-hydroxyindoleacetic acid (5-HIAA). In the caudate of infected rats, dopamine levels increased by 34% and 5-HIAA levels by 20%. DOPAC levels declined by 30% in the caudate, but this change was significant at only P less than 0.1. In the pons/medulla of infected rats the serotonin concentration dropped by 21% and the 5-HIAA concentration rose by 25%. Activity levels in infected rats decreased by 49% (gross movement) and 44% (total mov…

A 21-year-old man with a longstanding history of impaired visual acuity, strabismus, broad-based gait, and below-average intellectual capacity developed respiratory difficulties and intermittent generalized weakness at age 19. He subsequently showed signs of massive brainstem dysfunction and died. Postmortem examination demonstrated changes compatible with subacute necrotizing encephalomyelopathy of Leigh involving portions of the diencephalon, midbrain, pons, and medulla as well as portions of the spinal cord and optic nerves. Evaluation of family members for presence of the urinary inhibitor…

OBJECTIVES: To investigate the effect of orally administered cytidine-5'-diphosphocholine (citicholine) (50,100,200 mg/kg), α-tocopherol (Vit E; 25 mg/kg), caffeine (10 mg/kg), L-dopa (25 mg/kg) or the combination of Vit E, caffeine with citicholine (100 mg/kg) on nigrostriatal neuronal damage induced in the mice brain by subcutaneous (s.c.) rotenone.
METHODS: Swiss male mice received rotenone (1.5 mg/kg, s.c, three times per week) alone or with other drugs for 2 weeks. Mice were evaluated for brain malondialdehyde (MDA), reduced glutathione (GSH), and nitric oxide (NO), paraoxonase-1 (PON-1)

Teucrium leucocladum is among the most used traditional medicinal plants in Palestine, which is used for the treatment of hyperglycemia and colon spasms from ancient times. Therefore, the current investigation aimed for the first time to determine the hypoglycemic, hypolipidemic, and oxidative stress inhibitory effects of the aerial parts (stem and leaves) of T. leucocladum hydrophilic (water) extract in streptozotocin- (STZ-) induced diabetic rats (65 mg/kg), given intraperitoneally at a dose of 100 mg/kg for 21 days. The rats were divided into four groups as control (C), control + T. leucocl

PURPOSE: We aimed to study the oxidative damage induced by radiofrequency electromagnetic radiation (RF-EMR) emitted by mobile telephones and the protective effect of garlic extract used as an anti-oxidant against this damage.
MATERIALS AND METHODS: A total of 66 albino Wistar rats were divided into three groups. The first group of rats was given 1.8 GHz, 0.4 W/kg specific absorption rate (SAR) for 1 h a day for three weeks. The second group was given 500 mg/kg garlic extract in addition to RF-EMR. The third group of rats was used as the control group. At the end of the study, blood and brain

The pons is the most common site for brainstem cavernous malformations.1-3 Lesions in this region seem to be more aggressive than superficial lesions and often present unique surgical challenges due to their depth and proximity to eloquent structures.1,4-7 The cerebellar-mesencephalic fissure and the cerebellar interpeduncular region7,8 serve as critical microsurgical corridors for approaching intrinsic lesions of the posterolateral pons.8-13 However, their access is limited by surrounding structures such as the superior and middle cerebellar peduncles, dentate nucleus, and pontine fiber tract…

BACKGROUND: Although Parkinson's disease (PD) and Parkinsonism-Plus syndromes (PPS) exhibit distinct brain atrophy, current clinical differentiation heavily relies on manual, plane-specific 2D metrics, which are labor-intensive and observer-dependent. To fill this gap, this study developed an automated, software-independent metric-the parkinsonism volumetric index (PDSVI)-calculated as [(pons volume × third ventricle volume)/(midbrain volume × thalamus volume) × 10].
PURPOSE: To evaluate the performance of PDSVI in precisely differentiating progressive supranuclear palsy (PSP) from PD, multip…

The diagnosis of ocular motor disorders and the different forms of a nystagmus is based on a systematic clinical examination of all types of eye movements: eye position, spontaneous nystagmus, range of eye movements, smooth pursuit, saccades, gaze-holding function, vergence, optokinetic nystagmus, as well as testing of the function of the vestibulo-ocular reflex (VOR) and visual fixation suppression of the VOR. Relevant anatomical structures are the midbrain, pons, medulla, cerebellum, and cortex. There is a simple clinical rule: vertical and torsional eye movements are generated in the midbra…
