Postural tachycardia syndrome in a 28-year-old Japanese woman.

Postural tachycardia syndrome is defined as the development of orthostatic symptoms without orthostatic hypotension. We report a 28-year-old female patient with postural tachycardia syndrome who exhibited palpitation, low-grade fever and weight loss. Evaluation of autonomic nervous system functions showed that cardiovagal function was normal. Sweat response to acetylcholine was decreased. Excessive blood pressure elevation was seen in phase IV of the Valsalva maneuver. Pathophysiologic factors in this case were considered to be alpha adrenergic denervation and beta adrenergic hyperresponsivene

Automatic postural response systems in individuals with congenital total blindness.

This study examined the effects of the absence of vision from birth on automatic postural responses to platform displacements during stance. Postural responses were induced by producing randomly four types of perturbations which consisted of forward and backward translations, and toe up and down rotations. Nine congenitally totally blind and nine sighted adults served as subjects. EMG signals were recorded from four muscles in the right leg, and reaction time to somatosensory stimuli generated by platform displacements was measured by pushing a hand-held button. To assess the ability to contro

Visual vertigo: symptom assessment, spatial orientation and postural control.

Certain patients with balance disorders report a 'visual vertigo' in which their symptoms are provoked or aggravated by specific visual contexts (e.g. supermarkets, driving or movement of objects). In order to determine the causes of visual vertigo (VV), we assessed symptoms, anxiety and the influence of disorienting visual stimuli in 21 such patients. In 17 out of 21 patients, a peripheral vestibular disorder was diagnosed. Sixteen bilateral labyrinthine-defective subjects (LDS) and 25 normal subjects served as controls. Questionnaire assessment showed that the levels of trait anxiety and chi

Postural arm control following cervical spinal cord injury.

This study used estimates of dynamic endpoint stiffness to quantify postural arm stability following cervical spinal cord injury (SCI) and to investigate how this stability was affected by functional neuromuscular stimulation (FNS). Measurements were made in the horizontal plane passing through the glenohumeral joint on three SCI-impaired arms, which ranged in functional level from a weak C5 to a strong C6. Endpoint stiffness, which characterizes the relationship between externally imposed hand displacements and the resultant forces, was estimated during the application of planar, stochastic p

Postural tachycardia syndrome in syringomyelia: response to fludrocortisone and beta-blockers.

Orthostatic intolerance is occasionally reported by patients with syringomyelia and is usually attributed to vestibular symptoms or neurogenic orthostatic hypotension. Postural tachycardia syndrome has not been previously described in syringomyelia. A patient with long-standing syringomyelia and a Chiari type I anomaly developed disabling "panic-like" attacks associated to orthostatic intolerance five years after posterior fossa decompression and shunting of the syrinx. A head-up tilt test showed an early phase of postural orthostatic tachycardia followed by progressive arterial hypotension an

Postural behaviour responses to visual stimulation in patients with vestibular disorders.

Patients with different vestibular disorders exhibit changes in postural behaviour when they receive visual stimuli, reproducing environmental stimulation. Postural control was studied using an AMTI Accusway platform, measuring the confidential ellipse (CE) and sway velocity (SV). Postural responses were recorded according to the following stimulation paradigm: i) without specific stimuli; ii) smooth pursuit with pure sinusoids of 0.2 Hz (foveal stimulation); and iii) optokinetic stimulation (retinal stimuli). Patients with central vestibular disorders (CVD), cerebellar damage and unilateral p

[Epley and Semont maneuvers in the treatment of bening paroxymal postural vertigo].

To compare the effectiveness of the 'repositioning' Epley maneuver and the 'liberatory' Semont maneuver in the treatment of benign paroxysmal positional vertigo (BPPV) of the posterior semicircular canal, a prospective study was performed, with 3 months of followup. A consecutive sample of 100 patients was included in two groups (age-and-sex matched) with a similar number of patients with idiopathic BPPV in each group. Group I was treated using the 'repositioning' maneuver and group II, the 'liberatory' maneuver. At weeks 1, 4, and 12 during the study, the proportion of patients without positi

[Postural technique in prone position: hemodynamic and respiratory parameters and complications].

Therapeutic strategies used in the treatment of adult respiratory distress syndrome (ARDS) recommend placing the patient in prone position as an effective method for optimizing ventilation-perfusion parameters. We evaluated the therapeutic effect of postural treatment in prone position with the following goals: Comparison of hemodynamic and respiratory parameters before and after placing the patient in prone position, while in prone position, and before and after postural treatment. Complications associated with turning and time in prone position. A prospective study was made of 30 turns in 15

Postural instability and motion sickness in a fixed-based flight simulator.

We evaluated the prediction that postural instability would precede the subjective symptoms of motion sickness in a fixed-base flight simulator. Participants sat in a cockpit in a video projection dome and were exposed to optical flow that oscillated in the roll axis with exposure durations typical of flight simulation. The frequencies of oscillation were those that characterize spontaneous postural sway during stance. Head motion was measured prior to and during exposure to imposed optical flow. Of 14 participants, 6 were classified as motion sick, either during or after exposure to the optic

Postural compensation after intratympanic gentamicin treatment of Menière's disease.

We have treated 93 patients with severe Menière's disease with gentamicin (GM) applied with 1 to 4 or intratympanic injections in a prospective study. The patients were tested at frequent intervals and followed up for two years. Postural stability was evaluated on posturography. Unsteadiness and gait between attacks, working capacity and vertigo were scored. Before commencing the treatment 47 of the patients complained from moderate to severe postural instability. After treatment moderate to severe postural instability was met in 19 patients. Initially the sway velocity was 27 mm/s on average

Synergistic influences of the striatum and the globus pallidus on postural and locomotor control.

We have investigated the role of the globus pallidus in locomotor and postural control in a previously established animal model of striatal dysfunction. Striatal efferent activity was suppressed by intracerebral infusions of antisense oligodeoxynucleotides targeted to the messenger RNA of the proto-oncogene, c-fos. This suppression produced robust circling behavior and an atypical expression of c-fos in the ipsilateral globus pallidus following psychostimulant challenge. Simultaneous infusions of antisense oligodeoxynucleotides into both the caudate-putamen and the ipsilateral globus pallidus

Integration of the hand in postural reactions to sustained sideways force at the pelvis.

In order to investigate the potential hand contribution to sideways balance, sideways pushes to the right, which subjects resisted using either the lower limbs ("hip only") or the lower limbs assisted by the right upper limb ("hand and hip"), were delivered to the pelvis. Analysis of force and electromyogram recordings from the legs, arm and hand in the hand-and-hip condition showed a close co-ordination of upper and lower limbs in terms of mean latencies and amplitudes. However, trial to trial fluctuations of forces generated by the hand and leg did not correlate, suggesting parallel pathways

Postural set for balance control is normal in Alzheimer's but not in Parkinson's disease.

BACKGROUND: It has been suggested that patients with dementia of the Alzheimer type have abnormalities in the basal ganglia, and thus, may have similar sensorimotor problems as patients with basal ganglia degeneration from Parkinson's disease. Whether the similarity extends to balance control is unknown. One distinguishing feature of balance disorder in Parkinson's disease is difficulty with changing postural set in terms of adapting the amplitude of leg muscle activity as a function of support condition. We, therefore, tested whether patients with Alzheimer's disease without extrapyramidal si

Clinical and laboratory indices that enhance the diagnosis of postural tachycardia syndrome.

OBJECTIVE: To identify clinical and laboratory indices that improve the diagnosis of the postural tachycardia syndrome (POTS). DESIGN: We assessed associations of orthostatic intolerance by using multivariate regression analysis. MATERIAL AND METHODS: We evaluated autonomic symptoms and autonomic function in 30 patients with POTS, 30 patients with mild orthostatic intolerance, and 19 age- and gender-matched control subjects. Indices of parasympathetic and sympathetic functions were analyzed on the basis of (1) autonomic function tests (head-up tilt), (2) oscillations at respiratory and nonre

Prenatal exposure to alcohol affects the ability to maintain postural balance.

Prenatal exposure to alcohol is known to affect gross motor functioning. Animal studies have shown that balance is particularly affected, and there is some evidence that similar deficits exist in alcohol-exposed children. In the current study, postural balance, or the ability to maintain equilibrium, was assessed in a group of alcohol-exposed children (ALC group; n = 11) and controls (NC group; n = 11) individually matched for age and sex. Balance was measured across six conditions designed to systematically manipulate or eliminate visual or somatosensory information. Equilibrium and strategy

Evaluating sensory conflict and postural instability. Theories of motion sickness.

Two experiments were carried out to evaluate the sensory conflict and the postural instability theories of motion sickness. The central hypothesis of sensory conflict theory is that motion sickness is caused by conflict between the current pattern of sensory inputs about self-movement and the pattern that is expected on the basis of previous experience. A subsidiary hypothesis is that the degree of motion sickness is proportional to the magnitude of sensory conflict. The central hypothesis of postural instability theory is that motion sickness is caused by loss of postural control. A subsidiar

Postural vasoconstriction and leg ulceration in homozygous sickle cell disease.

1. Chronic leg ulceration is a major cause of morbidity in patients with homozygous sickle cell disease; the ulcers commonly resolve on bed rest. We have therefore compared the cutaneous vascular response to dependency in three groups of eight patients with sickle cell disease (those with an active ulcer, with an ulcer scar and with no history of ulceration) and in eight subjects with normal haemoglobin and no history of leg ulceration. 2. We monitored, with a laser Doppler flowmeter, the change in red cell (erythrocyte) flux induced in the skin of the leg, at two sites proximal to the malleol

Postural hypotension in a patient with cervical myelopathy due to craniovertebral anomaly.

We report a patient with craniovertebral anomaly leading to cervical cord compression who presented with disabling postural hypotension. A 60-year-old electrician presented with progressive weakness of the upper and lower limbs, which had started 7 years previously. He had difficulty in holding urine for the previous year and had blacked out on standing for the past 3 months. He had upper limb wasting and lower limb spasticity, with impaired joint position sense. Autonomic dysfunctions included postural hypotension, absence of sinus arrhythmia, impaired Valsalva ratio, and lack of increase in

Postural instability in diabetic neuropathic patients at risk of foot ulceration.

Diabetic peripheral neuropathy is believed to cause postural instability due to abnormal proprioception. We assessed body sway in four groups, each of 20 subjects, matched for age, sex, and BMI: non-diabetic controls, non-neuropathic diabetic controls, subjects with diabetic neuropathy and no history of foot ulceration, and subjects with diabetic neuropathy and a history of foot ulceration. Postural sway was assessed on a Kistler force plate using the Romberg test, measuring the standard deviation of the centre of pressure in both sagittal (antero-posterior movement) and frontal (side to side
