Abdominal aortic aneurysm repair complicated by infection with Clostridium septicum.

Aortic stent-graft infection after endovascular abdominal aortic aneurysm (AAA) repair is an uncommon, but very serious complication with potentially devastating consequences.(1) Traditional open techniques of repair of AAA demonstrate an infection rate of 0.5-3%. The exact rate of infection with endovascular repair is unknown, but literature review demonstrates an overall incidence of 0.43-1.17% retrospectively.(2,3) Etiology of endovascular graft infections typically results from flora derived from the skin or gastrointestinal tract.(4)Clostridium septicum is a naturally occurring anaerobic

Antimicrobial treatment of Brucella-infected prosthetic knee with sinus tract: A case report.

RATIONALE: Brucella infection of prosthetic joints is exceedingly rare. Current literature indicates that cases complicated by abscess and sinus tract, but without prosthetic loosening, have uniformly been managed with either debridement, antibiotics, and implant retention or revision arthroplasty combined with antimicrobial therapy. These approaches impose a significant economic burden on healthcare systems. Herein, we present a case of Brucella-infected prosthetic knee with sinus tract formation and radiographically stable components that was successfully cured with antimicrobial therapy alo

Low Driveline Infection Rates in Patients With a Novel Fully Magnetically Levitated Ventricular Assist Device.

OBJECTIVES: Driveline infection remains a major complication of long-term ventricular assist device therapy. Previous studies have associated the risk of infection with driveline design and physical properties. The CH-VAD is a fully magnetically levitated pump with an ultrathin (diameter 3.3 mm), highly flexible driveline designed to reduce infection risk. This study was conducted to assess the incidence, risk factors, and approaches to prevention and management of driveline infection in CH-VAD patients (NCT06878456). METHODS: All patients who underwent CH-VAD implantation between June 2017 a

Arthroscopy Within Three Months Prior to Unicompartmental Knee Arthroplasty Is Associated With an Increased Rate of Periprosthetic Joint Infection.

BACKGROUND: Arthroscopy remains a commonly performed procedure in patients who have meniscal or other osteochondral pathology prior to ultimately being indicated for unicompartmental knee arthroplasty (UKA). The purpose of this study was to examine the timing of knee arthroscopy prior to UKA and its association with 2-year periprosthetic joint infection (PJI) rates and medical and surgical complications. METHODS: Patients undergoing UKA who had history of ipsilateral arthroscopy within 2 years prior to the UKA were identified in a national insurance database. A control cohort of propensity ma

Patient Characteristics, Microbiology, and Mortality of Infective Endocarditis After Transcatheter Aortic Valve Implantation.

BACKGROUND: Infective endocarditis (IE) after transcatheter aortic valve implantation (TAVI) is associated with high mortality and surgery is rarely performed. Thus, to inform on preventive measures and treatment strategies, we investigated patient characteristics and microbiology of IE after TAVI. METHODS: Using Danish nationwide registries, we identified patients with IE after TAVI, IE after non-TAVI prosthetic valve (nTPV), and native valve IE. Patient characteristics; overall, early (≤12 m), and late IE (>12 m) microbiology; and unadjusted and adjusted mortality were compared. RESULTS: W

Outcomes after antibiotic megaspacer implantation in patients with severe chronic periprosthetic infections.

Periprosthetic joint infection (PJI) is a rare postoperative complication that is treated with antibiotic spacers. Some patients develop severe, treatment-resistant, chronic PJI despite multiple attempts at salvaging the joint. Permanent resection of the joint or amputation may be the only definitive treatment. The purpose of this study is to describe the outcomes, infection resolution rate, and complications of two-stage revision, utilizing extensive resection of the affected bone and application of antibiotic megaspacers as a modality for limb-salvage. A review of 12 patients, initially refe

Candidal Prosthetic Hip Infection in a Patient with Previous Candidal Sepsis - A Case Report.

A 60 year-old woman with hip dysplasia battled with duodenal cancer that was complicated with Candida tropicalis sepsis. Two years later, the patient underwent a total hip arthroplasty (THA). She complained of a persisting low-grade fever and local heat on the THA scar. Arthrocentesis of the hip was performed and the Candida tropicalis was detected. Debridement and polyethylene liner/modular head exchange were performed 28 days after the primary THA. Fluconazole was administrated for one year. The patient reported no symptoms five years later. It was found that periprosthetic infection could b

What Is the Role of Repeat Aspiration in the Diagnosis of Periprosthetic Hip Infection?

BACKGROUND: The American Academy of Orthopedic Surgeons clinical practice guideline currently recommends repeat joint aspiration when workup of periprosthetic joint infection (PJI) reveals conflicting data. This guideline is based on a single study of 31 patients published 25 years ago. We sought to determine the correlation between first and second aspirations and factors that may play a role in variability between them. METHODS: Sixty patients with less than 90 days between aspirations and no intervening surgery were identified at our institution and classified by Musculoskeletal Infection

Surgical Case Order Has an Effect on the Risk of Subsequent Periprosthetic Joint Infection.

BACKGROUND: Periprosthetic joint infection (PJI) after total joint arthroplasty (TJA) is a serious complication with multiple etiologies. Prior spine literature has shown that later cases in the day were more likely to develop surgical site infection. However, the effect of case order on PJI after TJA is unknown. This study aims to determine the influence of case order, prior infected case, and terminal cleaning on the risk for a subsequent PJI. METHODS: A retrospective, single-institution study was conducted on 31,499 TJAs performed from 2000 to 2014. Case order was determined by case start

What is the Diagnostic Accuracy of Aspirations Performed on Hips With Antibiotic Cement Spacers?

BACKGROUND: Periprosthetic joint infection is a serious complication after THA and commonly is treated with a two-stage revision. Antibiotic-eluting cement spacers are placed for local delivery of antibiotics. Aspirations may be performed before the second-stage reimplantation for identification of persistent infection. However, limited data exist regarding the diagnostic parameters of synovial fluid aspiration with or without saline lavage from a hip with an antibiotic-loaded cement spacer. QUESTIONS/PURPOSES: We asked: (1) For hips with antibiotic cement spacers, does saline lavage influenc

Projected Increase in Periprosthetic Joint Infections Secondary to Rise in Diabetes and Obesity.

Total joint arthroplasties (TJAs) are projected to increase, along with increased complications like periprosthetic joint infections (PJIs). However, no study has projected TJAs secondary to PJI and how these relate to comorbidities in these patients. The NIS database was assessed from 1993 to 2000 to evaluate primary and revision TJAs, and projections were made until 2028. ICD-9 codes were used to identify patients with obesity, diabetes, and PJI. Rates of diabetes and adult obesity are predicted to increase for both genders, and patients receiving revision surgery for PJI are predicted to in

Native and prosthetic valve infective endocarditis: clinicopathologic correlation and review of the literature.

BACKGROUND: The incidence of infective endocarditis is 1.5-4.95 cases per 100,000 individuals per year, with a mortality of 14-46% 1-year post infection. The management and decision to operate on selected patients remains controversial. Our study reviews cases of native and prosthetic valve endocarditis in a surgical population, in an attempt to identify and compare clinical and microbiologic features between the two groups. In addition, we compared our findings with other published series to identify if there are changes with these parameters over time. METHODS: A retrospective analysis of p

Infective endocarditis: an update on the role of echocardiography.

The evaluation of valvular and nonvalvular structures is of central importance in the diagnosis, management, and treatment of infective endocarditis (IE). The incidence of IE has remained constant due to changing substrate, with notably higher prevalence observed in the elderly. Mortality and morbidity continue to remain high, despite advances in medical and surgical treatment. This article reviews the technical and practical aspects of the use of echocardiography to evaluate patients with suspected IE.

Large-diameter metal-on-metal cementless total hip arthroplasty in the elderly.

To evaluate the clinical effectiveness and complications of large-diameter metal-on-metal prostheses, particularly in the elderly, we performed cementless total hip arthroplasty (THA) using metal-on-metal prostheses with large-diameter femoral heads (mean, 45 mm) in 59 patients (average age, 75.1 years) between January 2004 and December 2005. All procedures were performed using a posterolateral approach and spinal anesthesia. Average follow-up was 65 months. Pre- and postoperative Harris Hip Scores and SF-36 questionnaire results were recorded for all patients to evaluate the level of pain rel

[In situ replacement with cryopreserved homografts for infected aortic prostheses].

Although the incidence is rare, aortic prosthetic graft infection represents one of the most serious and challenging complications for cardiovascular surgeons. The basic strategy in the management of aortic graft infection consists of excision of the infected graft, debridement, reconstruction of the distal blood supply, and prolonged antibiotic therapy. As the most reliable procedure, in situ replacement with cryopreserved allografts has been used in our institution since 1998. The overall in-hospital mortality was 43%, higher than in other reports, because of a higher rate of thoracic cases

Aortic valve endocarditis: is there an optimal surgical timing?

PURPOSE OF REVIEW: Patients with aortic valve infective endocarditis are likely to undergo surgery during the active phase of the disease. The indication and best timing for surgery, however, are still debated. The present review discusses the benefits and risks of early surgery in aortic endocarditis. RECENT FINDINGS: Patients with acute aortic regurgitation and clinical or echocardiographic signs of poor tolerance require urgent surgery. Other indications for early surgery include severe perivalvular involvement and high embolic risk. Echocardiography plays an important role in the assessme

Corporoscopic excavation of the fibrosed corpora cavernosa for penile prosethesis implantation: optical corporotomy and trans-corporeal resection, Shaeer's technique.

INTRODUCTION: Implantation of a penile prosthesis in cases of neglected or resistant ischemic priapism, or delayed re-implantation following prosthesis infection and extraction, is usually a difficult and risky procedure due to fibrosis of the corpora cavernosa. Among the common complications are perforation of the urethra, tunica albuginea, and infection. The complications are usually due to the use of blind force against resistance. AIM: We propose the techniques of Trans-Corporeal Resection and Optical Corporotomy as adjuvant measures for excavating the fibrosed corpora cavernosa under vis
