DeCoP, a Dermatitis Control Program using a moderately absorbent surgical pad for head and neck cancer patients receiving radiotherapy: a retrospective analysis.

OBJECTIVE: We evaluated the clinical characteristics of a Dermatitis Control Program based on a moderately absorbent surgical pad for head and neck cancer patients undergoing (chemo)radiotherapy. METHODS: We retrospectively reviewed patients who underwent definitive radiotherapy or post-operative radiotherapy and were treated during radiotherapy with a Dermatitis Control Program using a moderately absorbent surgical pad from May 2011 through April 2012. The main protocol was the 'Dermatitis Control Program', a systematic program which consists of a three-step ladder. When radiation dermatitis

Acute skin reaction suggestive of pembrolizumab-induced radiosensitization.

The combination of localized radiotherapy and immune checkpoint inhibitors represents a promising therapeutic strategy for various cancers, including metastatic melanoma. Radiation therapy may enhance tumor antigen presentation and cytokine release, which may optimize the systemic antitumor immune response induced by these immunotherapeutic antibodies, with a potential delayed abscopal effect. However, clinical experience of using immune checkpoint inhibitors with concurrent radiotherapy remains scarce. We report here for the first time a case suggestive of acute skin radiosensitization induce

Clinical Significance of Accounting for Tissue Heterogeneity in Permanent Breast Seed Implant Brachytherapy Planning.

PURPOSE: The inhomogeneity correction factor (ICF) method provides heterogeneity correction for the fast calculation TG43 formalism in seed brachytherapy. This study compared ICF-corrected plans to their standard TG43 counterparts, looking at their capacity to assess inadequate coverage and/or risk of any skin toxicities for patients who received permanent breast seed implant (PBSI). METHODS AND MATERIALS: Two-month postimplant computed tomography scans and plans of 140 PBSI patients were used to calculate dose distributions by using the TG43 and the ICF methods. Multiple dose-volume histogra

Mild soaps and radiotherapy: a survey of the UK public to identify brands of soap considered mild and analysis of these to ascertain suitability for recommendation in radiotherapy departments.

Cancer agencies recommend that patients use mild soap when undergoing external beam radiotherapy to minimise skin reactions. They define 'mild soap' as non-alkaline, lanolin free, unperfumed soap with a neutral pH. This study aimed to identify which soaps the UK public perceive as mild and ascertain if these were clinically mild and could potentially be recommended within radiotherapy departments. A survey of 237 participants identified eight top brands of mild soap, which were then tested for pH and analysed for potential irritants. All soaps were lanolin free and non-alkaline, with Simple an

Reconstruction of extensive posterior mid-thoracic soft-tissue defects after spinal surgery on irradiated skin.

A 56-year-old patient presented with neurological disorders resulting from an extrinsic medullary compression extending from T3 to T5 associated with a T4 corporeal invasion by a high-grade non-Hodgkin's malignant lymphoma. Treatment consisted in a laminectomy without spinal stabilization followed immediately by chemotherapy (VCAD) and locoregional radiotherapy (20 cycles of 50 Gy between T3 and T7 followed by nine sequences of 18 Gy). The evolution revealed unfavourable with reappearance of a compressive syndrome thus requiring surgical decompression combined with spinal stabilization. The im

Oral and maxillofacial side effects of radiation therapy on children.

Radiation therapy of the head and neck frequently results in serious and sometimes unavoidable changes to orofacial structures, particularly for children. Acute and chronic complications have a great impact on their oral function and quality of life. This paper provides an overview of the side effects of radiation therapy on children's oral and dental tissues, and highlights appropriate preventive guidelines and management strategies to minimize these complications.

Gemcitabine-induced rectus abdominus radiation recall.

CONTEXT: Radiation recall has been described in the context of gemcitabine chemotherapy. However, this phenomenon has been largely limited to skin. CASE REPORT: We hereby report a case of radiation recall dermatitis and myositis occurring on gemcitabine monotherapy, five months after completing chemoradiation for locally advanced pancreatic cancer. Radiation recall resolved spontaneously with withdrawal of gemcitabine. CONCLUSIONS: This is the second case report that describes gemcitabine-induced radiation recall in rectus abdominus muscles after gemcitabine-based radiation therapy. Given th

Amifostine reduces side effects and improves complete response rate during radiotherapy: results of a meta-analysis.

PURPOSE: To evaluate the efficacy of amifostine in diminishing radiotherapy side effects and whether or not it protects the tumor. METHODS AND MATERIALS: We performed a systematic review and meta-analysis of 14 included randomized controlled trials, comprising 1451 patients, comparing the use of radiotherapy vs. radiotherapy plus amifostine for cancer treatment. RESULTS: The use of amifostine significantly reduced the risk of developing mucositis (odds ratio [OR], 0.37; 95% confidence interval [CI], 0.29-0.48; p < 0.00001), esophagitis (OR, 0.38; CI, 0.26-0.54; p < 0.00001), acute xerostomia

Histological examination in assessment of ultraviolet-induced suppression of contact hypersensitivity response.

The evaluation of contact hypersensitivity (CHS) reaction is one of the methods used in the assessment of the immune status of an organism after UV radiation. The aim of the study was to compare usefulness of visual scoring system and histological morphometry in the assessment of CHS response after exposure of humans to solar simulated radiation (SSR). The study included 140 healthy volunteers, 33 people were irradiated for 2 days, 34 - for 10 days and 33 - for 30 days with SSR. Forty non-irradiated individuals served as controls. All the volunteers were sensitized with diphenylocyclopropenone

Gemcitabine-induced radiation recall dermatitis: case report.

A 65-year-old male with lung adenocarcinoma received radiotherapy to the mediastinum and right upper lobe, followed by chemotherapy with gemcitabine. Radiation recall dermatitis developed in the area corresponding to the radiotherapy portal. This is one of just a few cases reported recently concerning radiation recall dermatitis stemming from gemcitabine.

Common toxicity criteria: version 2.0. an improved reference for grading the acute effects of cancer treatment: impact on radiotherapy.

In 1997, the National Cancer Institute (NCI) led an effort to revise and expand the Common Toxicity Criteria (CTC) with the goal of integrating systemic agent, radiation, and surgical criteria into a comprehensive and standardized system. Representatives from the Radiation Therapy Oncology Group (RTOG) participated in this process in an effort to improve acute radiation related criteria and to achieve better clarity and consistency among modalities. CTC v. 2.0 replaces the previous NCI CTC and the RTOG Acute Radiation Morbidity Scoring Criteria and includes more than 260 individual adverse eve

[Scleroderma of the shoulders after treatment with combined bleomycin and radiotherapy].

INTRODUCTION: There are many causes of scleroderma. We observed a case of scleroderma involving the shoulder and neck area after combined bleomycin and radiotherapy. CASE REPORT: After six cycles of a CHOP-bleo protocol followed by radiotherapy for lymph node lymphoma, a female patient presented scleroderma involving the shoulders and neck area. Total bleomycin dose was 252 mg. DISCUSSION: Among the imputable factors, particularly extrinsic factors, in a series of 33 cases of different chemotherapy protocols published by Peters et al. in 1988, we retained the bleomycin radiotherapy administr

Dose to superficial node for patients with head and neck cancer treated with 6 MV and 60Co photons.

The superficial neck nodes in only 1 out of 7 patients with head and neck cancer studied received more than 90% of the prescribed dose when treated with opposed 6 MV photons. Beam spoilers placed upstream from the patient enhanced the dose to the superficial node at the expense of higher dose to the skin.

Radiation port dermatophytosis.

We report two cases in which dermatophytic infection developed almost entirely within a radiation field mimicking an acute radiation effect. Radiotherapists and dermatologists should be aware of this possibility and be able to differentiate it from radiation dermatitis. Topical antifungal agents are the recommended treatment after diagnosis is established.
