Cytomegalovirus infection after lung transplant: Incidence, risk factors, and management in a multicenter cohort.

Cytomegalovirus (CMV) infection after lung transplant is associated with chronic rejection, but risk factors besides serology are not well-defined. Utilizing a multicenter, prospective cohort, CMV infection risk factors during and after prophylaxis were analyzed with an extended Cox model, occurring either after completion of primary prophylaxis or for a breakthrough CMV episode, while on prophylaxis. The overall cohort included 785 lung transplant recipients, of which 221 developed at least 1 CMV episode. Of the 362 recipients eligible for the off-prophylaxis analysis, 124 had a CMV episode a

Endoscopic Bariatric Therapies in Inflammatory Bowel Disease: Patient Selection, Nutritional Risk, and Periprocedural Management.

Background/Objectives: Obesity is increasingly recognised as a clinically relevant comorbidity in patients with inflammatory bowel disease (IBD), with potential effects on inflammatory burden, treatment response, surgical risk, nutritional status, and quality of life. Endoscopic bariatric therapies (EBTs), including intragastric balloons and endoscopic sleeve gastroplasty, offer minimally invasive weight-loss options between pharmacotherapy and bariatric surgery. However, their role in IBD remains poorly defined. This narrative review examines patient selection, nutritional risk, periprocedura

Management strategies for isolated premature thelarche: a risk-stratified clinical pathway favoring "watchful waiting".

Isolated Premature Thelarche (PT) is a common clinical issue in pediatric endocrinology. Because its early presentation resembles that of Central Precocious Puberty (CPP), which requires intervention, its management is highly controversial, caught between "active intervention" and "watchful waiting." This leads to inconsistent clinical practices and significant anxiety for affected families. This review aims to move beyond this traditional dichotomy by conducting an in-depth analysis of the natural history, pathophysiological mechanisms, diagnostic criteria, and progression risks of PT. We pro

Mucous Membrane Pemphigoid After Anti-PD-1 Therapy: Risk-Stratified Management and Treatment Outcomes.

Mucous membrane pemphigoid (MMP) following anti-programmed cell death-1 (PD-1) therapy is rare but increasingly reported. Management of high- and low-risk MMP in this setting and the potential oncologic trade-offs remain poorly defined. We performed a narrative synthesis of all published cases of anti-PD-1-associated MMP, following MEDLINE, Embase, and PubMed Central searches from January 2014 to June 2025. Fifteen cases from thirteen reports met the inclusion criteria, with a median MMP latency period of 12.5 weeks. The oral mucosa was frequently involved and was the index site in 84.6% (11/1

Progression of potentially aggressive pituitary neuroendocrine tumors after radiotherapy: risk factors, management, and outcomes.

OBJECTIVE: Radiotherapy plays a relevant role in uncontrolled pituitary neuroendocrine tumors (PitNETs). Radiotherapy controls tumor progression in most cases, but not always. Prognostic factors for tumor progression after radiotherapy remain poorly defined. The aim was to evaluate tumor progression after radiotherapy, to identify risk factors, and to report management and outcomes in a cohort of PitNETs with uncontrolled progression. DESIGN: This was a retrospective, single-center, observational study. METHODS: In total, 123 consecutive patients who underwent radiotherapy for PitNETs and we

Discontinuation of Antithrombotic Management Following Left Atrial Appendage Occlusion in High Bleeding Risk Patients.

BACKGROUND: Antithrombotic therapy is required after left atrial appendage occlusion (LAAO) whilst endothelialization of the device occurs. LAAO patients are usually at high bleeding risk and the optimal antithrombotic regimen post-implantation remains debated. AIMS: We aimed to study the efficacy of using maximum of 6-month antithrombotic treatment post-LAAO in high bleeding risk patients with atrial fibrillation. METHODS: Data from patients who had LAAO at our center between 2015 and 2024 were examined. Those who were discharged on a plan of 6 months antithrombotic therapy were included. I

Chronic Inflammatory Diseases and Cardiovascular Risk: Current Insights and Future Strategies for Optimal Management.

Chronic inflammation is a pivotal driver in the progression of atherosclerosis, significantly contributing to the burden of cardiovascular disease (CVD). Patients with chronic inflammatory diseases, such as inflammatory bowel diseases (IBDs) (e.g., ulcerative colitis and Crohn's disease), rheumatological disorders, as well as individuals with auto-immune diseases (such as systemic lupus erythematosus), present a higher risk of major adverse cardiac events (MACEs). Despite their elevated CVD risk, these populations remain underrepresented in cardiovascular research, leading to a critical undere

Diagnosis, risk stratification, and early management of non-ST-segment elevation acute coronary syndrome.

Non-ST-segment elevation acute coronary syndrome (NSTE-ACS) represents a unique clinical syndrome, comprising non-ST-segment elevation myocardial infarction (NSTEMI) and unstable angina. NSTEMI, as the more common and serious form of NSTE-ACS, is particularly noteworthy because of its diverse clinical presentation, electrocardiogram changes, and angiography findings, which may pose challenges in diagnosis and treatment and may subsequently influence prognosis. This review offers a comprehensive overview of current evidence-based approaches to NSTE-ACS management, focusing on diagnosis, risk st

Information needs of women with BRCA mutations regarding cancer risk management and decision-making.

PURPOSE: Providing women who have tested positive for a pathogenic variant in BRCA1 or BRCA 2 relevant information can help them to make informed decisions about managing their cancer risk. However, there is a lack of targeted informational support for BRCA positive women specific to the Irish context. The objective of this study is to identify the information needs of women diagnosed with a pathogenic variant in BRCA1 or BRCA 2 regarding cancer risk management and decision-making. METHODS: This is a descriptive qualitative study. Participants were recruited using purposive sampling and inclu…
