BACKGROUND: The valgus-impacted (VI) 4-part fractures are a subset of fractures of the proximal humerus with a unique anatomic configuration characterized by a relatively lower incidence of avascular necrosis after operative intervention.
OBJECTIVES: The present study reports the midterm clinical and radiological results of a large series of consecutive patients with 4-part VI fractures treated with a minimal invasive technique of internal fixation.
METHODS: Over a ten-year period (2004-2014), we treated 56 patients with a true 4-part valgus impacted fracture of the proximal part of the hume

The shoulder joint is a commonly dislocated joint in the human body. Shoulder dislocations are commonly associated with fractures of greater tuberosity and neurovascular injuries and are rarely associated with venous complications like deep vein thrombosis (DVT). Ten percent of all venous thrombosis is related to upper extremity venous thrombosis (UE-VT). Severe UE-VT cases are associated with endovenous catheters. Moreover, UE-VT often occurs in specific medical conditions, including hematological malignancy, solid neoplasia, and progressive infection. UE-VT can lead to serious complications

Posterior dislocations are infrequent injuries of the proximal humerus, counting for 1-4% of all shoulder dislocations. Frequently, they are related to epileptic seisures or electric current injuries or accompany complete anatomical neck fractures of the proximal humerus. Very frequently, they remain undiagnosed and their late diagnosis results in poor functional outcomes. The authors assess the surgical procedure and its results in five cases of primarily undiagnosed posterior dislocations. In two cases, transposition of the m. subscapularis was performed, in one case hemiarthroplasty, and in

INTRODUCTION: There are few data available about how patients resume car driving after proximal humerus fracture. The aim of this study was to evaluate return to car driving after surgical treatment of proximal humerus fracture. Secondary aims were to identify factors associated with return to driving.
METHODS: This retrospective monocentric study included 155 patients (mean age 64.3 ± 11.7 years), operated on for a traumatic proximal humerus fracture. Among them, 59% were treated by ORIF by nail, 18% by ORIF by plate and 23% by a reverse shoulder arthroplasty. All patients had a driver's lic

INTRODUCTION: There is no controversy about the need for surgical treatment of the displaced surgical neck fractures of the humerus, but there are few studies comparing the results of the three preferred types of surgical treatment. To expand the knowledge needed in decision making, a patient series is reviewed using health related quality of life (HRQoL), functional and radiographic data from patients treated with percutaneous pinning, locking plates or intramedullary nails.

PURPOSE: The aim of this study was to radiologically evaluate the risk of reduction loss after locking plate fixation of proximal humerus fractures.
METHODS: From September 2007 to April 2009, 71 patients (28 males, 43 females) with unstable proximal humeral fracture were treated with open reduction and internal fixation by locking plate. The mean follow-up time was 31.2 months (range: 26-47). The head-shaft angulation (HSA) and the humeral head height (HHH) in true anteroposterior (AP) were recorded and compared over time. All complications were noted. Shoulder function was measured by the C

From 1982 to 1986, a total of 106 children with supracondylar fractures of the humerus were treated at the trauma surgery clinic in Braunschweig. Undisplaced or slightly displaced fractures were treated with plaster casts. More severely displaced fractures were initially treated by closed reduction under general anesthesia and percutaneous Kirschner wiring. In 72 cases (= 85.7%) a follow-up examination revealed a very good or good result. In 9 cases (= 10.7%) satisfactory results were found and in 3 cases (= 3.6%) unsatisfactory results. The most common complication prejudicing the result was

OBJECTIVE: To compare the difference between MIPO and IMN methods in the treatment of displaced clavicular mid-shaft fractures.
METHODS: A total 163 patients were consecutively enrolled and randomly divided into the MIPO group or the IMN group. Clinical characteristics and outcomes including age, gender, causes of fractures, and AO/OTA type as well as hospitalization time, healing time, and postoperative complications were analyzed. Constant-Murley scores and DASH scores were used for assessment of shoulder joint and upper extremity function, respectively.
RESULTS: In this study, 82 cases re

BACKGROUND: This study compared the outcomes and complications of single lateral plating vs. dual plating for treating displaced scapular body fractures.
METHODS: Open reduction and internal fixation using locking plates was performed in 45 patients with displaced scapular fractures. A single lateral plate fixed in the lateral border was used in 22 patients (group A), and dual plates fixed in both the lateral and medial borders were used in 23 (group B).
RESULTS: The average follow-up duration in both groups was 20 months. A remarkable difference was seen between the 2 groups in mean operati

BACKGROUND: Humeral head pseudosubluxation (HHPS) in relation to the glenohumeral joint is a common finding following fractures of the proximal humerus. The temporary inferior subluxation of the humeral head may be secondary to a transient axillary nerve neuropraxia, pain inhibition of the deltoid, or hemarthrosis or capsular disruption that alters the physiologically negative pressure in the glenohumeral joint. Despite the frequency of this finding, it is not well described in the literature. This study sought to describe the incidence, risk factors, and rate of resolution of HHPS following p

BACKGROUND: The optimal technique for the displaced greater tuberosity (GT) fractures remains unclear; those in favor of arthroscopic techniques emphasize on the feasibility of arthroscopic reduction and fixation, while others report that anatomic reduction and osteosynthesis of the fracture are optimal through open surgery. Therefore, we performed this study to evaluate the clinical results of arthroscopic fixation for displaced and/or comminuted GT fractures using a bridging arthroscopic technique.
MATERIALS AND METHODS: We studied the files of 11 patients (4 men, 7 women; mean age, 55 year

CASE: Reported here is a single case of a highly comminuted fracture of the proximal aspect of the humerus with substantial humeral head displacement and a concomitant glenoid fracture encompassing >20% of the glenoid width. The fractures were treated simultaneously with reverse total shoulder arthroplasty and open reduction and internal fixation, respectively.
CONCLUSION: The 1-year follow-up demonstrated excellent pain control and a good functional outcome, with no signs of instability or implant complications.

INTRODUCTION: Frozen shoulder is a common condition, and current guidelines state that it is a diagnosis of exclusion. Along with a history and clinical examination, routine X-ray is mandated, to rule out any "masquerading" pathology such as fracture, dislocation, metastatic lesions or severe osteoarthritis. Despite the certainty of the guidelines there is a lack of evidence to support the use of routine X-rays in this situation.
METHODS: A retrospective review was performed of all X-rays obtained in the diagnosis of frozen shoulder between February 2014 and August 2017 in an integrated muscu

BACKGROUND: The lack of greater tuberosity (GT) healing in proximal humerus fractures has been negatively correlated with outcomes for hemiarthroplasty; however, there is still debate regarding the effects of GT healing in reverse shoulder arthroplasty (RSA). Our goal was to examine the effects of GT healing using a kinematic finite element analysis (FEA) model.
MATERIAL AND METHODS: Computer-aided design models of a medialized glenoid with a lateralized humerus (MGLH) RSA design were uploaded into an FEA shoulder model in 2 different configurations: healed greater tuberosity (HGT) and nonuni

Humeral hemiarthroplasty, anatomic total shoulder arthroplasty, and reverse total shoulder arthroplasty all rely on a prosthetic articular surface fixed to the proximal humerus. Humeral implant designs have changed considerably as a result of improved understanding of proximal humeral anatomy and prosthetic biomechanics. Fixed, monoblock implants have been superseded by modular implants with variable inclination, offset, version, and stem length. Press-fit designs now commonly have surface coatings that allow bony ingrowth. Metaphyseal fixation is often favored over diaphyseal fixation. Both c

American football is a collision sport played by athletes at high speeds. Despite the padding and conditioning in these athletes, the shoulder is a vulnerable joint, and injuries to the shoulder girdle are common at all levels of competitive football. Some of the most common injuries in these athletes include anterior and posterior glenohumeral instability, acromioclavicular pathology (including separation, osteolysis, and osteoarthritis), rotator cuff pathology (including contusions, partial thickness, and full thickness tears), and pectoralis major and minor tears. In this article, we will r

Both authors have dedicated most of their time since 1948 to the treatment of fractures in children. On the basis of their experience they are therefore submitting to the medical public those methods and results of the conservative treatment of fractures and dislocations in children which stood the test of time. The experiences of both authors as well as successful therapeutic methods of other surgeons are discussed in this book. Differences are stated between fractures in adults and in children which may be attributed to the growth factor of children's bones and their enormous biological driv

BACKGROUND: Concurrent acromioclavicular (AC) joint dislocation and coracoid process fracture is a rare injury pattern, representing a double disruption of the superior shoulder suspensory complex (SSSC) and resulting in mechanical instability. Due to its rarity, there is no consensus on optimal management.
CASE PRESENTATION: A 43-year-old male sustained a right shoulder injury during a snowboarding accident. Imaging revealed a right-sided AC joint dislocation with minimal superior displacement (coracoclavicular [CC] distance 13.6 mm on the right vs. 12 mm contralaterally) and a base-near cor
