The unstable shoulder girdle--a combination of a scapular neck fracture with a clavicular fracture, acromioclavicular dislocation, or sternoclavicular dislocation--represents a rare and often underestimated injury. Sufficient treatment to permit early function is necessary to avoid a frozen shoulder as well as to treat concomitant thoracic injuries. This is possible only by open reduction and internal fixation. In most cases this can be achieved by a sole osteosynthesis of the clavicular injury. We retrospectively analyzed 12 cases and classified the results according to the Constant Score. Th

Injuries of the shoulder joint require exact diagnostic assessment. Soft tissue lesions which lead to instability of the joint must be recognized and treated adequately. Besides clinical and radiological examination, arthroscopy and sonography are becoming increasingly important. Acute sprain of the shoulder joint without instability is treated by a short immobilisation followed by active exercises, sprain (distorsion) with instability (dislocation!) may be treated by short immobilisation (two weeks) and early functional exercises or operatively by repair of a Bankart lesion, rupture of the ro

BACKGROUND: Shoulder external rotation recovery in brachial plexus birth injury is often limited. Nerve grafting to the suprascapular nerve and transfer of the spinal accessory nerve to the suprascapular nerve are commonly performed to restore shoulder external rotation, but the optimal surgical technique has not been clearly demonstrated. We investigated whether there was a difference between nerve grafting and nerve transfer in terms of shoulder external rotation recovery or secondary shoulder procedures.
METHODS: This is a multicenter, retrospective cohort study of 145 infants with brachia

OBJECTIVES: Shoulder labral injuries in professional hockey players are often treated surgically to minimize missed ice time. Previous studies have shown that post-operative outcomes in these players are favorable, although they have not specifically focused on athletic performance and time to return to sport. Our objective was to report time to return to play and post-operative on-ice performance metrics after shoulder labral repair in professional ice hockey players.
METHODS: We performed a retrospective review of the clinical records of all professional hockey players (NHL) who underwent a

Long head biceps tendon is a common cause of anterior shoulder pain. Failure of conservative treatment may warrant surgical intervention. Surgical treatment involves long head biceps tenotomy or tenodesis. Several different techniques have been described for biceps tenodesis, including arthroscopic versus open and suprapectoral versus subpectoral. Most studies comparing tenodesis to tenotomy are limited by the level of evidence and confounding factors, such as concomitant rotator cuff tear. Many studies demonstrate similar outcomes for both procedures. Surgeon preference is likely more influen

The athlete with a failed instability procedure requires a thoughtful and systematic approach to achieve a good outcome. Goals of treatment should be defined and realistic expectations should be set. Revision stabilization has a high rate of recurrent instability, low rates of return to play, and low clinical outcome scores. Fundamental to successful revision surgery is choosing the correct procedure. The decision is straightforward in athletes with clear factors that predict recurrence (significant glenoid bone loss, engaging Hill-Sachs lesions) because only a bony procedure can restore the a

MRI provides excellent soft tissue contrast and allows for multiplanar imaging in anatomic planes. Because of these advantages MRI has become the study of choice for imaging of shoulder pathology. Some structures, such as the rotator cuff, humeral head contour, and glenoid shape, are evaluated well with conventional MRI. When more sensitive evaluation of the labrum, capsule, articular cartilage, and glenohumeral ligaments is required or when a partial-thickness rotator cuff tear is suspected, magnetic resonance arthrography with intra-articular contrast can be performed. For MR arthrography con

The attachment of the superior glenohumeral ligament (SGHL) to the upper pole of the glenoid is variable and 3 types have been described. We report an anatomic variant of SGHL attachment to the upper pole of the glenoid that has not heretofore been reported in the literature. In this case, the SGHL overrode the biceps origin, continued to the superior labrum posteriorly, and had no attachment to the middle glenohumeral ligament or the anterior labrum. This variant was detected during routine arthroscopic examination undertaken before surgery on a rotator cuff tear.

Arthroscopy : the journal of arthroscopic & related surgery : official publication of the Arthroscopy Association of North America and the International Arthroscopy Association1992eng
Arthroscopy is without doubt the most comprehensive procedure for shoulder lesions, providing even better inspection of the interior of the joint than conventional open procedures. In 174 diagnostic arthroscopies, it proved more reliable than the computed tomography scan, ultrasound, or arthrography. The results of ultrasonography proved disappointing despite the enthusiastic reports on this technique, probably due to imperfections of equipment and lack of expertise rather than any inherent defect in the method. Arthrography seems to have a high incidence of false results. Though an invasive m

OBJECTIVE: To describe neonatal complications related to shoulder dystocia.
METHODS: This systematic evidence review is based on PubMed search, Cochrane library and experts' recommendations.
RESULTS: The risks of brachial plexus birth injury, clavicle and humeral fracture, perinatal asphyxia, hypoxic-ischemic encephalopathy and perinatal mortality are increased after shoulder dystocia. The medical team should be able to provide neonatal resuscitation in the delivery room in case of perinatal asphyxia following shoulder dystocia, according to national and international guidelines. BACKGROUND: Nearly all studies describing shoulder stabilization focus on male patients. Little is known regarding the clinical outcomes of female patients undergoing shoulder stabilization, and even less is understood about females with glenoid bone loss.
PURPOSE: To assess the clinical outcomes of female patients with recurrent anterior shoulder instability treated with the Latarjet procedure.
STUDY DESIGN: Case series; Level of evidence, 4.
METHODS: All cases of female patients who had recurrent anterior shoulder instability with ≥15% anterior glenoid bone loss and underwent the Latarjet

Electrical injuries are associated with multiple factors, including the current itself, the amount of current flowing through the body during electrocution, the duration of the flow, and the duration of exposure. The instances of bone fracture following electrocution are limited. This article such a rare incident, where an elderly woman suffered an electrical shock at her residence while switching off the refrigerator. The woman complained of intense pain around her left shoulder, with restricted movement of the shoulder joint, without any history of a fall or other trauma. Detailed physical a

A 67-year-old woman was seen in the emergency department because of severe pain and inability to move her left shoulder after falling on an abducted left arm. The fall was the result of a near-syncopal episode. On examination, the left arm was hyperabducted and elevated approximately 80 degrees from the horizontal plane. Furthermore, the patient was unable to move her left arm. Radiographs of the shoulder revealed an inferior dislocation of the glenohumeral joint. Closed reduction in the emergency department was successful. This rare but classic case of inferior glenohumeral dislocation (luxat

Although conventional MR imaging allows direct visualization of major anatomic structures, smaller intraarticular structures, including the glenoid labrum, glenohumeral ligaments, and articular surface of the rotator cuff tendon can be difficult to evaluate in the absence of a joint effusion. The authors review the techniques employed in performing MR arthrography, imaging characteristics of MR arthrography, and diagnostic utilization of the technique. Suggested indications are provided.

146 cases of traffic multiple trauma associated with head injuries were studied. There was a significant difference in the ratio of associated injuries in various parts of the body. The three most encountered parts were chest, shoulder and leg. Death were mainly due to severe head injury, chest injury and shock. Atypical ushing syndrome was due to severe blood loss, even intracranial hypertension existed. The blood pressure decreased rapidly after operation. Since comatous patients were unable to provide chief complaint, misdiagnosis occurred during emergency stage.

The ability of the health care professional to make correct decisions about the progression of weight-training is critical to the rehabilitation process. The purpose of this article is to describe our approach to modification of weight-lifting techniques using the injured shoulder as a model. Additionally, the impact of various upper extremity weight-training techniques on healthy athletes is discussed. The effects of grip, hand spacing, bar trajectory, and start and finishing positions on microtraumatic injury and return to weight-training activities after injury are considered. Several weigh

Upper limb injuries in soccer represent only a marginal portion of injuries, however this is mainly true for outfield players. Goalkeepers are reported to have up to 5 times more upper extremity injuries, many of them requiring substantial time-loss for treatment and rehabilitation. The most common upper extremity injury locations are the shoulder/clavicle followed by the hand/finger/thumb, elbow, wrist, forearm, and upper arm. The mechanism of injury, presentation, physical examination, and imaging features all play a significant role in reaching the correct diagnosis. Taking to consideration

Landouzy-Dejerine myopathy (Facial-shoulder-shoulder myodystrophy) is a disease which causes weakness of the muscles of the shoulder girdle, back and hip muscles, which predisposes patients to an increased risk of injury and disability. The article presents a clinical observation of non-ischemic priapism, which developed as a result of perineal trauma with the formation of a fistula of the right cavernous body in a patient against the background of facial shoulder-shoulder myodystrophy. Methods for the differential diagnosis of this condition are also discussed.

Orthopaedic injuries are common among trauma patients and can result in long-term problems. Considerable data are available regarding functional outcomes following lower extremity trauma. There is, however, a paucity of data available for upper extremity trauma patients. Whilst currently available instruments appear to assess outcomes of relevance in trauma populations, the reliability, validity and responsiveness of these instruments have not been evaluated in the upper extremity trauma population. This paper reviews instruments designed for patient self-evaluation of musculoskeletal disorder

BackgroundRotator cuff tears, a common cause of shoulder pain, often require surgery when conservative treatment fails. Arthroscopic repair is standard, though re-tear rates remain high.ObjectiveThis study aimed to identify the shoulder function questionnaires and strength tests that are significant factors for determining the necessity of rotator cuff repair and to build an effective machine learning model.MethodsThis study included 183 patients between January 2008 and December 2016. Among them, 89 patients underwent non-surgical treatment for rotator cuff injuries, whereas 94 underwent arth
