Within the context of the comprehensive treatment of sleep disorders, which includes medical, neurologic, psychiatric, and social interventions, use of medication is often indicated. Among the three benzodiazepine hypnotics that are available in the United States for the treatment of insomnia, flurazepam is effective for both sleep induction and maintenance, and it retains most of its efficacy over a 4-week period of nightly administration; temazepam is effective only for sleep maintenance, and triazolam improves both sleep induction and maintenance with initial but not with continued administ…

Fasciculation potentials were quantitated during wakefulness (W) and the different sleep stages in 10 patients affected by motor neuron disease. Compared to W, no statistically significant variation was present in fasciculations during any sleep stage. Fasciculations remain unaffected by the decrease in central nervous system excitability which occurs during sleep.

The pulmonary artery pressure values of 65 patients with sleep apnea syndrome were measured at rest and during ergometer exercise up to 100 W. Pulmonary hypertension at rest was found in 13, and during exercise in 31 more patients. Only 8 patients with pathological pressure findings suffered from pulmonary hypertension in combination with a pulmonary or cardiac disease. In the other 36 patients, no indication of a primary cause of pulmonary hypertension apart from sleep apnea syndrome could be found. Out of the 65 patients, 11 with a finding of more than 20 apnea episodes per hour's sleep unde…

Eight patients (aged 8 to 22 years) with the Pierre Robin syndrome underwent sleep studies. Seven demonstrated significant although minor degrees of increased sleep disturbances and apneas, and less time spent in the rapid-eye-movement (REM) stage of sleep. One patient who had previously undergone mandibular corrective surgery had major sleep abnormalities (central apnea index of 81.7 although an obstructive sleep apnea index of only 1.9). The patients had small mandibles, as demonstrated by lateral cephalometric roentgenography, and mildly increased right ventricular diastolic dimensions, as …

Sleep, performance, and sleepiness were assessed in 10 elderly volunteers (8 women, 2 men; aged 61-77 years) before, during, and after 38 h of sleep loss. Recovery night 1 sleep showed increased total sleep and stages 3 and 4 sleep and decreased stage 1 sleep, wakefulness, brief arousals, and latency to stages 3 and 4 sleep. An increase in stage 4 sleep persisted to the second recovery night. Increased arousal threshold was suggested by a lengthening of respiratory events and a reduction in arousals associated with leg movements. Performance was impaired during sleep loss, associated with an i…

Disturbed nocturnal sleep is considered a symptom of narcolepsy. Polysomnographic recordings of 57 consecutive narcoleptic patients were reviewed for evidence of disturbed sleep. When disrupted sleep was present, it was attributable to recognized sleep disorders: nocturnal myoclonus and sleep apnea. Comparison of standard polysomnographically derived parameters of patients who had narcolepsy without sleep apnea or nocturnal myoclonus with those of a normal control group, showed no evidence of disturbed sleep in the patient population. The narcoleptics that also had nocturnal myoclonus or upper…

We present a case of severe breathing abnormality during sleep in a young man who had had poliomyelitis 20 yr before. His sleep disorder led to respiratory failure and cor pulmonale, which were greatly improved by oxygen therapy. A study of this case and those previously described supports the notion that brainstem damage during acute poliomyelitis is important in the later appearance of sleep-disordered breathing. In addition, such patients usually have mechanical abnormalities involving the thoracic cage and respiratory muscles. These ventilatory restrictions amplify the pathophysiologic eff…

We studied the effect of alcohol ingestion on sleep-induced breathing abnormalities and arterial oxyhaemoglobin saturation in seven patients with a range of sleep-induced upper airway occlusion. The characteristics of each patient's sleep-induced breathing abnormality was established on one or more control all-night studies, and then a further all-night study was done immediately following alcohol ingestion. Alcohol increased the duration and frequency of the occlusive episodes in five patients with obstructive sleep apnoea, and resulted in a marked increase in the degree of hypoxaemia in the …

Sleep paralysis is a sensation of an inability to speak or move other muscles when falling asleep or awakening. Sleep paralysis by itself has been reported as occurring infrequently and many clinicians are uncertain of its significance. In contrast, sleep paralysis in conjunction with sleep attacks has been reported as a concomitant of narcolepsy. To further examine the incidence of sleep paralysis, the responses of 80 first-year medical students, 16.25% had experienced predormital, postdormital, or both types of sleep paralysis. These episodes occurred infrequently--only once or twice for mos…

An 18-year-old student presented with a two-year history of daytime sleepiness and noisy breathing during sleep. Both he and his brother, aged 25 years, had Scheie's syndrome, a mucopolysaccharidosis characterised by small stature, micrognathia, corneal clouding, hepatosplenomegaly, raised urinary mucopolysaccharides, and undetectable levels of alpha-L-iduronidase assayed in cultured fibroblasts. Both brothers had sleep apnoea (apnoea index, 59 and 35 respectively) during which there was a significant fall in heart rate and arterial oxygen saturation. One brother had EEG changes suggestive of …

Five patients with severe kyphoscoliosis were studied during sleep. A spectrum of breathing abnormalities was found ranging from no abnormalities to severe episodes of prolonged central apnea. Rapid-eye-movement (REM) sleep was the period of greatest physiologic disturbance in all the patients, being the time of greatest oxygen desaturation. Within this small group, those with the most clinical evidence of chronic hypoxemia, polycythemia, and cor pulmonale, had the most severe derangements during sleep. Derangements in breathing pattern and arterial oxygen saturation (SaO2) had no apparent rel…

Sleep is affected in depression; insomnia is common, sleep of normal duration and hypersomnia less common. All-night studies have shown changes of the two types of sleep. Deep non-REM sleep is abolished during the course of the illness and sometimes also after remission. Paradoxical sleep, which may be reduced or increased in duration, starts sooner after the onset of sleep. According to Kupfler, ease of production of that sleep is specific to primary depression, unipolar or bipolar. A possible relationship between paradoxical sleep and certain types of depression is suggested by the fact that…

To enhance physicians' knowledge of the prevalent problem of sleep disorders, a 50-minute videotape program designed as a learning and self-assessment experience in evaluating and treating these disorders was coproduced with the Network for Continuing Medical Education. The program has been presented nationally to about 25,000 medical professionals. The mean correct score was 50% for the pretest, 69% for the instructional section, and 79% for the posttest; the composite score on the posttest was 59% higher than that on the pretest. Physicians' responses indicated strengths and weaknesses in th…

Apneic episodes in normal 10-, 20-, and 40-day-old kittens were assessed with polygraphic recordings. End expiratory apneas, usually preceded by somatic activity and/or augmented breaths, with durations less than 10 sec were observed in quiet sleep, active sleep, and transitions between states in all age groups. The highest apnea density was found at state transitions. Heart rate decelerations occurred before, during, and following apneas, but decelerations were not related to apnea duration. Combined central and obstructive components were associated with 9% of apneas in normal kittens.
