NIV Versus NVS: A Review of Respiratory Principles of Physical Medicine After 60 Years of Neglect.

Despite more than 60 years of evidence supporting it, continuous noninvasive ventilatory support (CNVS) via mouth, nasal, oronasal interfaces, and ventilation belts remains underrepresented in physical medicine training, along with the use of mechanical insufflation-exsufflation (MIE) as an alternative for tracheostomy ventilation (TMV) and invasive airway suctioning for patients with ventilatory pump failure. This includes patients with neuromuscular disease (NMD), spinal cord injury (SCI), and morbid obesity. This review summarizes the physiologic rationale of noninvasive ventilation ("NIV")

Management of upper airway obstruction in syndromic craniosynostosis: A lifespan approach from childhood to adulthood.

INTRODUCTION: Syndromic faciocraniosynostoses are malformations of the skull and face caused by premature closure of one or more cranial sut … s surgical procedures: fronto-facial advancement, adenoidectomy, turbinoplasty, septoplasty, etc. A tracheotomy may be necessary during early childhood, pending permeabilization of the upper airways. The age at which surgery is performed varies, depending on the individual patient, and it is not uncommon for patients to require surgery in adulthood, either as …

An Unknown Foreign laryngeal Object: an exotic complication of skull base osteoradionecrosis.

BACKGROUND: Osteoradionecrosis (ORN) of the skull base is a rare complication after head and neck radiation with a broad variety of subseque … cervical spine instability was ruled out, the patient's airway was initially secured with an awake tracheotomy and the clivus was removed transorally. The tracheostomy tube was removed during the ongoing inpatient stay, and the patient was discharged with significant pain relief. Multiple symmetric lipomatosis with secondary laryngeal obstruction: A case report.

INTRODUCTION: Multiple symmetric lipomatosis (MSL) is an uncommon medical condition characterized by symmetric fat accumulation mainly in the neck and other upper body regions. The involvement of the larynx is rare according to the literature, and we present a case of MSL with larynx involvement treated with a surgical approach. PATIENT CONCERNS: A 55-year-old male was admitted to our hospital due to progressively aggravated breathing difficulty, and tracheotomy was performed before transfer. When he tried to block the cannula, the breathing difficulty returned. The patient's neck had been th…

Acute laryngeal dyspnea as first presentation of granulomatosis with polyangiitis.

Granulomatosis with polyangiitis (GPA) is a multi-organ disease which mostly affects lungs, kidney, and head and neck region. We report a rare case of acute laryngeal dyspnea and rapidly progressive pulmonary changes as first manifestations of disease. A 53 year-old woman presented with symptoms of two-week dyspnea, which aggravated rapidly in the preceding hours. Laryngological examination revealed subglottic infiltrations and vocal fold oedema which required urgent tracheotomy. During few days she developed gingival ulcerations and pulmonary infiltration with negative serum c-ANCA titers. Th…
