Case Report: Traumatic Carotid-Cavernous Fistula.

Carotid-cavernous fistulae (CCFs) are a rare complication of head trauma, with potentially serious consequences. We report the case of a 45-year-old male patient who presented with posttraumatic CCF 2 months after sustaining a head injury. Appropriate imaging in the form of computed tomography of the head, magnetic resonance imaging of the head, and digital subtraction angiography of the intracranial vessels was performed, and the CCF was successfully coil embolized. This resulted in good resolution of the patient's symptoms. We discuss the cause, presentation, diagnosis, and treatment of CCFs

Impact of Alcohol on Outcomes in Hospitalized Major Trauma Patients: A Literature Review.

To critically appraise available studies investigating the impact of alcohol outcomes in hospitalized patients after major trauma. Outcomes included injury severity, mortality, complications, and hospital/intensive care length of stay. Data were sourced from MEDLINE, CINAHL, and SCOPUS databases. Abstracts were screened for inclusion/exclusion criteria with 15 articles appraised and analyzed based on in-hospital outcomes. Although alcohol intoxication is associated with a marginal increase in the incidence of in-hospital complications (sepsis and multiorgan failure), overall studies suggest it

Progressive Mobility Protocol Reduces Venous Thromboembolism Rate in Trauma Intensive Care Patients: A Quality Improvement Project.

The intensive care unit (ICU) trauma population is at high risk for complications associated with immobility. The purpose of this project was to compare ICU trauma patient outcomes before and after implementation of a structured progressive mobility (PM) protocol. Outcomes included hospital and ICU stays, ventilator days, falls, respiratory failure, pneumonia, or venous thromboembolism (VTE). In the preintervention cohort, physical therapy (PT) consults were placed 53% of the time. This rose to more than 90% during the postintervention period. PT consults seen within 24 hr rose from a baseline

Increased Screening for Child Physical Abuse in Emergency Departments in a Regional Trauma System: Response to a Sentinel Event.

A pediatric patient was assaulted while being treated at a Level 1 pediatric trauma center, prompting a Centers for Medicare & Medicaid Services site visit. The process of screening for physical abuse and protection of patients was reevaluated and revised, and a new guideline was implemented and shared with referral hospitals. During this same time period, 13 referral hospitals participated in an unrelated federally funded study determining the impact of recognition and care of injured children in states with and without a pediatric emergency care facility recognition program. A pre-post study

Penetrating knitting needle through the mediastinum in a child.

Thoracic injuries are second only to central nervous system injuries as the leading cause of traumatic death in patients of all ages. Penetrating chest injury is very uncommon in children, but it comes with significant morbidity and mortality. Presentation of penetrating thoracic injury in the child is unique with inherent opportunities for learning. The purpose of this case report was to disseminate information regarding rare thoracic trauma in the pediatric patient.

Risk factors for ventilator-associated pneumonia: among trauma patients with and without brain injury.

Ventilator-associated pneumonia (VAP) rates remain highest among trauma and brain injured patients; yet, no research compares VAP risk factors between the 2 groups. This retrospective, case-controlled study identified risk factors for VAP among critically ill trauma patients with and without brain injury. Data were abstracted on trauma patients with (cases) and without (controls) brain injury. Data gathered on n = 157 subjects. Trauma patients with brain injury had more emergent and field intubations. Age was strongest predictor of VAP in cases, and ventilator days predicted VAP in controls. T

Pharmacist's impact on acute pain management during trauma resuscitation.

The timely administration of analgesics is crucial to the comprehensive management of trauma patients. When an emergency department (ED) pharmacist participates in trauma resuscitation, the pharmacist acts as a medication resource for trauma team members and facilitates the timely administration of analgesics. This study measured the impact of a pharmacist on time to first analgesic dose administered during trauma resuscitation. All adult (>18 years) patients who presented to this level II trauma center via activation of the trauma response system between January 1, 2009, and May 31, 2013, wer

Pediatric Orthopedic Injuries Following an Earthquake: Experience in an Acute-Phase Field Hospital.

BACKGROUND: Following the 2010 earthquake in Haiti, the Israel Defense Forces Medical Corps deployed a field hospital in Port au Prince. The purpose of this study was to characterize the injuries sustained by the pediatric population treated in the hospital and examine the implications for planning deployment in future similar disasters. METHODS: Medical records of children treated in the hospital were reviewed and compared with medical records of the adult population. RESULTS: A total of 1,111 patients were treated in the hospital. Thirty-seven percent were aged 0 to 18 years. Earthquake-re

Complete tracheal transection following blunt trauma in a pediatric patient.

Tracheal injury from blunt trauma is a rare, life-threatening condition in pediatric patients. Rapid assessment and identification of the severity of the injury will increase the likelihood of survival. Subcutaneous emphysema, pneumomediastinum, and persistent pneumothoraces after chest tube placement should be considered in the index of suspicion for tracheal injury. Treatment for a complete tracheal transection includes rapid airway securement via endotracheal tube or tracheostomy and careful surgical repair in the operating room in conjunction with bronchoscopy.

Violence-related mild traumatic brain injury in women: identifying a triad of postinjury disorders.

Violence against women whether from domestic partner abuse, sex trafficking injuries, or sexual assault is a pervasive health problem without racial or social boundaries. Regardless of cause, violence results in a complex triad of physical, emotional, and psychological injuries. There is clear evidence that female victims of violence or "battered women" experience brain injury. What is less certain is whether the constellations of events surrounding brain injury including postconcussion syndrome, depression, and posttraumatic stress disorder are acute symptoms after the brain injury, premorbid

Full of sound and fury, signifying nothing: burden of transient noncritical monitor alarms in a trauma resuscitation unit.

We examined the types of patient monitor alarms encountered in the trauma resuscitation unit of a major level 1 trauma center. Over a 1-year period, 316688 alarms were recorded for 6701 trauma patients (47 alarms/patient). Alarms were more frequent among patients with a Glasgow Coma Scale of 8 or less. Only 2.4% of all alarms were classified as "patient crisis," with the rest in the presumably less critical categories "patient advisory," "patient warning," and "system warning." Nearly half of alarms were ≤5 seconds in duration. In this patient population, a 2-second delay would reduce alarms b

Development of a geriatric resuscitation protocol, utilization compliance, and outcomes.

Inconsistent application of trauma service resources and underevaluation of risk and resuscitation status in elderly trauma patients are problematic. We describe a geriatric protocol that includes initial lactate determination and trauma surgery admission. Protocol compliance rates were initial lactate determination, 67.9%; trauma service admission for overt or compensated (elevated lactate) shock, 73.6%; and trauma service consultation for nonshock patients, 67.8%. Implementation of this protocol resulted in a trend toward reduced mortality and reduced potentially preventable deaths.

Traumatic bladder laceration in the absence of pelvic fracture.

Traumatic bladder laceration commonly occurs because of blunt trauma with associated pelvic fracture. The incidence of bladder laceration without pelvic fracture is less than 10%. Although high mortality (44%) existed in the past, a timely diagnosis with appropriate medical and surgical management now offers an excellent outcome. Early clinical suspicion, appropriate and reliable radiologic studies, and prompt surgical intervention, when indicated, are the keys to successful diagnosis and management.

Interventions for marital discord in military couples: strengthening the home front.

This article discusses the use of role-exit theory within military couples as they attend counseling for martial discord. The adjustment of roles by both members of the marriage during a military deployment can create tension on the soldier's return. The goal of counseling is to assist the couples in regaining a balance in their marriage through cohesion and generating comfort in the roles assumed during deployment and on the soldier's return.A need for flexibility and understanding is important for a successful marriage. This need increases in a military couple. Frequent changes due to milita

Propofol infusion syndrome.

Propofol (Diprivan) is an intravenous sedative hypnotic that is used in the induction and maintenance of anesthesia and sedation. High-dose infusions have been associated with several serious adverse effects and, when combined, they are known as propofol infusion syndrome (PRIS). Although PRIS is rare, it is frequently fatal if not identified early. The purpose of this article is to raise practitioner awareness to this syndrome, with recommendations for early identification, prevention, and treatment of PRIS.

Carp versus cranium: a fishy story.

Issues of drowning or near drowning in aquatic sporting have received considerable attention in the literature; however, there is not a lot of attention to nonsubmersion injuries such as water tubing. Water tubing is similar to water skiing but with much less control by the rider, leaving the rider at the mercy of the driver or any obstacle in their path. This report describes unusual events surrounding the injury of a 5-year-old boy enjoying water fun with his father on his boat. The importance of adequate safety precautions during aquatic recreational activities will also be highlighted.

Young riders: all-terrain vehicle use in north Texas.

Annually, there are more than 54,000 visits to nationwide emergency departments as a result of injuries sustained while riding all-terrain vehicles. Of those visits, nearly one-half involve children younger than 16 years. Children's Medical Center Dallas admitted 211 patients over a 5-year period from March 2002 until May 2007 as a result of all-terrain vehicle-related incidents with 207 of those patients aged 13 years or younger. The primary mechanisms of these injuries were secondary to collision with stationary objects, rolling of the vehicles, and falling from the vehicles.

Bicycle injury prevention and safety in senior riders.

Senior bicycle riders have special safety and injury prevention needs. They are at greater risk for injury and mortality. Very few educational materials have been developed to assist this group in improving their riding abilities and preventing injury to themselves. This article reviews the risks encountered by older bicyclists, discusses injury revention, and describes the development of an educational video developed especially for senior riders consistent with the goals of the National Bicycle Safety Conference.
