[Cavernous transformation of portal vein].

Cavernous transformation of the portal vein occurs with long-standing portal vein thrombosis (PVT) because of the development and dilatation of multiple small vessels in and around the recanalising main portal vein. Thrombosis and occlusion of the portal vein leads to portal hypertension with enlarged spleen and the development of porto-systemic collaterals. The main clinical presentation is gastroesophageal variceal bleeding and hematologic abnormalities due to splenomegaly (hyperspleenismus-pancytopenia). We described the young patient with splenomegaly and extensive cavernous transformation

Effect of xylazine sedation on testicular blood flow, testicular echotexture, and circulating hormones in Shiba goats.

Under field conditions, sedation may be required for a full assessment of the reproductive potential of farm animals. The present study aimed to investigate the effect of xylazine sedation on testicular hemodynamics (TBF), echotexture, testicular volume (TV), and circulating hormones in goats. Sixteen male Shiba goats were sedated using the recommended dose of xylazine (0.05 mg/Kg BW). Testicular hemodynamics were evaluated using color-pulsed Doppler ultrasonography before and after sedation. Echotexture of the testicular parenchyma and TV were assessed using computerized image analysis. Conce

Effects of maternal hyperoxia with and without normocapnia in uteroplacental and fetal Doppler studies.

OBJECTIVE: One hundred percent oxygen is given in pregnancy to improve fetal oxygenation, yet has been shown in both animal and human studies ex utero to increase cerebral vascular resistance. Adjusting end-tidal pCO2 (ET-pCO2) levels to normocapnic levels during hyperoxygenation offsets this effect in non-pregnant individuals. We aimed to evaluate the effect of maternal hyperoxygenation with and without maintaining normocapnia on the fetal and uteroplacental circulations in healthy near-term human pregnancies. METHODS: Eight healthy pregnant women, serving as their own controls, sequentially

A case of massive subchorionic thrombohematoma diagnosed by ultrasonography and magnetic resonance imaging.

Massive subchorionic thrombohematoma is uncommon but associated with a poor perinatal prognosis. Placental enlargement was detected in a 25-year-old Japanese primipara woman with fetal growth retardation and oligohydramnios at 23 weeks' gestation. Ultrasonography (USG) showed an abnormal sonolucency within the placenta at 28 weeks' gestation, but could not give an unequivocal differentiation from placental abnormalities such as hematomas, cysts and other tumors. Magnetic resonance imaging (MRI) pointed to a large hematoma in the subchorionic region. Simultaneously, the amniotic fluid was brown

US of nerve entrapments in osteofibrous tunnels of the upper and lower limbs.

The diagnosis of nerve entrapment at osteofibrous tunnels relies primarily on clinical and electrodiagnostic findings. Recently, the refinement of high-frequency broadband transducers with a range of 5-15 MHz, sophisticated focusing in the near field, and sensitive color and power Doppler technology have improved the ability to evaluate peripheral nerve entrapment in osteofibrous tunnels with ultrasonography (US). In the upper limb, osteofibrous tunnels amenable to US examination include the carpal tunnel for the median nerve and the cubital and Guyon tunnels for the ulnar nerve. In the lower

Comparative Assessment of Transcranial Doppler and MRI Perfusion Imaging in a Japanese Child With Probable Brain Death.

The determination of brain death differs internationally, although in Japan, it is legally recognized only when organ donation is intended. Otherwise, the condition is termed probable brain death. Ancillary tests, including transcranial Doppler (TCD) ultrasonography, are not routinely incorporated into the clinical framework for pediatric brain death evaluation. In this report, we describe the case of a Japanese child with probable brain death, in whom both TCD and MRI revealed an absence of cerebral perfusion, providing valuable comparative data. A four-year-old girl with influenza A who deve

Ultrasonography in the diagnosis of complications in patients with portal hypertension.

This review focuses on ultrasonography (US) to diagnose patients with complications in portal hypertension. Clinicians first use US to evaluate patients with suspected portal hypertension, because US is quick, simple, and radiation free. US is necessary for grading and performing paracentesis for ascites. Doppler US-based detection of reverse splanchnic vein flow or the presence of a spontaneous portosystemic shunt is highly specific in patients with cirrhosis. Since it is important to estimate spleen size in patients with portal hypertension, spleen size is usually measured by US. Spleen volu

Ultrasound diagnosis of appendiceal tumor with synchronous liver metastasis mimicking acute appendicitis and liver abscess.

Appendiceal tumors (AT) are rare gastrointestinal cancers with high incidence of synchronous and metachronous colorectal cancer metastases. AT are rarely associated with hepatic metastases. Sometimes, metastases are the first evidence of the tumor. We report the clinical, surgical and imaging records of a female patient, 81 years old, admitted for a suspect liver abscess, only subsequently diagnosed as metastasis from an undiagnosed AT. Ultrasound (US) based imaging techniques, such as Color-Doppler Ultrasound, Contrast enhanced US, US-guided biopsy of the hepatic nodule played an important ro

Diagnostic value of contrast-enhanced ultrasound (CEUS) and comparison with color Doppler ultrasound and magnetic resonance in a case of scrotal trauma.

Intratesticular hematoma is one of the most frequent presentations of scrotal trauma. Ultrasound is the first-line imaging modality in the evaluation of scrotal trauma, and its findings are crucial for the choice of treatment. An intratesticular hematoma may represent a diagnostic pitfall for the investigating physician since its appearance may mimic other conditions, such as testicular neoplasms or segmental infarction. Although the gold standard imaging modality for the characterization of a testicular lesion is contrast-enhanced magnetic resonance (MR), MR equipment is not always available

A Video of Ultrasound-Guided S1 Transforaminal Epidural Injection Using Color Doppler: Technical Reports.

Lumbosacral radicular pain with or without radiculopathy is a fairly common clinical condition. In such cases, lumbosacral transforaminal epidural injection (TFEI) is one of the most commonly performed interventions. Recent studies have explored the effectiveness of ultrasound (US)-guided lumbosacral TFEI. Most of these studies have focused on lumbar TFEI via an in-plane approach. S1 TFEI can also be easily performed, since the S1 foramen is more superficial than the lumbar foramen. However, most studies regarding US-guided sacral TFEI use an out-of-plane technique. Therefore, in this report w

The added value of contemporary ultrasound technologies in the diagnosis of malignant tumours of the gastrointestinal system - a case report.

Ultrasound of the gastrointestinal system has become more popular among clinicians and is an important tool for evaluation and follow-up. Ultrasound has a special value in patients with Crohn's disease for the evaluation of disease activity but also for the evaluation of abdominal complications and shows comparable results to other imaging modalities. With the introduction of elastography and contrast-enhanced ultrasound into clinical routine, THE sonographic evaluation of the small bowel is even more sophisticated and even more precise. This case report summarizes all available modern ultraso
