"Atypical Glandular Cells" on Cervical Cytology: Correlation Between Glandular Cell Component Volume and Histological Follow-Up.

BACKGROUND: Atypical glandular cells (AGC) in cervical cytology, as defined by the Bethesda System, indicate nuclear atypia beyond reactive changes but without definitive features of malignancy. Although clinically significant because it prompts follow-up procedures, no quantitative threshold exists for AGC diagnosis. This study evaluated whether the volume of glandular cell clusters (GCC), regardless of atypia, influences AGC interpretation and may contribute to unnecessary sampling. METHODS: Following IRB approval, all cervical cytology cases diagnosed as AGC between January 2014 and June 2

Outcome of topical steroid application in children with non-retractile prepuce.

True phimosis is overdiagnosed due to the failure to distinguish it from physiological phimosis, which is a normal developmental non retractability of the foreskin. The non-retractile prepuce in children is a cause of parental anxiety and concern. This leads to the majority of the children undergoing surgical procedures. Pathological phimosis needs to be differentiated from physiologic phimosis to avoid unnecessary circumcision. In recent years, topical steroid application use in cases of non-retractile prepuce has shown a good success rate and is well accepted by the parents. It has low risks

An evaluation of the relationship between Enterobius vermicularis infestation and acute appendicitis in a paediatric population--A retrospective cohort study.

BACKGROUND: Enterobius vermicularis is an often unexpected finding in appendectomy specimen, most commonly seen in paediatric cases. Predicting the presence of E. vermicularis in the setting of appendectomy is important to avoid unnecessary appendectomy and associated morbidity. We sought to identify the incidence of E. vermicularis in a paediatric population undergoing appendectomy for clinically suspected acute appendicitis and identify predictive factors for E. vermicularis. METHODS: This study was performed in an 800-bed University Teaching Hospital, in the Republic of Ireland. We identif

Analysis of errors reported by surgeons at three teaching hospitals.

BACKGROUND: Little is known of the factors that underlie surgical errors. Incident reporting has been proposed as a method of obtaining information about medical errors to help identify such factors. METHODS: Between November 1, 2000, and March 15, 2001, we conducted confidential interviews with randomly selected surgeons from three Massachusetts teaching hospitals to elicit detailed reports on surgical adverse events resulting from errors in management ("incidents"). Data on the characteristics of the incidents and the factors that surgeons reported to have contributed to the errors were rec

Persistent oxidized regenerated cellulose mimicking locoregional recurrence after thyroidectomy: diagnostic challenges and associated risk factors.

BACKGROUND: Oxidized regenerated cellulose (ORC) is widely used as a local hemostatic agent during thyroid surgery. Although manufacturers report complete absorption within 7-14 days, persistent ORC is frequently observed in postoperative follow-up, often mimicking residual or recurrent thyroid tissue. This misinterpretation can result in unnecessary investigations or biopsies. The present study aimed (1) to define the ultrasonographic features of persistent ORC to prevent misleading radiological interpretations and (2) to identify the risk factors associated with delayed ORC resorption. Evaluation of the safety of multiple nut oral food challenges in the management of nut allergy.

Background. Tree nut and peanut allergy are common, and often persistent conditions, that significantly affect quality of life. Multiple nut avoidance is common due to cross-reactivity concerns but may cause unnecessary restrictions. Data regarding multiple nut oral food challenges (MN-OFC) remains limited. Our aim was to evaluate the safety and clinical impact of MN-OFC in the management of tree nut and/or peanut allergy. Methods. Retrospective analysis of 25 patients who underwent MN-OFCs between 2022 and 2024 at a tertiary-level Allergy Department. Patients with profilin and/or PR-10 sensit

Unnecessary dental extractions in patients with Persistent Idiopathic Facial Pain: a qualitative and questionnaire-based study of patient perspectives.

BACKGROUND: This study aimed to explore the perspectives of individuals with Persistent Idiopathic Facial Pain (PIFP) who sought dental extractions for pain relief and to identify common themes from their experiences. PIFP significantly impacts the quality of life, leading many patients to undergo unnecessary dental procedures on healthy teeth for pain relief. Recognizing unique characteristics in patients with a history of such interventions could help prevent unnecessary treatments and associated complications. METHODS: We conducted qualitative research at the Headache Clinic, Tel Aviv Sour

Severe Lower Urinary Tract Dysfunction in Otherwise Healthy Children: A Three-Case Series and Narrative Review.

BACKGROUND: Severe lower urinary tract dysfunction (LUTD) in neurologically and anatomically normal children is uncommon and frequently underdiagnosed. When severe, functional voiding disorders may closely mimic obstructive or reflux pathology, leading to diagnostic errors, unnecessary invasive procedures, and potential risk to the upper urinary tract. CASE PRESENTATION: We present three pediatric cases (aged 3-10 years) referred for recurrent febrile urinary tract infections, incontinence, or acute urinary retention in the absence of neurological or structural abnormalities. Measuring and governing potentially low-value invasive coronary angiography in Europe: a systematic review, construct-specific meta-analysis, and policy framework.

BACKGROUND: Potentially low-value invasive coronary angiography is defined inconsistently across European health systems. It remains unclear which indicators can validly support measurement, comparison and governance. OBJECTIVE: To determine a measurement framework for identifying and governing distinct forms of potentially low-value invasive coronary angiography without encouraging underuse, using construct-specific synthesis to assess existing indicators. METHODS: We conducted a PRISMA 2020 systematic review and construct-specific random-effects meta-analysis. PubMed/MEDLINE, Web of Scienc

Granulomatosis with polyangiitis presenting with rare urogenital involvement: a case based review.

Granulomatosis with polyangiitis (GPA) is a rare systemic necrotizing vasculitis, typically involving the upper and lower respiratory tract and kidneys. Urogenital manifestations are uncommon (< 2%) and frequently mimic malignancy. We describe a female patient in whom GPA initially presented with urogenital manifestations and review the available literature regarding urogenital involvement in the course of GPA. We describe the case of a 58-year-old woman with granulomatosis with polyangiitis initially presenting as a bladder mass, leading to radical cystectomy before the autoimmune etiology wa

Prospective validation of a urine-based proteomics test for predicting clinically significant prostate cancer in biopsy-naïve patients.

BACKGROUND AND OBJECTIVE: Prostate cancer (PCa) remains a major health burden, while existing diagnostic approaches are limited by overdiagnosis that leads to unnecessary biopsies, and by underdetection of clinically significant disease (csPCa). This study aims to prospectively validate a urine-based 19-biomarker model (19-BM) using capillary electrophoresis-mass spectrometry (CE-MS) for predicting csPCa in patients at risk. METHODS: One hundred sixty-one biopsy-naïve patients at risk for csPCa underwent pre-biopsy CE-MS analysis in urine, multiparametric magnetic resonance imaging (mpMRI), a

Not Every Draining Wound is Infection: Vicryl Reaction after Total Knee Arthroplasty Mimicking Prosthetic Joint Infection - A Case Series and Diagnostic Insights.

INTRODUCTION: Vicryl reaction, also known as suture-related pseudo-infection (SRPI) or suture granuloma, is a rare but important differential diagnosis of post-operative wound complications following total knee arthroplasty (TKA). It mimics prosthetic joint infection, often leading to diagnostic confusion and potential overtreatment. Given the widespread use of braided absorbable sutures like polyglactin 910, recognizing this entity is essential to avoid unnecessary interventions. MATERIALS AND METHODS: We present a case series of four patients who developed delayed wound complications follow

Role of Spleen Stiffness Measurements with 2D Shear-Wave Elastography for Esophageal Varices in Patients with Compensated Advanced Chronic Liver Disease.

Background/Objectives: Esophageal varices (EVs) represent an important portal hypertension complication in patients with compensated advanced chronic liver disease (cACLD). Although upper gastrointestinal endoscopy is currently the gold standard for EV diagnosis, recent guidelines recommend non-invasive approaches to assess EV risk in cACLD patients to reduce unnecessary endoscopies. Our study aims to evaluate spleen stiffness measurement (SSM) using 2D shear-wave elastography (2D-SWE) as a non-invasive predictor of EV presence and severity in patients with cACLD. Methods: We included 73 cACLD

Optimizing Scan Range in Computed Tomography of Kidneys, Ureters, and Bladder: A Retrospective Study on Reducing Overscanning.

Background and Objectives: Computed tomography of the kidneys, ureters, and bladder (CT KUB) is essential for evaluating urinary stones but also exposes patients to significant radiation. The scanning field should be minimized to only the necessary area to limit this radiation exposure. This study aims to assess the extent of CT KUB overscanning in renal colic procedures and identify the appropriate vertebral level for starting CT KUB scans. Materials and Methods: A retrospective analysis of 299 adult patients who underwent CT KUB examinations for kidney stone assessment was performed. To asse

Non-union in 3 of 15 osteotomies of the distal radius without bone graft.

BACKGROUND AND PURPOSE: Open-wedge osteotomies of the distal radius create a void that is usually filled with either iliac crest bone graft or bone substitute. Previous studies have suggested that this is unnecessary. We investigated the safety of omitting the filling procedure. PATIENTS AND METHODS: We included 15 patients with a dorsal malunion of a distal radius fracture. A palmar approach and angle-stable plates were used. The patients were followed until there was radiographic and clinical healing. RESULTS: Non-union occurred in 3 of the 15 patients. The study, which had been planned to

Transsexual healing: medicaid funding of sex reassignment surgery.

Federal requirements for state Medicaid programs are surveyed, and case law regarding Medicaid funding of sex reassignment surgery is reviewed. States have attempted to exclude sex reassignment surgery (SRS) from Medicaid coverage on various bases, concluding, for example, that the procedure constituted "cosmetic surgery." Judicial scrutiny of such exclusions has usually resulted in the state action being found violative of the federal Medicaid statute and accompanying regulations. In those cases upholding the state exclusion, the primary judicial obstacle to funding has been a determination t

Real-time detection of cucurbit chlorotic yellows virus by RT-LAMP.

Cucurbit chlorotic yellows virus (CCYV) of the genus Crinivirus and family Closteroviridae, is an emerging infectious agent transmitted by whiteflies that mainly infects cucumber, melon and watermelon plants. Here, we developed a procedure based on reverse transcription (RT) followed by loop-mediated isothermal amplification (LAMP) to detect CCYV in real-time by using a fluorescent dye and incubating at 65 °C. This method detected CCYV in RNA extracts at about 10 min and in nylon membrane-filtered crude extracts between 55 and 95 min. The detection was sensitive, ten times higher than RT follo
