Rapidly progressive hepatic alveolar echinococcosis in an ABO-incompatible renal transplant recipient.

We report on the case of an ABO-incompatible renal re-transplant recipient maintained on an intensified immunosuppressive regimen for recurrent cellular rejection episodes and transplant glomerulopathy who presented with rapidly growing hepatic tumors, radiologically suggestive of hemangiosarcoma. Upon resection and pathological work-up, the lesions revealed alveolar echinococcosis, a rare but potentially life-threatening parasitosis. Usually infection with Echinococcus multilocularis remains asymptomatic for extended periods of time and can go unrecognized for years. In the case presented, we

Hemolytic Disease of the Newborn Caused by Anti-Cellano (Anti-k) Alloimmunization: A Case Report.

This report describes a rare case of hemolytic disease of the newborn caused by anti-Cellano (anti-k) antibodies, part of the clinically significant Kell blood group system, the third most important blood group system after ABO and Rh. We present the case of a 43-year-old woman with a history of multiple miscarriages and one previous live birth conceived through in vitro fertilization with a donor oocyte. At 38 weeks of gestation, she delivered a B-positive neonate who subsequently developed anemia and jaundice. Laboratory evaluation demonstrated a positive direct Coombs test and the presence

Severe Neonatal Hyperbilirubinemia Decreased after the 2007 Canadian Guidelines.

OBJECTIVES: To estimate the incidence of severe neonatal hyperbilirubinemia in Canada from 2011-2013 following the implementation of the Canadian Pediatric Society's published guidelines on the management of hyperbilirubinemia in 2007. Our previously reported incidence of hyperbilirubinemia in Canada was 1 in 2480. STUDY DESIGN: Term infants ≤ 60 days of age, with a peak serum total bilirubin level > 425 μmol/L or who had an exchange transfusion were followed prospectively through the Canadian Pediatric Surveillance Program from 2011-2013. Infants with rhesus isoimmunization or born < 35 week

SOME BIOLOGICAL CHARACTERISTICS ASSOCIATED WITH GESTATIONAL DIABETES.

This study investigated the relationships between specific biological factors and the risk of gestational diabetes mellitus (GDM). A retrospective case-control study was conducted in pregnant women with GDM and in healthy pregnant women who served as a control group. ABO and Rh blood groups, maternal age, pre-pregnancy and third-trimester body mass index (BMI), and first-trimester thyroid-stimulating hormone (TSH) and free thyroxine (FT4) levels were analyzed in both groups. Associations were assessed using the chi-square (χ²) test, together with odds ratios (OR) and 95% confidence intervals (

ABO genotyping with next-generation sequencing to resolve heterogeneity in donors with serology discrepancies.

BACKGROUND: ABO subtypes are characterized by the alteration of antigens present and their expression levels on red blood cells and many are linked to genetic changes in the ABO gene. Weakened expression of antigens should be identified to prevent transfusion reactions or ABO-incompatible transplantations. Genotyping can be applied to identify subtypes to complement serologic testing. Next-generation sequencing (NGS) has shown to provide sensitive and accurate genotyping results as well as valuable cis/trans information. Here we took advantage of NGS and applied it to resolve serology discrepa

Frequency and prognostic value of D alloantibodies after D-mismatched allogeneic hematopoietic stem cell transplantation after reduced-intensity conditioning.

BACKGROUND: Due to the fact that the ABO and D system is inherited independently from the HLA system, approximately 40% of allogeneic hematopoietic stem cell transplants (HSCT) are performed across the blood group barrier. Reports on the development of de novo anti-D in patients undergoing reduced-intensity conditioning (RIC) followed by D-mismatched allogeneic HSCT are rare. The objective of this study was to evaluate the frequency of anti-D alloimmunization after D-mismatched HSCT following RIC and its prognostic impact on transplant outcome. STUDY DESIGN AND METHODS: Forty patients with he

Impact of ABO mismatching on the outcomes of allogeneic related and unrelated blood and marrow stem cell transplantations for hematologic malignancies: IPD-based meta-analysis of cohort studies.

BACKGROUND: The impact of donor-recipient ABO matching on outcomes after allogeneic stem cell transplantation has been a matter of controversy. STUDY DESIGN AND METHODS: Individual patient data-based meta-analysis was conducted with a pooled data set provided through six published and one unpublished cohorts. Outcomes in recipients of peripheral blood or bone marrow transplantation for hematologic malignancies were evaluated. A multivariate Cox model was used to adjust differences in outcomes of patients receiving ABO-matched grafts with those receiving major, minor, or bidirectional mismatch

Prophylactic red blood cell exchange for prevention of severe immune hemolysis in minor ABO-mismatched allogeneic peripheral blood progenitor cell transplantation after reduced-intensity conditioning.

BACKGROUND: Delayed severe immune hemolysis due to donor-derived passenger lymphocytes is observed in minor and/or bidirectional ABO-mismatched transplants, especially after reduced-intensity conditioning (RIC). The incidence is reported in up to 30 percent of patients and can result in multiorgan failure (MOF) and death. STUDY DESIGN AND METHODS: A first group of 32 patients (historical control) underwent RIC followed by allogeneic hematopoietic peripheral blood progenitor cell transplantation at our institution. In 5 of 10 patients with a minor and/or bidirectional ABO-mismatched graft, sev

Predicting the effect of transfusing only phenotype-matched RBCs to patients with sickle cell disease: theoretical and practical implications.

BACKGROUND: Transfusing only phenotype-matched RBCs has been recommended to reduce the incidence of alloimmunization to blood group antigens in patients with sickle cell disease (SCD). STUDY DESIGN AND METHODS: The expected benefit of phenotype matching was determined by identifying which of the existing blood group alloantibodies in patients with SCD who received conventional transfusions would not have been formed if they had received only phenotype-matched RBCs. By use of each patient's alloantibodies as a baseline, it was possible to identify specific alloantibodies that would not have be

ABO blood group distribution in patients with upper urinary tract stones: a case-control study.

Stones can form anywhere in the urinary system but most commonly develop in the upper urinary tract, particularly the kidneys and ureters. In this study, we examined the relationship between ABO blood type and upper urinary tract stone formation in a region with high disease incidence. This retrospective case-control study included 1,000 patients diagnosed with upper urinary tract stones (kidney or ureter) and 1,000 randomly selected controls presenting to urology outpatient clinics between January 2022 and December 2024. Demographic characteristics, ABO blood groups, body mass index (BMI), an

Effects of immunoadsorption combined with membrane filtration on complement markers - results of a randomized, controlled, crossover study.

The complement system has been implicated in several kidney diseases, such as antibody-mediated rejection after kidney transplantation. Antibody-depletion techniques allow successful ABO- and/or HLA-incompatible transplantation. Considering the IgG removal, the use of semi-selective immunoadsorption (IA) has been advocated. However, because of results on incomplete IgM depletion, the adjunctive use of membrane filtration (MF) has been proposed to enhance the removal of macromolecules and to interfere with complement activation. This secondary endpoint analysis of a recently published randomize
