Pediatric internuclear ophthalmoplegia.

BACKGROUND: Internuclear ophthalmoplegia (INO) is a rare eye movement disorder in the pediatric population. METHODS: We performed a retrosp … res a full neurologic evaluation and careful follow-up to assess eye position and potentially treat amblyopia.

Large orbital osteoma arising from orbital roof: Excision through an upper blepharoplasty incision.

Osteoma is the most common benign tumor of facial bony structure. Intraorbital extension is extremely rare. We report a case of intraorbital, pedicled osteoma originating from the left orbital roof. The patient's ophtalmological examination revealed; only light perception due to the severe amblyopia and posterior capsular opacification in the right eye, and complete visual acuity in the left. His left eye was proptotic and directed to downward lateral gaze position. Using upper blepharoplasty incision, the osteoma was completely excised. There were not any postoperative complications and all o…

Hunter syndrome (MPS II-B): a report of bilateral vitreous floaters and maculopathy.

PURPOSE: To describe bilateral vitreous opacities and maculopathy in a child with Hunter syndrome. METHODS: A case involving fundus examination, photography, and optical coherence tomography. Medline and Embase searches found no reference to vitreous or macular abnormalities in Hunter syndrome. RESULTS: A two-year-old boy with Hunter syndrome was found to have asymmetric, bilateral, free-floating vitreous opacities that remained stable over six years. Amblyopia and exotropia improved with occlusion therapy and spectacle correction. Visual acuity decreased when bilateral maculopathy supervene…

The ocular manifestations of Jacobsen syndrome: a report of four cases and a review of the literature.

PURPOSE: We report ophthalmic manifestations in four Jacobsen syndrome cases, review the literature, and suggest phenotype-genotype correlations. METHODS: Chart review of Ocular Genetics Program patients at The Hospital for Sick Children, Toronto, Canada. RESULTS: Four del11qter cases are presented. Hypertelorism/telecanthus, abnormally slanted palpebral fissures, abnormal retinal findings, nasolacrimal duct obstruction, anomalous extraocular muscles, amblyopia, and microcornea were found. CONCLUSIONS: We report typical findings and novel ocular presentations. Visual prognosis is generally …

Corneal ulcers in patients with Apert syndrome.

Morbidity from corneal ulcers is often severe in patients with Apert syndrome. These patients are at an increased risk of developing corneal ulcers because of the compromised corneal environment secondary to exophthalmos. During the past 6 years, three of five patients treated for Apert syndrome at our hospital have developed corneal ulcers. We present a case series discussing each patient, reasons for the development of ulceration, treatment, and outcomes. Morbidities in our group of patients included decreased visual acuity, opacified corneas, amblyopia, and blindness. Treatment is often dif…

Monocanalicular intubation for dacryostenosis in oculo-auriculo-vertebral dysplasia (hemifacial microsomia) with congenital corneal anesthesia.

PURPOSE: To present a case of oculo-auriculo-vertebral dysplasia associated with corneal anesthesia and ipsilateral dacryostenosis that was … e constant medial gaze because of wide lateral tarsorrhaphy and contralateral occlusion therapy for amblyopia. CONCLUSIONS: Monocanalicular stents may aid the treatment of dacryostenosis in the face of compromised corneal sensation.

Implantation of posterior chamber IOL was significantly associated with good visual outcomes (60.7%). Stereopsis was achieved in 9.3% of patients, and 70.9% of patients demonstrated BSV. Amblyopia independently reduced the likelihood of achieving BSV (odds ratio = 0.26, P = 0.015). CONCLUSIONS: Long-term motor alignment is maintained in about half of IET patients, though stability declines over time. Male sex, larger deviation, and higher su …

DESIGN: R … s of treatment at earlier ages, increased incidence of treatment by all modalities, higher rates of amblyopia, worse final visual acuity, and more frequent developmental delay. CONCLUSION: While treatment can stabilize the disease, patients face ongoing challenges, including visual and developmental complications. Lower GA, lower BW, and central nervous sy …

Retinopathy of prematurity screening: prevalence and risk factors of ophthalmic complications in non-treated preterm infants.

INTRODUCTION: Retinopathy of prematurity (ROP) is a vision-threatening disease of premature infants. Practice guidelines recommend that all infants screened for ROP receive follow-up eye examinations to screen for ophthalmic complications.1 The purpose of this study was to identify risk factors for the development of strabismus, amblyopia, high refractive error, and cataracts among ROP-screened, non-treated infants. METHODS: Retrospective single-centre study of ROP-screened, non-treated premature infants with ophthalmic follow-up. Clinical variables were screened for association with ocular f…

Penetrating Keratoplasty in Congenital Glaucoma.

BACKGROUND: Childhood glaucoma is one of the most common causes of corneal opacity in childhood and is associated with various pathological corneal changes, including corneal enlargement, corneal clouding, and edema. Congenital glaucoma (CG) may cause a decrease in vision outcomes due to corneal opacity or clouding, which is often associated with stimulus deprivation amblyopia. Therefore, to create a balance between preventing amblyopia and sustaining corneal clearance, patients with CG can be managed with early penetrating corneal transplantation surgery along with advanced glaucoma managemen…

Aim: To report the cases of five children with unilateral advanced glaucoma in isolated congenital ectropion uveae (CEU) with ipsilateral pt … iltration surgery, resulting in reasonable IOP control. Despite the older age, postoperative strict amblyopia treatment resulted in significant improvement in vision. Conclusions: Although ectropion uveae and ptosis have been present since birth, unilaterality, and the asymptomatic nature of the disease led to the late presentation with irreversible damage. …

Posterior chamber intraocular lenses: long-term results in pediatric cataract patients.

Major problems in pediatric cataract patients include noncompliance with contact lenses resulting in amblyopia. Advances in intraocular cataract surgery have provided a better environment to perform intraocular lens (IOL) implantation in children. We prospectively analyzed the results of 34 consecutive pediatric patients who underwent cataract removal and insertion of an IOL. Operative technique performed was posterior scleral beveled or frown incision and IOL endocapsular fixation. Subgroups included 10 eyes with traumatic cataract, and 24 eyes with developmental cataracts. Six patients had b…

Management of iatrogenic globe perforation during peribulbar anesthesia with submacular hemorrhage.

A patient with an amblyopia and poor vision in the fellow eye experienced iatrogenic globe perforation during peribulbar anesthesia for cataract surgery. The injection and surgery were immediately aborted. Visual acuity was hand motion, and retinal examination revealed retinal detachment, hemorrhagic choroidal detachment, and submacular hemorrhage. These were initially managed medically until the hemorrhagic choroidal detachment resolved and afterward by combined phacoemulsification with pars plana vitrectomy, subretinal recombinant tissue plasminogen activator, and gas tamponade.
