Acid-related disorders are common conditions that negatively impact quality of life for a significant number of people nationwide. The pathology of these conditions involves an imbalance between acid secretion by gastric parietal cells and the ability of upper GI tract mucosa to defend against the effects of the acid. Therefore, therapy is targeted at elevating gastric pH. PPIs are used to control the effects of excessive acid secretion. This class of drugs has a unique mechanism of action that inhibits the final pathway to gastric acid secretion--the parietal cell proton pump.

Topical therapy can be considered the standard treatment for distal ulcerative colitis. The group of drugs of first choice are the aminosalicylates which are effective in inducing remission in acute disease as well as in preventing relapse. The isolation of Helicobacter pylori from human gastric mucosa by Warren and Marshall in 1984, has given new approaches in etiology of peptic ulceration. Approximately all of duodenal ulcer patients have displayed antral gastritis with Helicobacter pylori, made strong suspicion of the bacteria in the etiology of duodenal ulcer. In therapy, combination of antibiotics to acid suppressive therapy should be preferred to antibiotics only.

All vertebrates secrete gastric acid. Acid denatures the proteins in the food and thus makes them more accessible to proteolytic enzymes, and it kills swallowed micro-organisms. Gastric acid plays an important pathogenetic role in peptic ulcer disease and reflux oesophagitis. In these diseases, drugs that inhibit secretion of gastric acid will heal the lesions and suppress the symptoms. The effect of colloidal bismuth subcitrate (De-Nol) on symptoms, Helicobacter pylori status and histological features was studied in 35 patients with non-ulcer dyspepsia.
