Efficacy of a portable acustimulation device in controlling seasickness.

BACKGROUND: Nausea and vomiting caused by motion sickness are serious and sometimes debilitating symptoms for commercial and recreational seafarers. HYPOTHESIS: In order to treat these symptoms, a portable device, the Relief Band, was designed to deliver acustimulation to the Neiguan (P6) acupuncture point. METHOD: Nine volunteers used the device on a placebo point or the P6 active point on the open seas outside the San Francisco Bay. Motion sickness symptoms were graded from 1 ("feel fine") to 5 ("intermittent vomiting, with or without nausea"). RESULTS: Five subjects with motion sickness

Effects of three hydration beverages on exercise performance during 60 hours of heat exposure.

Without adequate hydration, profound heat stress and dehydration can occur in people who work in hot environments. This study evaluated the effectiveness of three beverages on temperature regulation, cardiovascular response, and work performance during 60 h of dry heat exposure. Volunteer subjects lived in a climatic chamber in simulated desert conditions varying from 25-45 degrees C at 20% relative humidity. They performed three submaximal exercise sessions on a motorized treadmill at 4-h intervals each day. All of the subjects consumed one of three beverages: a water placebo, a 5% carbohydra

G-LOC recovery with and without G-suit inflation.

During acceleration (+Gz) training in the human centrifuge, the anti-G suit (AGS) is usually deflated as acceleration decreases upon termination of the exposure, regardless of the reason for termination, including +Gz-induced loss of consciousness (G-LOC). This is when the trainee most needs the support provided by the AGS. A method to reduce the time of incapacitation resulting from G-LOC was evaluated. The standard CSU15-P suit worn by 30 aircrew while undergoing +Gz tolerance training was inflated to 10 psi immediately upon G-LOC (GS group). Incapacitation times and flailing activity were r

Astronaut psychiatric selection procedures: a Japanese experience.

This paper presents the results of the first application of a structured NASA psychiatric interview for Japanese astronaut selection. We undertook a semi-structured, diagnostic, clinical psychiatric interview with 45 Japanese astronaut applicants for the position of mission specialist-astronaut, and performed "select-out" psychiatric evaluations according to DSM-III-R psychiatric diagnostic criteria. Two applicants (4.4%) who met Axis I or Axis II disorder criteria in DSM-III-R were evaluated as "Disqualified." There were 13 applicants "Qualified with Reservation"; the majority of their diagno

Perceived exertion during submaximal G exposures before and after physical training.

Ratings of perceived exertion (RPE) were registered at submaximal levels in G endurance tests of a combined strength and endurance training program in 17 pilots. After 12 months of physical training, the endurance G tolerance (time to exhaustion during simulated aerial combat maneuver), increased by a mean of 40% (p < 0.001), while the mean RPE at 5 min submaximal G exposure decreased by 1.2 units (p < 0.02). Following 12 months of physical training, a significant relationship was observed between the improvement of the endurance G tolerance and the decrease of the RPE at 5 min (p = 0.05). Mea

A review of otolaryngologic aircrew disqualification in the Chinese Air Force, 1961-90.

Otolaryngologic disease was the third most common medical cause of permanent grounding, accounting for 145 (12.2%) of 1,186 aircrew members permanently grounded for various medical reasons in the General Hospital of the Chinese Air Force, 1961-90. We reviewed the medical files of these 145 grounded aircrew members. Their mean age was 31.7 years, their mean flight time before grounding was 878.3 hours, and the mean course of disease from onset to disqualification for flying was 3.9 years. Nearly half of them flew in fighter aircraft and 91 (62.8%) were pilots. The most common otolaryngologic co

Changes in the central nervous system and their clinical correlates during long-term spaceflight.

The purpose of this paper is to review the metabolic, neurotransmitter, and morphological changes that occur in the human central nervous system (CNS) during long-duration spaceflight. If there are to be either permanent space habitats or long-duration interplanetary missions, we must determine if there will be any detrimental reversible or irreversible effects on the brain from this prolonged exposure to space. We must also determine how these effects might manifest themselves in performance, psychological, or cognitive dysfunction. The space environment has many characteristics, including mi

Effects of phenytoin on vection-induced motion sickness and gastric myoelectric activity.

The purpose of this study was to test the prophylactic effects of a single low dose of phenytoin on motion sickness. In this double-blind study, fasted male subjects who were susceptible to motion sickness were given either a 200-mg tablet of phenytoin (N = 19) or a placebo (N = 16). Electrogastrograms (EGG's) were recorded predrug, postdrug (4 h after ingestion of drug), before drum rotation, and during drum rotation. During testing, subjects were exposed to an optokinetic drum which was stationary for 8 min and which then rotated at 10 rpm for 16 min. The results showed that the phenytoin su

A comparison of the efficacy of cinnarizine with scopolamine in the treatment of seasickness.

Scopolamine was compared with cinnarizine in a double-blind sea trial involving 179 subjects from the crews of 2 warships. Medication was initiated prophylactically when weather information indicated the approach of nauseogenic conditions. Ship motion was measured during drug treatment periods. In one ship, moderate to severe nauseogenic conditions were encountered; a parallel group comparison was achieved in this. In the other ship, the motion experienced was of a mild nature; a crossover comparison was achieved. Scopolamine was shown to be more effective than cinnarizine in protecting agains

Coriolis effects are principally caused by gyroscopic angular acceleration.

A cause of nausea evoked by cross-coupled rotation (termed Coriolis stimulus) was determined. Subjects were provided with two types of cross-coupled rotations: neck-forward flexion (Neck Flx) and upper body-forward flexion (Body Flx) during horizontal whole body rotation at a constant angular velocity. These Coriolis stimuli were given alternatively in an experimental sequence, and the severity of the nausea they evoked was compared by the subjects. The results indicated that the same quality of nausea was evoked by a slightly higher angular velocity during Body Flx (100.5 degrees/s) than duri

Differences in otolith and abdominal viscera graviceptor dynamics: implications for motion sickness and perceived body position.

Human graviceptors, located in the trunk by Mittelstaedt, probably transduce acceleration by abdominal viscera motion. As demonstrated previously in biodynamic vibration and impact tolerance research, the thoraco-abdominal viscera exhibit a resonance at 4 to 6 Hz. Behavioral observations and mechanical models of otolith graviceptor response indicate a phase shift, increasing with frequency between 0.01 and 0.5 Hz. Consequently, the potential exists for intermodality sensory conflict between vestibular and visceral graviceptor signals, at least at the mechanical receptor level. The frequency ra

The possible role of nystagmus in motion sickness: a hypothesis.

An alternative hypothesis to that provided by conflict theory is formulated to account for motion sickness. The new approach is predicted on the oculocardiac reflex (i.e., bradycardia produced by extra-ocular muscle traction) (18) and empirical evidence that retrobulbar anesthesia significantly lowers the incidence of emesis after strabismus surgery (14). Eye muscle traction is presumed to elicit afferent signals that ultimately stimulate the vagus nerve (15). The same neuromotor sequence is presumed to occur during reflexive eye movements under vestibular control and during more complicated c

Perfusion of the visual cortex during pressure breathing at different high-G stress profiles.

The effects of pressure breathing for G protection (PBG) on perfusion of the visual cortex were studied in a subject during various high-G stress profiles. Blood flow velocity was measured in the posterior cerebral artery using a transcranial Doppler (TCD) ultrasound instrument. The G profiles examined included gradual and rapid onset rates. Mean cerebral blood flow velocity (MCBFV) declined with increasing +Gz with G-suit protection alone. The MCBFV increased in direct proportion with increase in +Gz acceleration with PBG. The mediating mechanisms for the effects of PBG may include improved g

Effects of orthostatic and anti-orthostatic stress on patent and stenotic coronary arteries in swine.

Head-up tilt (HUT) followed by head-down tilt (HDT) has been used to simulate the acute phase of adaptation to microgravity. This study evaluates the effects of HUT and HDT on the coronary circulation before and during coronary stenosis. Seven pigs were placed in the prone position and exposed to the following orientations for 20 min each: 1) 0 degrees horizontal (HZ); 2) +70 degrees HUT; and 3) -15 degrees HDT. The swine were then placed in the HZ position for 20 min while hemodynamics returned to baseline. The diameter of the left anterior descending (LAD) coronary artery was reduced to a po

Operation Everest II: gas tensions in expired air and arterial blood at extreme altitude.

Measurements in alveolar gas have suggested extreme hypocapnia and alkalosis on the summit of Mt. Everest. However, tensions in both expired gas and arterial blood have not been reported for the summit of Mt. Everest (PIO2 = 43 mm Hg). To approach the problem, we examined alveolar (and end-tidal) and arterial gas tensions in 6 healthy men who completed a 40-d chamber study to the simulated "summit," with 20 d above 6,400 m and 9 d above 8,000 m. In 27 simultaneous samples of alveolar air and arterial blood for inspired oxygen tensions ranging from PIO2 of 55 mm Hg (7,380 m) to 43 mm Hg, the me

Computerized task battery assessment of cognitive and performance effects of acute phenytoin motion sickness therapy.

Phenytoin was previously evaluated as an efficacious anti-motion sickness therapy in more than 24 individuals. To be effective, this short-term, novel therapy requires serum levels in the anticonvulsant range; therefore, it became imperative that the effects of phenytoin upon cognition and performance be quantified. Each subject was tested on a performance battery (Criterion Task Set) on two occasions while on either phenytoin treatment or placebo for subsequent statistical comparison. We evaluated 23 subjects with phenytoin serum levels ranging from 8.9 to 23.9 micrograms/L. While subjects wi

Treatment efficacy of intramuscular promethazine for space motion sickness.

Intramuscular promethazine and its efficacy in the treatment of Space Motion Sickness (SMS) were evaluated using standardized questions administered during postflight debriefings to crewmembers immediately after their first Shuttle flight. Space Motion Sickness was graded as none, mild, moderate, or severe, based on published criteria. Immediate symptom relief (within 1-2 h) was evaluated by subjective reports; medication efficacy was based on scores derived from the four most frequently reported symptoms of SMS: nausea, vomiting, loss of appetite, and stomach awareness. Scores were given for

Phenytoin as a countermeasure for motion sickness in NASA maritime operations.

Seasickness is the most prevalent form of motion sickness and is an operational problem during Space Shuttle Solid-fueled Rocket Booster (SRB) retrieval. Phenytoin has been shown to protect against motion sickness induced by Coriolis stress. We exposed SRB recovery personnel to off-vertical rotation and sea motion after phenytoin or placebo. Phenytoin blood levels of at least 9 micrograms/ml were protective against motion sickness at sea. No change in susceptibility to nitrogen narcosis was seen in divers in chamber tests at 460 KPa. Phenytoin was used during performance of critical and hazard

Motion sickness, vascular changes accompanying pseudo-coriolis-induced nausea.

We have previously shown that objective measurements of blood flow changes constitute a useful index in assessing susceptibility to nausea produced by cross-coupled angular accelerations (Coriolis stimulation). The findings of this study confirm the value of using the same technique for assessing the degree of nausea produced by pseudo-Coriolis effects.
