Fatal Hepatitis C after Chemotherapy in a Patient with Malignant Lymphoma: Possible Reactivation of Seronegative Occult Hepatitis C Virus Infection Due to Chemotherapy.

A 79-year-old man with lymphoma who tested negative for anti-hepatitis C virus (HCV) antibody received rituximab-containing chemotherapy. Liver dysfunction of unknown cause had persisted since the second cycle of chemotherapy. Ten months after treatment, he rapidly developed massive ascites and atrophy of the liver, and we detected HCV RNA in his serum using real time polymerase chain reaction. Furthermore, medical interviews showed that the patient had no episodes for acute HCV infection, but he did have a history of unspecified liver dysfunction. These findings support the possibility of the

Frequency of bacille Calmette-Guerin (BCG) and Mycobacterium tuberculosis in tissue biopsy specimens of children vaccinated with BCG.

Vaccination of all newborns with bacille Calmette-Guérin (BCG) vaccine is a standard practice in developing countries. Disseminated mycobacterial infection in an immunocompromised child can be caused by BCG and other mycobacteria. A total of 21 patients with a histopathologic diagnosis of mycobacterial infection were studied in a period of 4 years. DNA was extracted from formalin-fixed, paraffin-embedded tissues. Real-time polymerase chain reaction was performed to determine the mycobacterial species. The overall sensitivity of the assay was 71.5%. The prevalence rates of BCG, Mycobacterium tu

A fatal case of West Nile virus infection in a bone marrow transplant recipient.

West Nile virus (WNV) can cause severe, potentially fatal neurological illnesses, which include encephalitis, meningitis, Guillain-Barré syndrome, and anterior myelitis. Because of the short viremic phase, WNV infection is most commonly diagnosed by detection of immunoglobulin M antibody to WNV in serum or cerebrospinal fluid (CSF). We describe a patient with T cell lymphoma who had undergone a T cell-depleted bone marrow transplantation and developed fatal WNV infection. The results of serological tests of blood samples and of CSF tests were negative. Diagnosis was made postmortem by a positi

Colonic Histoplasma capsulatum Infection Simulating a Polypoid Lesion: Reactivation of a Long-Dormant Disease in a Patient With Chronic Renal Failure.

A 73-year-old Japanese man undergoing hemodialysis for chronic renal failure, with a history of sigmoid colon low-grade adenoma, underwent follow-up colonoscopy. A 5 mm rectal polypoid lesion was identified and resected by endoscopic mucosal resection. Histopathological examination revealed granulomas and clusters of histiocytes containing intracytoplasmic yeast-like organisms. Polymerase chain reaction analysis of paraffin-embedded tissue identified the genome of Histoplasma capsulatum. The patient had visited Brazil 30 years earlier, where he was presumed to have acquired the infection witho

Radiological Finding of Crazy-Paving Pattern in COVID-19 Pneumonia.

The recent global pandemic of coronavirus disease 2019 (COVID-19) has brought many radiographic findings in other respiratory disease processes. One of these radiological findings is crazy paving. This paper discusses crazy paving in a 75-year-old female with dyspnea, nonproductive cough, pleuritic chest pain, and a polymerase chain reaction (PCR) positive test for COVID-19 infection. Chest CT showed ground-glass opacities and interlobular septal thickening consistent with a crazy-paving appearance. As part of the common CT findings of patients with active COVID-19 infection, crazy paving shou

Systemic polyarteritis nodosa associated with acute Epstein-Barr virus infection.

Polyarteritis nodosa (PAN) is a rare cause of systemic vasculitis in children, affecting medium and small-sized arteries. We report on a patient who presented with prolonged fever, shock, acute renal failure with nephrotic range proteinuria, hypertension, and sudden deterioration of consciousness. Cranial tomography revealed a left extensive hemorrhagic lesion. Renal biopsy revealed a large fibrinoid necrosis lesion consistent with PAN. Epstein-Barr virus (EBV) infection was confirmed by real-time polymerase chain reaction (RT-PCR) detection of EBV DNA. The patient was successfully treated wit

Congenital enterovirus 71 infection: a case study with virology and immunohistochemistry.

Previously reported enterovirus 71 (EV71) infections associated with hand-foot-mouth disease, aseptic meningitis, encephalitis, polio-like myelitis, and paralysis all have involved young children. We report a 28-year-old woman who possibly contracted EV71 infection during pregnancy. Obstetric ultrasonograms at 25 weeks of gestation revealed an abnormal placenta, as well as hepatosplenomegaly, liver calcification, excessive ascites, and mild hydrocephalus of the fetus. Presence of EV71 was determined by immunodot blotting, virus culture, and partial sequencing of the amplified product of nested

Cytomegalovirus-associated Disseminated Intravascular Coagulation in an Immunocompetent Patient: A Case Report and Literature Review.

A healthy 20-year-old woman presented with high fever and fatigue. Laboratory findings included normal complete blood counts and the absence of atypical lymphocytes. The patient had markedly elevated C-reactive protein, hyperferritinemia, and coagulation abnormalities consistent with disseminated intravascular coagulation, while imaging studies and extensive infectious and autoimmune workups were unremarkable. Serological testing and polymerase chain reaction confirmed a primary cytomegalovirus (CMV) infection. The administration of ganciclovir followed by corticosteroid therapy resulted in th

Ehrlichia canis infection in two dogs that emigrated from endemic areas.

Two dogs, emigrated from Zambia and China to Japan, were diagnosed with Ehrlichia canis infection. Both cases had thrombocytopenia, non-regenerative anemia, and hypergloblinemia with polyclonal gammopathy. Case 1 had ataxia of the hind limbs. Severe meningitis was revealed by magnetic resonance imaging examination. Intracytoplasmic inclusions were observed in mononuclear cells of cerebrospinal fluid. Case 2 had a history of bilateral epistaxis, and severe pancytopenia was noticed in complete blood count. Diagnosis was finally achieved by nested polymerase chain reaction and sequence analysis.

Is pyogenic granuloma associated with Bartonella infection?

Lobular capillary hemangioma and bacillary angiomatosis due to Bartonella infection share several clinical and histopathologic characteristics. We sought to determine whether lobular capillary hemangioma is caused by the same agent as bacillary angiomatosis. Forty-five pathology specimens with a histologic diagnosis of lobular capillary hemangioma obtained from patients with the same clinical diagnosis were tested by immunohistochemistry and polymerase chain reaction for the presence of DNA elements of Bartonella spp. None of the 45 lobular capillary hemangioma specimens tested positive for Ba

[The association between infection of Epstein-Barr virus and chronic periodontitis].

OBJECTIVE: To investigate the relationship between Epstein-Barr virus (EBV) infection and chronic periodontitis. METHODS: In 62 chronic periodontitis patients, subgingival plaque samples were collected from active periodontitis site, quiescent periodontitis site, and initial gingivitis sites of each subject. The nested polymerase chain reaction (nested-PCR) was used to determine the presence of EBV. RESULTS: The EBV was detected in 58.0% of the samples from active periodontitis sites, which was significantly higher than that from quiescent periodontitis sites (58.0% vs 22.6%, P < 0.01) and f

[Parvovirus B19 infection--an incidious chameleon].

Parvovirus B19 is a common source of infection with a seroprevalence of 60-70 per cent in the adult population. The most common manifestation is erythema infectiosum ("fifth disease"), with exanthem, fever and upper airway symptoms in children. The infection can give rise to a multifaceted clinical picture and is probably underdiagnosed, particularly in risk groups (individuals with haemolytic anaemia or immunosuppression, and fetuses). Serological diagnosis can now be complemented with the demonstration of viral DNA using the PCR (polymerase chain reaction) test in various body fluids, or tis

Endoscopic revision of an infected Syme amputation. Case report and review of the literature.

Arthroscopic revision of intra-articular infection is more and more accepted. To our knowledge, there is no description of endoscopic management of an infected amputation stump. This paper presents the case of a 54-year-old man after a Syme amputation of his left leg because of diabetic gangrene of the foot. A local infection of the stump was treated successfully with two endoscopic debridements and lavage. Additionally, the patient received local antibiotics by the application of antibiotic collagenous fleece and chains. This therapeutic management combined with early weight-bearing produced

[The significance of small mammals in the epizootiology of leptospirosis in livestock].

1. Favourite species of rodents in sheds and their surroundings are rats (Rattus norvegicus, R. rattus) and house mouse (Mus musculus). From the epizootiological point of view only R. norvegicus is important as the maintenance host of L. copenhageni. 2. In surrounding of sheds vole (Microtus arvalis) and back-striped field mouse (Apodemus agrarius) are sources of infection for livestock. 3. Transmission of leptospira occurs via infected environment. The tenacity of Leptospira mozdok and L. grippotyphosa under field conditions suffices to maintain the chain of infections.

West Nile virus in mosquitoes of northern Ohio, 2003.

From June 19, 2003 to August 18, 2003, we surveyed the mosquitoes of Oberlin, OH, for West Nile Virus (WNV) infection using reverse transcriptase-polymerase chain reaction. A total of 12,055 mosquitoes, representing 17 species or species groups and 4 genera, were collected in gravid traps at seven sites throughout the city, with Culex pipiens/restuans being the most abundant and showing the highest minimum infection rate (MIR) of 0.78. This represents a decrease in WNV enzootic activity from the previous year. Both Cx. pipiens/restuans abundance and MIR increased significantly with date. Howev

Spastic ataxia associated with human T-cell lymphotropic virus type II infection.

Human T-cell lymphotropic virus type one (HTLV-1) is associated with tropical spastic paraparesis or HTLV-I--associated myelopathy. We report 2 women with a spastic ataxic illness similar to HTLV-I--associated myelopathy infected solely with HTLV-II. Identification of HTLV-II infection was made serologically, by polymerase chain reaction, and by viral culture (in 1 woman). One woman, treated with 200 mg of danazol orally, three times daily, had pronounced improvement in ambulation, nocturnal spasticity, and nighttime urinary frequency. It appears that infection with HTLV-II may cause an illnes

The frequency of human parvovirus B19 infection in nonimmune hydrops fetalis.

Intrauterine infection of human parvovirus B19 has recently been identified as an etiology for nonimmune hydrops fetalis (NIHF) and fetal death. To examine the frequency of B19 infection in cases of NIHF, forty two cases of NIHF of unknown cause were tested for the presence of B19 antibodies by enzyme-linked immunosorbent assay (ELISA) and B19 DNA by polymerase chain reaction (PCR). Three maternal sera were positive for B19 IgM antibody and one fetal serum was positive for B19 DNA. Overall, four (10%) of the 42 cases were positive for B19 infection. All of the four cases positive for B19 infec
