Cochlear implantation in an intralabyrinthine acoustic neuroma patient after resection of an intracanalicular tumour.

This case study describes a therapeutic strategy using a cochlear implant for a bilateral acoustic neuroma deafened patient. The cochlear nerve had previously been sacrificed on one side during tumour removal, but on the remaining side a functioning cochlear nerve was assessed by electric promontory stimulation in spite of a neuroma extending into the vestibular labyrinth. The patient was successfully stimulated with a Nucleus 22-channel implant after removal of the intracanalicular portion of the neuroma via a middle fossa approach.

Survival of spiral ganglion cells in profound sensorineural hearing loss: implications for cochlear implantation.

Ninety-three temporal bones from 66 patients who were profoundly deaf during life were reconstructed by analysis of serial light microscopic sections. The correlations of total and segmental spiral ganglion cell counts with age, duration of hearing loss and profound deafness, and cause of hearing loss were evaluated. Bivariate analysis demonstrated that total spiral ganglion cell count tended to be lower in older than in younger deaf individuals and lower with longer duration of hearing loss and total deafness. However, multiple regression analysis demonstrated that the cause of hearing loss w

Systematic Review of the Adverse Events in Patients with Cochlear Implants following MRI.

BACKGROUND: Cochlear implants (CIs) are increasingly used for the treatment of sensorineural hearing loss. MRI is an essential diagnostic technique, but concerns about adverse events related to CIs have prompted device and imaging protocol innovations. The transition from axial to diametric internal magnets has significantly improved MRI safety. Despite regulatory approvals for MRI use with CIs under specified conditions, adverse events continue to be reported. In addition, many patients still have CIs with axial internal magnets. PURPOSE: Our goal was to summarize the types and frequencies o

Biohybrid cochlear implants: neural interfaces, regenerative pathways, and translational benchmarks.

Cochlear implants (CIs) restore hearing by electrically stimulating the auditory nerve, yet the millimeter-scale electrode-neuron separation limits spatial selectivity and temporal fidelity. Rather than relocating the electrode array, we propose a biohybrid strategy that preserves the conventional scala tympani insertion and closes this gap biologically by interposing living neural constructs between the array and the modiolus. This approach leverages the Canaliculi Perforantes of Schuknecht (CPS) to deliver spatially confined biochemical, mechanical, and electrical cues from the scala tympani

Establishing reliable neural health measures in adult cochlear implant users.

Following cochlear implantation, measures of auditory nerve function based on implant-driven electrical stimulation can be used to characterize neural health and support longitudinal monitoring and treatment outcome assessment, with particular relevance for advanced therapies targeting neural hearing loss. This study evaluated the reliability and measurement precision of neural-health measures derived from three complementary electrode-specific tests: panoramic electrically-evoked compound action potentials (PECAPs), interphase-gap (IPG) effects, and polarity effects. Six experienced adult coc…

Effects of Preoperative Factors on the Learning Curves of Postlingual Cochlear Implant Recipients.

OBJECTIVES: The substantial variability in speech perception outcomes after cochlear implantation complicates efforts to develop valid predictive models of these outcomes. Existing predictive regression models are too unreliable for clinical application, possibly because speech intelligibility (SI) after cochlear implant (CI) rehabilitation is often based on a limited number of assessments. The development of SI after CI has rarely been detailed, although knowing the shape of the learning curve can potentially improve predictive modeling. Knowing the learning curve after CI could also aid in s…

Spontaneous use of gestures during vocabulary testing in toddlers with cochlear implants.

In typically developing (TD) children, early gestures emerge before first words and predict spoken language. Language development in children with cochlear implants (CI) vary due to factors such as implantation age and exposure to spoken language, but variation is largely unexplained. Little is known about gestures in children with CI and how this is related to vocabulary. In this study, spontaneous use of gestures during assessment of receptive and expressive vocabulary was examined and related to vocabulary test results in ten children with CI, aged 25-39 months. The results were compared to

Listening Effort and Memory Effort in Cochlear Implant Users: A Pupillometry Study.

OBJECTIVES: Following a conversation involves numerous cognitive processes. Speech needs to be processed, retained, encoded and an answer needs to be prepared. Hearing impairment leads to degraded speech signal, which results in the need for additional cognitive resources to processing and encoding. This is particularly true for cochlear implant (CI) users, where the auditory nerve is stimulated electrically. The present study examined the cognitive effort required for listening (listening effort) and retaining sentences in memory (memory effort) among CI users by means of pupil dilation. The

Imaging the developing brain with near-infrared spectroscopy in cochlear implanted children.

Cochlear implants (CIs) have revolutionized how we treat hearing impairment. Despite major technological and clinical advances, some CI children's language abilities remain below those of their age-matched peers, and there is still considerable individual variability in final outcomes. One important factor underlying this may be individual differences in brain plasticity before and after implantation. However, the neural changes induced in the developing brain by deafness, language deprivation, and the restoration of hearing due to implantation are little understood, in part because the method

Preferences and Listening Efficiency of Adults With Cochlear Implants During Online Communication.

OBJECTIVES: In recent years, there has been a profound increase in the use of remote online communication as a supplement to, and in many cases a replacement for, in-person interactions. While online communication tools hold potential to improve accessibility, previous studies have suggested that increased reliance on remote communication poses additional challenges for people with hearing loss, including those with a cochlear implant (CI). This study aimed to investigate the preferences and speech-reception performance of adults with a CI during online communication. DESIGN: An online experi
