Mediation of Latinx health status disparities during the COVID-19 pandemic by social determinants of health.

PURPOSE: The hypothesis that health status disparities exist because of high levels of social disadvantages, the Social Determinants of Health - Health Disparities (SDOH-HD) hypothesis by Ramírez García [1], was investigated among adults (N = 1,833) attending SARS-CoV-2 testing events at 71 community sites. METHODS: A Multilevel Structural Equation Model (CFI = 1.0, SRMRs ≤ 0.03), with cross-sectional self-reported data, operationalized health status with latent individual level and latent community site level variables. At the individual level, health variables met full or partial metric equ

Perceived Homonegativity and Psychological Distress in Gay Men in Brazil: Does Skin Color Matter?

Homonegativity is associated with several adverse physical and mental health outcomes in gay men. However, the intersection between homonegativity and racism remains little investigated in Brazil. The aim of this study was to assess, in a sample of cisgender Brazilian gay men, associations between skin color, homonegativity, psychological distress, and socioeconomic variables. Method: A cohort of 229 Brazilian gay men, 151 (66%) white and 78 (34%) black or brown were assessed. Participants completed the Internalized Homophobia Scale, the Beck Depression Inventory-II, and the State-Trait Anxiet

The impact of patient-centred care on the relationship between access to care and subjective health outcomes amongst people experiencing homelessness: A mediation analysis.

People experiencing homelessness (PEH) have stated that impersonal care and feelings of discrimination are barriers to accessing healthcare which is associated with poor health outcomes amongst PEH. This study examined whether patient-centred care (PCC) mediates the relationship between access to care and subjective health outcomes. Using the 2014-15 Healthcare Center Patient Survey, we identified 1873 homeless and vulnerably housed adults. Items related to the delay or inability to receive primary or mental healthcare, PCC provided by healthcare providers, general health status, and the Kessl

Angiotensin Regulation of Amygdala Response to Threat in High-Trait-Anxiety Individuals.

BACKGROUND: The antihypertensive drug losartan has been shown to improve memory in humans as well as learning and fear extinction in rodent models, highlighting its potential to have similar synergistic effects on exposure-based cognitive behavioral therapy for anxiety disorders. This study investigated the effect of losartan on neural correlates of processing threat versus safety stimuli in highly anxious individuals to identify potential pathways of how the drug might facilitate psychological treatment. METHODS: Thirty healthy volunteers high in trait anxiety were randomly assigned to a sin

A biopsychosocial path model linking tobacco smoking to the Native American chronic pain disparity: findings from the Oklahoma Study of Native American Pain Risk.

BACKGROUND: Among US groups, Native Americans (NAs) have the highest rates of smoking and chronic pain. No study has examined whether smoking contributes to NA chronic pain disparities. PURPOSE: We tested a biopsychosocial model linking smoking with chronic pain among NAs. METHODS: Social, psychological, and biological variables associated with chronic pain risk and smoking were assessed in healthy, pain-free NAs and non-Hispanic Whites (NHWs). Participants were followed for 5 years to assess who did (N = 49) and did not (N = 151) develop chronic pain (pain rated ≥3/10 on most days lasting ≥

Child abuse and experiences of discrimination are linked to fear of childbirth among younger patients in the US: a cohort study.

BACKGROUND: Studies have documented more fear of childbirth (FoC) among younger people, resulting in psychological distress, health-risk behaviors, pregnancy termination, and elective Caesarian delivery. Little is known about FoC in the US, particularly among African Americans, despite higher rates of maternal morbidity and mortality. The purpose of this study is to examine FoC in a US sample of pregnant adolescents and young adults. METHODS: We surveyed FoC as part of a prospective cohort study focused on pregnant people ages 14-21 (n = 147). FoC was assessed online during the third trimeste

"When you first walk out the gates…where do [you] go?": Barriers and opportunities to achieving continuity of health care at the time of release from a provincial jail in Ontario.

We aimed to explore continuity of health care and health barriers, facilitators, and opportunities for people at the time of release from a provincial correctional facility in Ontario, Canada. We conducted focus groups in community-based organizations in a city in Ontario, Canada: a men's homeless shelter, a mental health service organization, and a social service agency with programs for people with substance use disorders. We included adults who spoke English well enough to participate in the discussion and who had been released from the provincial correctional facility in the previous year.

Discrimination, Bullying and Harassment in Surgery: A Systematic Review and Meta-analysis.

BACKGROUND: In 2015, the public media in Australia reported a series of life stories of victims who had been subjected to inappropriate behaviors in their surgical careers, bringing the profession into disrepute. Currently, limited data are available in the medical literature on discrimination, bullying and harassment (DBH) in surgery. This significant information gap prompted a systematic review to compile relevant information about DBH in surgical practice and training, in particular, its prevalence and impact. METHODS: A literature search was conducted using the MEDLINE, EMBASE and PubMed

Accessing HIV testing and treatment among men who have sex with men in China: a qualitative study.

Barriers to HIV testing and HIV care and treatment pose significant challenges to HIV prevention among men who have sex with men (MSM) in China. We carried out a qualitative study to identify barriers and facilitators to HIV testing and treatment among Chinese MSM. In 2012, seven focus group (FG) discussions were conducted with 49 MSM participants in Nanjing, China. Purposive sampling was used to recruit a diverse group of MSM participants. Semi-structured interviews were conducted to collect FG data. Major barriers to testing included gay- and HIV-related stigma and discrimination, relationsh

Dissociation, somatization, and affect dysregulation: the complexity of adaptation of trauma.

OBJECTIVE: A century of clinical research has noted a range of trauma-related psychological problems that are not captured in the DSM-IV framework of posttraumatic stress disorder (PTSD). This study investigated the relationships between exposure to extreme stress, the emergence of PTSD, and symptoms traditionally associated with "hysteria," which can be understood as problems with stimulus discrimination, self-regulation, and cognitive integration of experience. METHOD: The DSM-IV field trial for PTSD studied 395 traumatized treatment-seeking subjects and 125 non-treatment-seeking subjects w

Prefrontal NMDA receptors expressed in excitatory neurons control fear discrimination and fear extinction.

N-methyl-D-aspartate receptors (NMDARs) are critically involved in various learning mechanisms including modulation of fear memory, brain development and brain disorders. While NMDARs mediate opposite effects on medial prefrontal cortex (mPFC) interneurons and excitatory neurons, NMDAR antagonists trigger profound cortical activation. The objectives of the present study were to determine the involvement of NMDARs expressed specifically in excitatory neurons in mPFC-dependent adaptive behaviors, specifically fear discrimination and fear extinction. To achieve this, we tested mice with locally d

Limited modulation of the abuse-related behavioral effects of d-methamphetamine by disulfiram.

Disulfiram (Antabuse), an acetaldehyde dehydrogenase and dopamine-beta hydroxylase inhibitor, has shown promise in preclinical and clinical studies as a pharmacotherapy for cocaine addiction. However, the extent to which disulfiram may alter the abuse-related behavioral effects of related psychostimulants, such as methamphetamine, is unknown. Here, the therapeutic potential of disulfiram was evaluated by examining its impact on the reinforcing and discriminative stimulus effects of d-methamphetamine in adult rhesus monkeys (N = 4 per group). In subjects trained to respond for injections of met

Assessing the Cognitive Translational Potential of a Mouse Model of the 22q11.2 Microdeletion Syndrome.

A chromosomal microdeletion at the 22q11.2 locus is associated with extensive cognitive impairments, schizophrenia and other psychopathology in humans. Previous reports indicate that mouse models of the 22q11.2 microdeletion syndrome (22q11.2DS) may model the genetic basis of cognitive deficits relevant for neuropsychiatric disorders such as schizophrenia. To assess the models usefulness for drug discovery, a novel mouse (Df(h22q11)/+) was assessed in an extensive battery of cognitive assays by partners within the NEWMEDS collaboration (Innovative Medicines Initiative Grant Agreement No. 11500

A mobile, high-throughput semi-automated system for testing cognition in large non-primate animal models of Huntington disease.

BACKGROUND: For reasons of cost and ethical concerns, models of neurodegenerative disorders such as Huntington disease (HD) are currently being developed in farm animals, as an alternative to non-human primates. Developing reliable methods of testing cognitive function is essential to determining the usefulness of such models. Nevertheless, cognitive testing of farm animal species presents a unique set of challenges. The primary aims of this study were to develop and validate a mobile operant system suitable for high throughput cognitive testing of sheep. NEW METHOD: We designed a semi-automa
