[Surgical management of traumatic ear amputations: literature review].

Traumatic ear amputation (TEA) is a complete avulsion of a part or of the total auricular tissue. TEA are rare (only 74 cases have been described in the literature) and their handling is complex. The surgeon's objective is to obtain the best cosmetic result without demolishing the auricular area in order to allow future ear reconstruction in case of replantation failure. Many techniques of ear replantation have been described in the literature during the last 30 years: microsurgical replantation, pocket techniques and reattachment techniques. Microsurgical replantation should be achieved every…

Identification of Id1 in acquired middle ear cholesteatoma.

OBJECTIVES: To determine (1) the relationship between chronic inflammatory changes in the ossicular chain area (OCA) and the formation of cholesteatoma and (2) the correlates between aberrant gene expression and abnormal proliferation of cholesteatoma. METHODS: Two hundred sixty-four ears with chronic otitis media that had undergone ear surgery were included in this study for statistical analysis of the relationship between abnormalities in the OCA and cholesteatoma. Fourteen middle ear cholesteatoma specimens were collected for immunohistochemical analysis of candidate molecules involved in …

Vestibular evoked myogenic potentials in middle ear effusion.

CONCLUSION: The vestibular evoked myogenic potential (VEMP) responses were significantly affected by middle ear effusion (MEE). The VEMP was either non-responsive or significantly delayed. However, after the conductive hearing loss (CHL) was significantly reduced with tympanic aspiration, the VEMP latencies and asymmetry ratio returned to the range of healthy controls. In addition, the recovery of VEMP response was prompt and immediate. OBJECTIVE: To evaluate the VEMP in various degrees of CHL resulting from MEE and to clarify the responses after the CHL was reduced. MATERIALS AND METHODS: V…

[Effects of nitrous oxide on middle ear pressure].

INTRODUCTION: Recent literature data suggest that permanent or reversible hearing loss may occur after general anesthesia. The etiology varies, while hearing loss following middle ear surgery is attributed to exposure to nitrous oxide (N,0). The objective of our study was to measure, using tympanometry, the middle air pressure change caused by nitrous oxide during general anesthesia and to establish its emetogenic effects during the postoperative period. MATERIAL AND METHODS: This academic (non-commercial) prospective study included two groups of patients (a total of 58), with ASA status I, I…

[Cerebrospinal fluid leak due to inner ear malformations].

A case of inner ear anomalies associated with recurrent meningitis found in a 66-year-old female is reported. The patient had had 2 episodes of meningitis in the previous one year period. Multidetector-row CT of the temporal bone and three-demensional fast imaging employing steady-state acquisition MRI revealed dysplasia of the bony labyrinth and enlarged fundus of the internal auditory canal (IAC). During surgical treatment, we confirmed the cerebrospinal fluid (CSF) leakage through a bony defect in the stapedial footplate. The inner ear was obliterated and contained small pieces of temporal …

[Correction of ear malformations with autologous rib cartilage].

INTRODUCTION: Ear malformations are mainly isolated deformations but exist also in combination with various syndromes. A visible malformation in cases of microtia is a problem for the entire family. Auricular reconstruction with autologous cartilage has been well established in the last decade. Optimization of the technique has led to improved and reliable results. OPERATION AND RESULTS: Based on the experience of 120 cases we developed a concept that allows total ear reconstruction in two operative steps. In the first operation autologous rib cartilage is harvested and a natural framework is…

Cochlear implantation in children with inner ear malformations.

OBJECTIVES: The aim of this study was to assess the outcomes of cochlear implantation in children with inner ear malformations (IEMs). METHODS: A retrospective review of 212 children who received implants from September 1994 to May 2004 was performed. Forty-six of them had radiologic evidence of IEMs. The preoperative evaluations, intraoperative findings, postoperative complications, and performance outcomes were analyzed. For the comparative analysis of performance outcomes, the children with IEMs were matched and compared with children with a normal inner ear who had received implants. Stat…

Cerebellopontine angle meningioma resulting in middle-ear polyp.

Extracranial spread of meningiomas to involve the middle ear is very rare. We present the case of a 43-year-old woman with a known cerebellopontine angle meningioma who subsequently presented with left-sided otalgia and a middle-ear mass extruding through the tympanic membrane due to local invasion of the meningioma. The tumour was excised surgically. A discussion of the relevant literature is also presented.

Severe acute middle ear infections: microbiology and treatment.

OBJECTIVE: The database at the ENT-Department, Aarhus University Hospital describing the specific local flora of acute otitis media (AOM) and acute mastoiditis (AM) in Aarhus County has become the key element in securing a rational and specific antibiotic treatment. We present our data concerning AOM, mastoidismus and AM. Our purpose is to determine (I) clinical presentation, (II) causative pathogens, (III) resistance patterns, (IV) type of antibiotic treatment, and (V) accuracy of the antibiotic treatment. METHODS: Patients under the age of 18 years, treated for AOM with extensive affected w…

Otosyphilis mimics immune disorders of the inner ear.

Syphilis is a well established cause of hearing loss. Sensorineural hearing loss may develop in the congenital or acquired form. The clinical course of the early acquired and late congenital forms are similar: sudden or rapidly progressive bilateral sensorineural hearing loss with mild vestibular symptoms. Cochleovestibular involvement in early acquired syphilis has been related to a basilar meningitis with lymphocytic infiltration of the labyrinth and VIIIth nerve. However, neurosyphilis and inner ear syphilis are not the same disease. Prompt diagnosis and treatment with corticosteroids and p…

Parotid masses in patients with previous ear surgery.

This paper describes three cases of patients presenting with lumps in their parotid gland, the origin of which was difficult to define. In each case the past medical history revealed that the patients had undergone previous ipsilateral middle ear surgery. We highlight the fact that where there has been previous incisions in the skin about the ear, there is a risk of epidermal inclusion cysts in the parotid gland. These cysts can occur many years after the initial surgery and therefore may not be identified as an obvious origin to lumps in the parotid gland. Rarely as this series highlights the…

Objective high-frequency tinnitus of middle-ear myoclonus.

Tinnitus produced by middle-ear myoclonus is a rare condition. In this article, a rare case of unilateral continuous high-frequency objective tinnitus caused by middle-ear myoclonus is described. This condition appears to be the second case reported in the literature. Otoscopic examination revealed visible rhythmic movements of the tympanic membrane. Weak clicking sounds were heard around the right ear by auscultation. Direct stimulation of the soft palate showed no evidence of palated myoclonus. Tympanometry confirmed rhythmic changes in the middle-ear compliance. The condition was effectivel…

Immune-mediated inner ear disease: 10-year experience.

BACKGROUND: Autoimmune inner ear disease (AIED) was first described in 1979 and the disease has become more widely recognized over the last decade. Limited information is available regarding clinical features of the disease, disease course, and response to treatment. OBJECTIVE: To analyze data from 42 patients with documented immune mediated inner ear disease to further define this syndrome. METHODS: A retrospective chart review was conducted on all patients considered to have AIED by the Otolaryngology division physicians and on all patients positive for antibody testing to inner ear antige…

Ear-nose-throat manifestations of autoimmune rheumatic diseases.

Ear-nose-throat (ENT) manifestations of connective tissue disorders represent a diagnostic challenge for clinicians as they often constitute the initial sign of an otherwise asymptomatic autoimmune disease. Moreover, in patients with known autoimmune rheumatic diseases, ENT manifestations can be overlooked. Hearing disturbances may be seen in patients with systemic lupus erythematosus, Wegener's granulomatosis, relapsing polychondritis, polyarteritis nodosa, Cogan's syndrome, Sjögren's syndrome, and less frequently in Churg-Strauss syndrome and Adamantiades-Behçet's disease. Nose and paranasal…

[Aural polyp in chronic inflammatory middle ear disease].

240 patients with chronic otitis media (COM) were studied: 166 ears termed as non cholesteatomatous otitis media and 74 with cholesteatoma. In 38 ears an aural polyp was found with no evidence of cholesteatoma in 19 ears (11.4%) whereas a cholesteatoma was present in the remaining 19 ears. The histology of the polyp and the characteristics of the chronic process were matched: a) The aural polyp is an infrequent complication in COM. b) After histological analysis was found to present two different pictures: The inflammatory reaction polyp, present in non cholesteatomatous COM; and the polyp wit…

Körner's septum (petrosquamosal lamina) and chronic ear disease.

To assess the clinical significance of Körner's septum (KS) or petrosquamosal (PS) lamina in different chronic ear pathologies, a surgical and anatomic study was carried out, in which the significance of KS was assessed in various otologic diseases. Of 639 patients who underwent ear surgery between 1998 and 2001, 426 were included in the study. The diagnoses were chronic otitis media without cholesteatoma (COM), tympanosclerosis (TS), adhesive otitis media (AOM) and cholesteatoma (COL) in 160, 100, 70 and 96 patients respectively. The operative records of the patients were reviewed for the pre…
