The treatment of intra-articular calcaneus fractures with severe soft tissue damage with a hinged external fixator or internal stabilization: long-term results.

We developed a hinged external fixator for the treatment of dislocated intra-articular calcaneus fractures with severe soft tissue damage. The external fixation was performed with a known external fixator system. The screw insertion points were biomechanically tested by defining a virtual rotation axis through the center of the talus to allow early active motion in the ankle joint. Long-term follow-up was performed after an average of 7.3 years. Results were graded with the American Orthopaedic Foot and Ankle Society (AOFAS) score. Radiographs were reviewed according to Sanders classification.

Paralysis of the first dorsal interosseous muscle after external fixation of a distal radial fracture treated by transfer of the extensor indicis proprius tendon.

We describe a patient in whom the motor branch to the first dorsal interosseous muscle was injured by the pins of an fixator used to treat an unstable fracture of the distal radius. She was successfully treated by extensor indicis proprius transfer to the base of the proximal phalanx of the index finger.

Unilateral hinged external fixation frame for elbow compression arthrodesis: the stepwise attainment of a stable 90-degree flexion position: a case report.

We report a case of an elbow arthrodesis using a modular hinged external fixation device in a patient who sustained a complex high-energy injury. The extensive bone and soft-tissue loss and local infection precluded restoration of the articular anatomy. Other reconstructive options, including total elbow arthroplasty and allograft reconstruction, were not appropriate in this clinical situation. A transelbow hinged external fixator was applied with the elbow in extension. Subsequently, the hinge was brought gradually into 90 degrees of flexion as the elbow fused. Fixation and arthrodesis were s

[Open wedge osteotomy and callus distraction by means of the external fixator in distal femur and proximal tibia in knee arthrosis with valgus and varus deformity].

Various methods can be applied for treatment of knee deformity. One of them is open wedge osteotomy and callus distraction by means of the external fixator. This paper presents the results of treatment of 16 patients with knee arthrosis associated with varus and valgus deformities (10 varus and 6 valgus deformities). Open wedge osteotomy of the varus deformity was performed in the proximal tibia, and in case of the valgus deformity in the distal part of the femur. The patients who were operated on had a knee varus larger than 10 degrees and a knee valgus larger than 12 degrees. Prior to open w

[Acute carpal tunnel syndrome after distal radius fractures--long term results of surgical treatment with decompression and external fixator application].

Acute Carpal Tunnel Syndrome (ACTS) is a relatively rare sequel of distal fractures of the radius. This paper is an analysis of the causes of ACTS and of long-term results of treatment by surgical decompression combined with use of external fixators. Out of 128 patients with distal radius fractures treated in our department 11 cases required surgical treatment because of ACTS. Fractures were classified according to the AO classification as C2 and C3. CTSA was diagnosed basing on typical clinical signs: pain numbness over the radial three and a half fingers. All patients were treated surgically…

Radiostereometric analysis of distal radial fracture displacement during treatment: a randomized study comparing Norian SRS and external fixation in 23 patients.

In a randomized study, we included 23 osteoporotic patients with a distal radial fracture and loss of reduction after 1 week. The facture was re-reduced. In one group, a self-setting hydroxyapatite, Norian SRS, was injected into the fracture and the wrist was immobilized for 2 weeks with a dorsal splint (n 12). In the other group, the fracture was immobilized for 5 weeks with an external fixator (n 11). During the operation, the fracture fragments were marked with tantalum markers, so that loss of reduction during the immobilization and after mobilization could be studied with radiostereometri

Salvage After Patient-Driven Delayed Conversion From External Fixation to Intramedullary Nailing in an Open Tibial Shaft Fracture: A Case Report.

BACKGROUND: Open tibial shaft fractures require timely management of both skeletal instability and soft-tissue injury. Temporary external fixation is useful for early stabilization, but prolonged retention without progression to definitive fixation may increase the risk of delayed union and other complications. This case illustrates a patient-driven delay from the preferred staged pathway and subsequent salvage after restoration of mechanical stability and biological support. CASE PRESENTATION: A 40-year-old man sustained a Gustilo-Anderson Type II open fracture of the right tibial shaft with

Staged non-bridging circular external fixation with sequential osteoporosis therapy for MRSA-infected distal radius non-union: a case report.

BACKGROUND: Infected distal radius non-union in elderly patients with severe osteoporosis is particularly challenging because infection eradication, stable fixation, and preservation of wrist function must be achieved simultaneously. Conventional internal fixation in an infected osteoporotic field carries a high risk of recurrent infection and fixation failure, whereas bridging external fixation may result in prolonged immobilization and functional deterioration. We report a case of methicillin-resistant Staphylococcus aureus (MRSA)-infected distal radius non-union treated with staged non-brid

Outcomes of Internal, External, and Hybrid Fixation in Hindfoot Charcot Neuroarthropathy: A Descriptive Systematic Review and Single-Arm Meta-analysis of Observational Studies.

BACKGROUND: Charcot neuroarthropathy (CN) of the hindfoot often requires internal fixation (IF), external fixation (EF), or hybrid constructs. This descriptive systematic review and single-arm meta-analysis summarizes outcomes of these strategies, focusing on amputation, fusion, ambulation, and complications. METHODS: Following PRISMA guidelines, 30 studies with 957 patients undergoing hindfoot or ankle reconstruction were included. A single-arm meta-analysis assessed amputation and fusion rates; qualitative analysis examined ambulation, infection, ulceration, hardware failure, and revision.

Case Report: Effect of transcutaneous auricular vagus nerve stimulation on acute pain after external fixation for bilateral open tibiofibular fractures.

A 50-year-old female patient sustained bilateral tibiofibular fractures due to a traffic accident and underwent open external fixation surgery on both lower limbs. Postoperatively, she reported severe pain (VAS ≥ 8). The patient had no significant underlying medical conditions or contraindications to vagal nerve stimulation. Transcutaneous auricular vagus nerve stimulation (taVNS)-a non-invasive modality that activates vagal pathways to modulate sympathetic-parasympathetic balance, suppress inflammation, and inhibit pain signaling-was trialed as an adjunctive analgesic intervention. Provisional Reduction Plating Versus External Fixation in the Staged Management of Gustilo-Anderson Type II-III Open Tibial Shaft Fractures.

Background/Objectives: Open tibial shaft fractures are severe injuries associated with high risks of infection and malunion. Although external fixation is commonly used for provisional stabilization, it provides limited control over alignment and soft tissue handling. This study aimed to evaluate and compare provisional reduction plating with external fixation for the staged management of Gustilo-Anderson type II-III open tibial fractures. Methods: Fifty-nine patients (mean age, 38.5 years) treated with staged debridement and delayed intramedullary nailing (IMN) were retrospectively reviewed.

Temporary bridging trans-hip external fixation in damage control orthopaedics treatment after severe combat trauma: A clinical case series.

The role of external fixation in Damage Control Orthopaedics has been well described. Temporary external fixation has been recommended to provide relative bone stability while the soft tissue heals, prior to formal open reduction and internal fixation. Temporary bridging external fixation, that spans the joint, is recommended as primary skeletal stabilization in complex intra-articular and peri-articular fractures, in extensive peri-articular soft-tissue damage around the knee, ankle, elbow and wrist joints. Works devoted to temporary trans-hip external fixation in treatment of complex high-en

3D-printed external fixation guide combined with video-assisted thoracoscopic surgery for the treatment of flail chest: a technical report and case series.

BACKGROUND: Flail chest is a common and serious traumatic condition in thoracic surgery. The treatment of flail chest often includes open reduction and internal fixation, which is relatively traumatic, complicated, and expensive. As three-dimensional (3D) printing technology is widely used in the clinical field, the application of 3D-printed products to chest trauma will become a new treatment option. To date, the use of 3D-printed external fixation guides for flail chests has not been reported. Thus, we aimed to assess the short-term efficacy of a new technology that treated flail chests with

Rate of progressive healing with a carbon-fiber orthosis in cases of partial union and nonunion after ankle arthrodesis using the Ilizarov external fixator.

BACKGROUND: The Ilizarov fixator is a popular device for treating arthrodesis of the ankle joint in complex situations. However, the therapy can fail, with nonunion or partial union that might not be load stable. There is the possibility of follow-up surgery or extended wearing of the fixator. Full weight bearing with a carbon orthosis remains another treatment option, which has not yet been investigated. The aim of the study was to determine the rate of progress that can be obtained with a carbon orthosis in cases of partial union or nonunion after fixator removal. METHODS: In this retrospect

Combined use of minimal screw synthesis and external articulated fixation is effective for the management of supra-intercondyloid humeral fractures in the elderly? A retrospective study.

BACKGROUND: Fractures of the distal humerus are relatively rare and can be a source of disabling outcomes especially if not properly treated. Therefore, the objective of the treatment must be to obtain a stable synthesis that allows early mobilization, avoiding complications such as muscular hypotonotrophy, joint rigidity or delays in consolidation that may be due to prolonged immobilization of this joint. Although ORIF treatment of these fractures may intuitively appear to be the gold standard, there is still no consensus in the literature on which type of treatment is most suitable. PATIENT
