Otological manifestations in head injury and their management - a case report.

32 year old male presented with right facial palsy following head injury. Clinical evaluation showed hemotympanum on right with moderately severe conductive hearing loss, grade III LMN facial palsy and BPPV. HRCT of temporal bone demonstrated fracture base of skull involving the roof of mastoid with no evidence of bony injury to fallopian canal. Conservative management of head trauma resulted in satisfactory outcome except for conductive hearing loss and faciel palsy. Facial palsy was managed conservatively [physiotherapy and oral steroids] with full recovery of function after 4 weeks. Conduct

Muscular subunits transplantation for facial reanimation.

PURPOSE: To present an alternative technique for reconstruction of musculocutaneous damages in the face transferring innervated subsegments (subunits) of the latissimus dorsi flap for replacement of various facial mimetic muscles. METHODS: One clinical case of trauma with skin and mimetic muscles damage is described as an example of the technique. The treatment was performed with microsurgical transfer of latissimus dorsi muscle subunits. Each subunit present shape and dimensions of the respective mimetic muscles replaced. The origin, insertions and force vectors for the mimic muscle lost wer

Laser decompression of the facial nerve.

Laser decompression of the facial nerve is an experimental approach to evaluating the parameters for laser usage at a nerve-bone interface. This article summarizes the anatomic considerations and indications for facial nerve decompression, and the advantages and disadvantages of laser decompression of the facial nerve. Preliminary studies using the Vanderbilt free electron laser are described, and future directions for research are outlined.

Restoration of selective facial nerve function by the nerve-muscle pedicle technique.

The nerve-muscle pedicle technique for selective reinnervation of paralyzed facial muscles, when combined with a modified muscle-fascia sling and neurotization from the temporal muscle, has been applied in 26 patients to date. The success rate has been 88 per cent for restoration of tonus and 77 per cent for restoration of voluntary motion as well. This procedure offers several significant advantages over other accepted means of reinnervation in patients in whom the seventh nerve itself is not available as a source for restoration of function. It also may have value as a method for rapid resto

A review of paediatric dog bite injuries presenting to plastic surgery services.

BACKGROUND: Dog bites represent a significant number of presentations to plastic surgery services in Ireland. Recent changes to legislation around ownership of restricted breeds of dog has highlighted safety in the public domain. The aim of this study was to provide a descriptive analysis of paediatric dog bite injuries. METHODS: This was a retrospective case series of all dog bite injuries referred to the plastic surgery service in Children's Hospital Ireland at Crumlin over a 3-year period (2020-2023). Patient demographics, group or breed of dog, details of injuries and interventions requir

[Injuries caused by automobile safety glass].

The report initially discuss the properties and application of tempered glass (TG) and laminated glass (LG) in motor vehicles. Although the trunk and extremities are injured these injuries are seldom life threatening. In contrast, head or neck injuries present a greater threat to life. Especially serious are TG injuries resulting in blindness. The resultant vehicle damage can appear not only as isolated glass cracking, which occurs regularly accompanying injuries without loss of consciousness, but as deformation of the vehicle interior adjacent to the windscreen. In the cases where more solid

A review of research progress of mirror therapy in peripheral nerve injury.

BACKGROUND: Mirror therapy, as a novel rehabilitation approach, has been widely applied in the treatment of various diseases, including post-stroke limb dysfunction, unilateral neglect, peripheral facial paralysis, complex regional pain syndrome, and childhood cerebral palsy. PURPOSE: This study aims to investigate the effectiveness of mirror therapy in treating peripheral nerve injuries, expand the scope of diseases to which mirror therapy can be applied, and provide a novel treatment strategy for peripheral nerve injuries. METHODS: This study conducted a comprehensive search and analysis o

Selective postmortem arterial angiography of head and neck using digital x-ray or computed tomography: A practical and rapid method to identify vascular injuries.

Identification of vascular injuries is crucial for complete postmortem evaluation and understanding of trauma deaths by the Medical Examiner. Some vascular injuries are difficult to evaluate due to challenging anatomic locations, especially in the head and neck. Documenting injuries of the facial and vertebral arteries is challenging and necessitates time-consuming dissections that can create artifacts and disfigurement. In busy medical examiner offices with a significant number of traumatic injuries, finding a creative solution to employ reliable postmortem angiography is desirable. At the Of

[Gunshot injuries in peacetime and their therapy].

Between 1976 and 1982 46 patients with gunshot injuries (43 m., 3 f.) were seen at the University of Heidelberg Department of Surgery. Frequent causes of injury in 42 adults (40 m., 2 f.) and 4 children (3 m., 1 f.) were suicide attempts (19) and carelessness in the handling of shotguns (11). Criminal offence was related to nine injuries; no retrospective classification of the event causing the injury was possible in further seven cases. Affected in descending order of frequency were brain and lung (10 each), soft tissue of thigh (9), intraabdominal organs (5), heart (4), liver and bone (3 eac

METHODS: we conducted a cross-sectional, retrospective epidemiological study of 134 trauma victims with 153 facial fractures. RESULTS: the male gender was the most affected (86.6%) and the incidence was higher in the age group from 21 to 30 years. The main etiology was assault (38.8%), followed by motor vehicle accidents (14.2%), motorcycle accidents (13.4%)

Endoscopic approaches to subcondylar fractures of the mandible.

The endoscope-assisted technique for the repair of subcondylar fractures of the mandible has been in use for approximately a decade, and its acceptance continues to grow as more surgeons gain experience. It provides for transoral access to the condylar neck region of the mandible, allowing for repair of fractures in this region with minimal if any facial scarring, while also minimizing the risk of facial nerve injury. The authors describe their technique for performing the transoral, endoscopic-assisted approach for repair of subcondylar fractures and report their experience with 48 endoscopic

Lower branches of the facial nerve.

Various aesthetic procedures have been developed over the past several decades for facial rejuvenation. One of the parallel goals is protection of vital structures such as the facial nerve. In this study, 10 cadaveric hemifacial dissections were performed, and the course of the lower portion of the facial nerve was studied with specific attention addressed toward the three-dimensional location of the nerve as it relates to the usual superficial tissue planes that are dissected during the course of an open facial rejuvenation procedure. The locations of the nerve as it runs within the different

Surgical correction of patients with asymmetries.

Facial asymmetry is quite common and, when sufficiently severe, can warrant surgical orthodontic intervention. The causes of facial asymmetry are numerous and can be generally classified as congenital, developmental, or subsequent to pathology or injury. A systematic and comprehensive examination, diagnosis, and treatment plan are requirements for successful correction of facial asymmetry. Several of the more common facial asymmetries presenting for treatment to the orthodontist and surgeon are discussed in this article. Some fundamental surgical and orthodontic principles for treating these d

Hemihypoglossal-Facial Nerve Anastomosis for Facial Nerve Reanimation: Case Series and Technical Note.

BACKGROUND: Hypoglossal nerve injury may result in swallowing and speech problems. To reduce this morbidity and allow the performance of the hypoglossal-facial nerve anastomosis bilaterally, a technique that includes partial splitting of the hypoglossal nerve and skeletonization of the facial nerve within the mastoid process has been applied. The aim of this study is to present clinical results regarding the facial and hypoglossal nerves after the procedure. METHODS: Prospectively collected data from 56 consecutive patients who underwent hemihypoglossal-facial nerve anastomosis (HHFA) were an
