The prevalence and impact of early childhood trauma in Chronic Fatigue Syndrome.

BACKGROUND: Although some studies have found high rates of early childhood trauma in Chronic Fatigue Syndrome (CFS), the role of early trauma in this condition remains controversial. METHODS: This study examined the prevalence of early childhood trauma and its impact on daily fatigue and pain levels over a 14-day period in a sample of 90 carefully screened CFS patients using a diary method approach. Data were analyzed using multilevel analysis. RESULTS: More than half of the patients (54.4%) had experienced at least one type of early trauma, with the majority of these patients reporting mult…

Clinical determinants of primary and secondary fatigue in patients with Parkinson's disease.

Clinical and psychosocial factors associated separately with primary and secondary fatigue in Parkinson's disease (PD) patients have not been thoroughly studied before. The aim of our study was to assess factors associated with different fatigue domains in groups with primary and secondary fatigue in PD separately. We divided 165 non-demented PD patients according to the absence/presence of depression, anxiety and excessive somnolence into groups with primary fatigue (N = 63) and with secondary fatigue (N = 102). Fatigue domains examined using the multidimensional fatigue inventory were associ…

Fatigue in patients with epilepsy and its association with depression and sleep quality.

OBJECTIVE: It was to evaluate the relationships between fatigue and excessive daytime sleepiness, sleep quality, depression and anxiety. METHODS: This was a cross-sectional study on 98 unselected adult patients with epilepsy (PWE) at a tertiary center. It used clinical-sociodemographic characteristics, fatigue measured by the SF-36 vitality subscale (VsSF-36) and the Epworth Sleepiness Scale, Pittsburgh Sleep Quality Index and Beck Depression and Anxiety Inventories. RESULTS: We observed that our patients presented vitality or fatigue similar to drivers with chronic headaches evaluated in th…

Aggravation of fatigue by steroid therapy in terminally ill patients with cancer.

Steroids are commonly used for fatigue relief in terminally ill cancer patients. However, steroid-induced adverse effects including depression, myopathy, and hyperglycemia may contribute to fatigue. We report our experiences with aggravation of fatigue with steroid use in three cases. Case 1 was a 65-year-old man with advanced gastric cancer. He was started on betamethasone (2 mg/d) for fatigue, but the fatigue worsened due to steroid-induced depression. Discontinuation of steroids and initiation of an antidepressant ameliorated the fatigue. Case 2 was a 68-year-old man with advanced lung canc…

Fatigue in patients with lung cancer is related with accelerated tryptophan breakdown.

BACKGROUND: Patients with cancer often suffer from fatigue and decreased quality of life which might be related to the breakdown of essential amino acid tryptophan. METHODS: In 50 patients with lung cancer we examined fatigue and the deterioration of quality of life in patients using the Functional Assessment of Cancer Therapy Anemia (FACT-An) and -Fatigue (FACT-F) subscales of FACT-General and the Mental adjustment to Cancer (MAC) questionnaires. Results were compared with tryptophan breakdown as well as serum concentrations of immune activation markers. RESULTS: Scores of psychological tes…

The relation of falls to fatigue, depression and daytime sleepiness in Parkinson's disease.

OBJECTIVE: To characterize non-motor symptoms in individuals with Parkinson's disease (PD) who experience falls compared to those who do not fall. METHODS: Fifty-four individuals with PD were studied. Thirty-six were fallers and 18 were non-fallers. Fatigue was assessed by the Iowa Fatigue Scale. Excessive daytime sleepiness was assessed by the Epworth Sleepiness Scale, and depressive symptomatology was assessed by the short-form Center for Epidemiologic Studies Depression Scale. RESULTS: Compared to non-fallers, fallers had more severe disability, greater general physical fatigue (p = 0.024…

Peripheral and central mechanisms of fatigue in inflammatory and noninflammatory rheumatic diseases.

Fatigue is a common symptom in a large number of medical and psychological disorders, including many rheumatologic illnesses. A frequent question for health care providers is related to whether reported fatigue is "in the mind" or "in the body"-that is, central or peripheral. If fatigue occurs at rest without any exertion, this suggests psychological or central origins. If patients relate their fatigue mostly to physical activities, including exercise, their symptoms can be considered peripheral. However, most syndromes of fatigue seem to depend on both peripheral and central mechanisms. Somet…

Research needs and opportunities for reducing the adverse safety consequences of fatigue.

Although there has been a significant amount of research on fatigue globally, it remains a major contributor to workplace and highway mortality and morbidity. Given its importance, a Hopkinton Conference was organized to review and discuss the state of knowledge in the area and to define future directions for research aimed at preventing or mitigating the consequences of fatigue. In all, five groups of international contributors produced six articles for this special issue, comprising state of the art reviews, along with a discussion of knowledge gaps and future research needs. In this conclud…

The effects of muscle fatigue and movement height on movement stability and variability.

Performing repetitive manual tasks can lead to muscle fatigue, which may induce changes in motor coordination, movement stability, and kinematic variability. In particular, movements performed at or above shoulder height have been associated with increased shoulder injury risk. The purpose of this study was to determine the effects of repetitive motion-induced muscle fatigue on posture and on the variability and stability of upper extremity movements. Ten healthy subjects performed a repetitive task similar to sawing continuously until volitional exhaustion. This task was synchronized with a m…

On doing nothing: descriptions of sleep, fatigue, and motivation in encephalitis lethargica.

Epidemics of encephalitis lethargica (EL), from 1917 to the 1930s, are an important milestone in the history of movement disorders. Today, t … s retrospectively. Descriptions of lethargy are explored as a way of elucidating concepts of sleep, fatigue, and motivation during the study period. The literature described many patients who had (1) lethargy without interruption in consciousness; (2) slowness of movement and catalepsy without other prominent parkinsonian features; and (3) apathy and lack …

Determinants of excessive daytime sleepiness and fatigue in adults with heart failure.

Little is known about excessive daytime sleepiness (EDS) in heart failure (HF). The aim of this cross-sectional descriptive study was to describe the prevalence of EDS and factors associated with it in HF. A secondary purpose was to explore the correlates of fatigue. We enrolled a consecutive sample of 280 adults with a confirmed diagnosis of chronic HF from three outpatient settings in the northeastern United States. Patients with major depressive illness were excluded. Clinical, sociodemographic, behavioral, and perceptual factors were explored as possible correlates of EDS. Using an Epworth…

Sleep disorders and fatigue in multiple sclerosis: evidence for association and interaction.

Fatigue is highly prevalent in multiple sclerosis (MS). It appears to be multifactorial, with "primary" or disease-related factors involved, as well as "secondary" factors, including comorbidities. Sleep disturbances are frequent in MS as well, and often result from disease-related factors. Subjective sleep disturbances in MS have been extensively studied and have been associated with fatigue. Sleep disorders in the general population have been associated with fatigue as well. However, data on objectively diagnosed sleep disorders in MS are less conclusive. Studies of sleep in MS have often su…

Fatigued breast cancer survivors and gene polymorphisms in the inflammatory pathway.

Chronic fatigue (CF) in breast cancer survivors (BCSs) has been associated with increased serum C-reactive protein-levels (CRP), pro-inflammatory cytokines and cytokine gene single nucleotide polymorphisms (SNPs). Still, there are few studies on these topics, and due to small study-cohorts the possibility to adjust for other conditions related to inflammatory processes, e.g. depression, has been limited. In 302 BCSs, examined approximately four years after treatment for breast cancer stage II/III, data on high sensitivity (hs)CRP, leukocytes and mRNA interleukin (IL)1β and IL6R expression, dep…

Effect of acute hypoxia on respiratory muscle fatigue in healthy humans.

BACKGROUND: Greater diaphragm fatigue has been reported after hypoxic versus normoxic exercise, but whether this is due to increased ventilation and therefore work of breathing or reduced blood oxygenation per se remains unclear. Hence, we assessed the effect of different blood oxygenation level on isolated hyperpnoea-induced inspiratory and expiratory muscle fatigue. METHODS: Twelve healthy males performed three 15-min isocapnic hyperpnoea tests (85% of maximum voluntary ventilation with controlled breathing pattern) in normoxic, hypoxic (SpO2 = 80%) and hyperoxic (FiO2 = 0.60) conditions, i…

Impaired sleep in chronic fatigue syndrome: how is it best measured?

The goal was to examine comparative efficacy of polysomnography, actigraphy, and self-report in evaluating the sleep/wake experience of individuals with chronic fatigue syndrome (CFS). Sleep parameters were evaluated by the three measurement modalities for the same night in 49 participants with CFS. Psychological and daytime functioning were measured by self-report. Results indicate that: (a) objectively measured nocturnal sleep time effectively approximated subjective experience although nocturnal wakefulness did not; (b) total sleep time and sleep efficiency differentiated individuals with a…

Heat acclimation does not reduce the impact of hyperthermia on central fatigue.

We tested the hypothesis that improved physiological symptoms of passively induced heat acclimation (HA) might impair the impact of hyperthermia on central fatigue and consequently cause greater peripheral fatigue. Subjects in the experimental group (7 males and 6 females) completed seven sessions of HA involving passive heating of the lower body by immersion up to the waist in a water bath at approximately 44 degrees C (air T 23 degrees C, rh 40%) for 45 min, repeated every other day for 2 weeks. During the first and the last HA sessions, participants performed a 2-min MVC of the knee extenso…

Fatigue fracture of the femoral component in a mobile bearing knee prosthesis.

Stress fracture of the femoral component is a rare complication following total knee arthroplasty. We report one such complication which occurred 6.5 years after primary implantation of an uncemented meniscal Low Contact Stress (LCS) prosthesis. The fracture affected the medial condyle of the femoral component, and a large osteolytic lesion was present in the underlying femoral condyle. The patient underwent revision arthroplasty with a cemented constrained condylar knee, which is still in place with a satisfactory result 12 years later. This complication appears to be very rare, although a nu
