Endocrine and metabolic programming during intrauterine development.

In humans, low birth weight is associated with an increased risk of metabolic dysfunction in adult life. Many of these metabolic disorders have an endocrine origin and are accompanied by abnormal hormone concentrations. This has led to the hypothesis that adult metabolic disease arises in utero as a result of programming of key endocrine systems during suboptimal intrauterine conditions associated with fetal growth retardation. This review examines the experimental evidence for prenatal endocrine programming with particular emphasis on endocrine axes involved in growth and metabolism, namely,

The role of the placenta in iron transfer from mother to fetus and the relationship between iron status and fetal outcome.

During pregnancy, iron is transferred from the mother to the fetus across the placenta. The mechanism has been extensively studied. Altered iron metabolism changes transfer, but also has other consequences. In this review, we examine how the placenta adapts to altered iron supply, both in terms of changing cytokine expression and in relation to the proteins of iron transfer. Changing iron levels alters the levels of other metals, especially copper, and we review how this is related to changing function. There are also consequences to the placenta itself, to vascularisation and other aspects of

Sonographic considerations with multiple gestation.

Determination of chorionicity is of paramount importance in risk assessment and management. Best performed in the first trimester, dichorionic placentation can be reliably assumed when the membrane is easily seen, there is a "twin peak" sign, there are clearly separate placentas, and there is discordant fetal gender. In a monochorionic twin pregnancy, there is a single placental mass, the dividing membrane is difficult to visualize until the end of the first trimester, and the membrane inserts onto the placental surface without a peaked appearance. Amniotic fluid volume assessment is important

In vitro effect of S-adenosyl methionine on ethanol embryopathy in the rat.

S-adenosyl methionine (SAM) is a universal methyl donor for biological systems. Chronic consumption of ethanol results in depletion of available SAM and reduces its biosynthesis in the transmethylation pathway. Administration of excess SAM may reduce the embryopathic effects of ethanol. The in-vitro effects of SAM on ethanol embryopathy was investigated by culturing 9.5 day old whole rat embryos for 48 hours in ethanol alone (Group II), 0.05 mM SAM (Group III), ethanol + 0.05 mM SAM (Group IV), ethanol + 0.1 mM SAM (Group V), ethanol + 1 mM SAM (Group VI), and in ethanol + 3 mM SAM (Group VII)

[Estimation of passive smoking during pregnancy by cotinine measurement and its effect on fetal growth].

Maternal cigarette smoking has been associated with some complications of pregnancy, including low birth weight and increased morbidity. Recently, it has been reported that maternal passive smoking also affects the fetal environment and causes fetal growth disturbance. In this study, we aimed to investigate the effects of maternal passive smoking on pregnant women and their fetuses by measuring cotinine concentrations in maternal urine and umbilical cord blood. The results were as follows: 1) Among 259 pregnant women, 17 cases (6.6%) were active smokers. The women who were not aware of passive

Developmental disturbances of the fetal brain in guinea-pigs caused by methylmercury.

Pregnant guinea-pigs of Hartley strain were orally administered methylmercuric chloride once at a dose of 7.5 mg Hg/animal (weighing 500-800 g) on one of days 21, 28, 35, 42 or 49 (3-7 weeks) of gestation. They were killed on day 63 (9 weeks) and their fetuses were removed. Both maternal and fetal blood, brain, liver and kidney, and fetal hair, urine, gastric content and amniotic fluid as well, were sampled for mercury analysis. The fetal brains were also examined pathologically. The maternal kidney contained mercury at a high concentration but the fetal kidney did not. The mercury concentrati

Experimental pulmonary hypoplasia and oligohydramnios: relative contributions of lung fluid and fetal breathing movements.

Inhibition of fetal breathing movements or increased loss of fetal lung fluid into the amniotic space have been suggested as two possible mechanisms causing pulmonary hypoplasia in the setting of oligohydramnios (OH). We examined the effect of OH produced by amniotic fluid shunting (AS) into the maternal abdominal cavity, ablation of fetal breathing by high cervical cord transection (CT), and CT and AS combined on fetal rabbit lungs at 24 days gestation. Lung growth at term (31 days) was measured by lung DNA content and wet lung weight. Compared to unoperated controls, newborns undergoing eith

Surgical alteration of uterine space influences embryonic loss and fetal growth in the contemporary pig.

BACKGROUND: Litter size is a key economic trait in the swine industry, and ongoing selection for this trait has increased ovulation rates without producing a corresponding increase in uterine capacity. Overcrowding, intensifies competition between fetuses, increasing within-litter variation and the occurrence of intrauterine growth restriction (IUGR). METHODS: We utilized a combination of unilateral oviductal ligation (UOL) and unilateral hysterectomy-ovariectomy (UHO), to create apposing biological extremes of intrauterine crowding. A total of twenty gilts including N = 7 UHO, N = 7 UOL and

Placental nanoparticle gene therapy normalizes gene expression changes in the fetal liver associated with fetal growth restriction in a fetal sex-specific manner.

Fetal growth restriction (FGR) is associated with increased risk of developing non-communicable diseases. We have a placenta-specific nanopa … to the sex-specific mechanistic adaptations seen in FGR fetuses and demonstrates that disruption to fetal developmental mechanisms may be returned to normal by treatment of the placenta.

Proposing a new approach to measuring birth size asymmetry.

BACKGROUND: Existing methods of measuring birth size asymmetry based on ratios of growth parameters are clinically useful but simplistic, and as such may have limited usefulness in studies of aetiology. OBJECTIVES: We aimed to develop a novel method of measuring asymmetric fetal growth at birth and demonstrate its utility in characterising the perturbed growth associated with a number of prenatal exposures and neonatal outcomes. METHODS: Data were drawn from the Queensland (QLD) Perinatal Data Collection, which included all livebirths in the Australian state of QLD between July 2010 and Dece

Anti-M induced severe haemolytic disease of foetus and newborn in a Malay woman with recurrent pregnancy loss.

Haemolytic disease of the foetus and newborn (HDFN) is caused by maternal red blood cells (RBC) alloimmunisation resulted from incompatibility of maternal and foetal RBCs. However, only a few HDFN attributed to anti-M were reported, varying from asymptomatic to severe anaemia with hydrops foetalis and even intrauterine death. A case of severe HDFN due to anti-M alloantibody from an alloimmunized grandmultiparous Malay woman with recurrent pregnancy loss is reported here. The newborn was delivered with severe and prolonged anaemia which required frequent RBC transfusions, intensive phototherapy

C1q and tumor necrosis factor-related protein 3 is present in human cord blood and is associated with fetal growth.

BACKGROUND: To determine the presence of C1q and tumor necrosis factor-related protein 3 (CTRP3) in cord blood and its relationship with fetal growth among Chinese newborns. METHODS: This pilot study recruited 126 infants (small for gestational age [SGA], n=34; appropriate for gestational age [AGA], n=60; large for gestational age [LGA], n=32); cord blood CTRP3 levels were measured, and fetal growth parameters were collected. RESULTS: Median (25-75th percentile) CTRP3 levels in the SGA, AGA, and LGA groups were 297.2 (236.4-360.2), 297.5 (261.0-369.9), and 368.6 (298.5-507.1) ng/ml, respecti

A novel systematic approach to the evaluation of the fetal venous system.

Sonographic evaluation of the fetal venous system in normal and abnormal conditions has drawn increasing interest in recent years. Whereas the assessment of the fetal heart and the related arteries is standardized using well-defined planes, the fetal venous system is still lacking a systematic approach. In this article we present a novel sonographic algorithm for a systematic examination of the fetal venous system using six planes of transverse and oblique views of the fetal abdomen and chest. These planes, using two-dimensional and color Doppler, enable a targeted demonstration of the typical

A global reference for fetal-weight and birthweight percentiles.

BACKGROUND: Definition of small for gestational age in various populations worldwide remains a challenge. References based on birthweight are deficient for preterm births, those derived from ultrasound estimates might not be applicable to all populations, and the individualised reference can be too complex to use in developing countries. Our aim was to create a generic reference for fetal weight and birthweight that overcame these deficiencies and could be readily adapted to local populations. METHODS: We used the fetal-weight reference developed by Hadlock and colleagues and the notion of pr

Maturation of pancreatic beta-cell function in the fetal horse during late gestation.

At birth, the endocrine pancreas becomes more directly involved in the control of glycaemia than in utero. However, compared with other tissues, relatively little is known about the maturational changes that occur in the fetal endocrine pancreas in preparation for extrauterine life. This study examined the pancreatic beta-cell response to exogenous administration of glucose and arginine in fetal horses with respect to their gestational age and concentration of cortisol, the hormone responsible for prepartum maturation of other fetal tissues. Glucose administration had no effect on fetal insuli

Maternal-placental-fetal interactions in the endocrine regulation of fetal growth: role of somatotrophic axes.

Inadequate growth in utero is associated not only with adverse fetal, perinatal, and neonatal outcomes, but also with an altered propensity for disease later in life. Conversely, fetal overgrowth is also associated with increased medical risks for both mother and fetus. The interaction between the fetal genome and the intrauterine environment determines in great part how fetal growth will progress. The placental, maternal, and fetal somatotrophic axes (growth hormone/insulin-like growth factor-1) play key roles in modulating this interaction. Experimental undernutrition in animal models has nu

Growing kidneys.

The number of kidney transplantations performed per year is restricted by the limited availability of donor organs. One possible solution to this shortage is the use of renal xenografts. However, the transplantation of xenografts is complicated by hyperacute and acute rejection. A second possible solution is to 'grow a kidney' from a transplanted renal anlage. It has been postulated that the host immune response might be attenuated after the transplantation of such an anlage (metanephros) instead of a developed kidney. Transplanted metanephroi become chimeric organs in that their blood supply

Effects of antibacterials on the unborn child: what is known and how should this influence prescribing.

Antibacterials are among the most commonly prescribed drugs worldwide. In general, infections occur in pregnant women at much the same rate as in the general population. However, as a result of physiological changes brought about by pregnancy, some infections, such as those of the urinary tract, may have an increased incidence. It is important to remember that almost every drug crosses the placenta, ensuring that the unborn fetus is also exposed. When prescribing an antibacterial agent to a pregnant woman, it is important that the mother is treated appropriately while at the same time protecti

Effect of persistent follicles on early embryonic losses in beef cows.

Low doses of progestogens in regimens for synchronization of estrus result in persistent follicles (PF) and reduced conception rates in cattle. The effects of PF on recovery of oocytes/embryos, fertilization, and embryonic development were examined in beef cows. To produce PF, cows received a used Controlled Internal Drug Releasing device on Days 4 through 13 after pretreatment estrus (Day 0) and two injections of 20 mg prostaglandin (PG) F2 alpha, i.m., 12 h apart on Day 6 (PF; n = 20). Control cows received only PGF2 alpha on Day 6 so they would ovulate growing follicles (GF; n = 22). With t
