The results of the operative treatment of 27 humeral shaft fractures treated at the University of Louisville during a 2-year period were reviewed. Twenty-one patients with severe tibial plafond fractures were treated by unilateral large screw external fixation. In 15 patients, this was combined with limited internal fixation. The GUEPAR(®) implant is a metallic bipolar radial head prosthesis designed to treat comminuted radial head fractures when anatomic realignment of the articular surface of the radiocapitellar joint is not possible. We report herein the rare case of an acute complete disassembly of this implant, discuss the reason for this occurrence and review the literature. In the presented case, the complete removal of the prosthesis provided a satisfactory outcome with an excellent Mayo Elbow Performance Score at 12months follow-up.

Devastating hand injuries often require multiple microvascular reconstructions. We report a patient in whom two flaps were used for late reconstruction of a devastating hand injury involving devascularization of the right hand, severely comminuted fractures of the hand and forearm, and multiple tendon avulsions. We believe the sequential vascular connection of free flaps offers the best method of reconstruction in this severe case, allowing composite tissue transfer, monitoring of the osseous flap, and optimal positioning of the two free tissue transfers.
