Statins and skeletal muscles toxicity: from clinical trials to everyday practice.

The mechanism(s) underlying the occurrence of statin-induced myopathy are ill defined, but the results of observational studies and clinical trials provide compelling evidence that skeletal muscle toxicity is a frequent, dose-dependent, adverse event associated with all statins. It has been suggested that reduced availability of metabolites produced by the mevalonate pathway rather than intracellular cholesterol lowering per se might be the primary trigger of toxicity, however other alternative explanations have gained credibility in recent years. Aim of this review is: (i) to describe the mol

Surgical pathology of lung cancer.

The diagnosis, treatment, and management of lung tumors represent a complex set of decision algorithms and require the cooperation and interaction of a team of experts and support systems. The surgical pathologist, an early, important member of the diagnostic team, uses clinical and radiological evidence to differentiate benign from malignant tumors and renders a unique diagnosis that provides both prognostic and treatment information. Using routine histopathologic criteria, histochemical and immunohistochemical stains, and molecular and genetic testing, surgical pathologists and cytopathologi

Biomarkers in prostate cancer diagnosis and prognosis: beyond prostate-specific antigen.

PURPOSE OF REVIEW: To review the most recent advances in genetic testing for prostate cancer risk and of new molecular diagnostic assays to improve diagnostic accuracy and treatment decision beyond prostate-specific antigen (PSA) testing. RECENT FINDINGS: Multiple independent studies had demonstrated evidence that genetic variations in three regions of chromosome 8q24 and one each at 17q12 and 17q24.3 are independent predictors of prostate cancer risk in addition to family history and serum PSA levels. The small percentage of individuals with several anomalies can have up to 10 times the risk

Pairwise coalescence times under the structured coalescent: A computational overview.

Pairwise coalescence times are of fundamental importance in population genetic theory and inference. However, computing expectations and related quantities under the structured coalescent remains challenging. Here, we analyze two commonly used approximation strategies-the time-decomposition approach and the lineage-independence assumption-and characterize their underlying theoretical assumptions and computational properties. We also summarize recent algorithmic advances that relate the problem to the generalized Lyapunov equation. These methods enable exact computation with reduced complexity,

Assessing Pulmonary Function in ALS using Electrical Impedance Tomography.

This study aimed to determine an optimal model involving thoracic electrical impedance tomography (EIT) metrics and patient geometric information to best correlate to standard pulmonary function test (PFT) measures in a cohort of 32 ALS patients and 32 age-matched healthy controls. Thoracic EIT is a non-invasive technology in which an electrode belt chest allows for real-time impedance imaging of respiratory function. The optimal form of the model was determined via a genetic algorithm a novel technique for model generation in EIT applications. Combining multiple metrics yielded optimal r2 val

Statin-Induced Myopathy in a Patient with Schwartz-Jampel Syndrome.

Schwartz Jampel syndrome (SJS) is a genetic disorder characterized by myotonia and chondrodysplasia. Mutations of the Perlecan gene (HSPG2), which encodes a key component of the extracellular matrix of muscle, bone, and cartilage is cause for the characteristic dysmorphisms of SJS. Clinically remarkable creatinine phosphokinase (CPK) levels are typical and can be associated with myotonia as an underlying cause in SJS patients. We report a unique case of a symptomatic adverse event of statin use in a SJS patient who demonstrated heightened levels of CPK to baseline following a statin induced my

Clinical Reasoning: A Young Man With Subacute Onset of Spastic Paraparesis.

Leukodystrophies are a group of rare neurodegenerative disorders, usually presenting in infancy with a variable combination of cognitive, motor, and coordination impairment. Adult-onset cases are even more rare, often representing a diagnostic challenge even for experienced neurologists. Here, we present a case of a 44-year-old man with subacute and rapidly progressive spastic paraplegia, whose brain MRI revealed white matter abnormalities compatible with a diagnosis of leukodystrophy. We discuss how to apply a simplified diagnostic algorithm to distinguish acquired leukoencephalopathies from

Diagnosis and treatment of cardiac amyloidosis. A position statement of the European Society of Cardiology Working Group on Myocardial and Pericardial Diseases.

Cardiac amyloidosis is a serious and progressive infiltrative disease that is caused by the deposition of amyloid fibrils at the cardiac level. It can be due to rare genetic variants in the hereditary forms or as a consequence of acquired conditions. Thanks to advances in imaging techniques and the possibility of achieving a non-invasive diagnosis, we now know that cardiac amyloidosis is a more frequent disease than traditionally considered. In this position paper the Working Group on Myocardial and Pericardial Disease proposes an invasive and non-invasive definition of cardiac amyloidosis, ad

An autosomal recessive syndrome of severe cognitive impairment, dysmorphic facies and skeletal abnormalities maps to the long arm of chromosome 17.

Cognitive impairment (CI) is one of the most challenging referrals to the clinical genetics service. The different algorithms proposed to assist in the molecular diagnosis of CI rest largely on the distinction between syndromic and non-syndromic forms. We have identified what appears to be a novel syndromic form of CI, the variable phenotype of which comprises severe CI, hirsutism, dysmorphic facies and skeletal abnormalities, and have mapped it to a single locus on chromosome 17q21.31-17q22 spanning 12.2 Mb. Two candidate genes, HOXB6 and PPP1R9B were sequenced but no pathogenic alterations w

Diagnosis of cerebral cryptococcoma using a computerized analysis of 1H NMR spectra in an animal model.

Viable cryptococci load in biopsy material from an animal model of cerebral cryptococcoma were correlated with 1H NMR spectra and metabolite profiles. A statistical classification strategy was applied to distinguish among high-resolution 1H NMR spectra acquired from cryptococcomas, glioblastomas, and normal brain tissue. The overall classification accuracy was 100% when a genetic-algorithm-based optimal region selection preceded the development of linear discriminant analysis-based classifiers. The method remained robust despite differences in the microbial load of the cryptococcoma group when

Microbial community differentiation and machine learning-driven optimization of nitrogen removal in multi-configured anammox systems.

Integrated fixed-film activated sludge (IFAS) reactors are widely adopted for anammox wastewater treatment, but few studies have systematically compared microbial community structure, interactions and functions among suspended sludge, pure biofilm and the two phases of IFAS. A total of 472 16S rRNA gene sequencing samples collected from 2017 to 2023 were analyzed through bioinformatic tools, diversity assessment, co-occurrence network construction and machine learning modeling. The biofilm of IFAS exhibited significantly higher alpha diversity as well as a more complex and stable microbial net

Integrating genetic and imaging information to enhance cardiovascular risk stratification: rationale and design of the CVRISK-IT randomized controlled trial.

AIMS: Cardiovascular disease (CVD) remains the leading cause of morbidity and mortality worldwide. Current guidelines recommend algorithms to estimate 10-year CVD risk in apparently healthy individuals, but their precision remains limited. Whether the integration of imaging and genetic information can improve individual risk stratification remains uncertain. METHODS AND RESULTS: The CVRISK-IT trial is a multicentre, open-label, randomized controlled study designed to enhance CVD prevention through precision-based risk assessment and targeted communication strategies. A total of 30 000 adults

A Multi-Omics and machine learning platelet-related prognostic signature in multiple myeloma.

While platelets are well-documented contributors to tumorigenesis, their role in multiple myeloma (MM) progression and risk stratification remains underexplored. To assess platelet function in MM and verify the prognostic value of platelet-related genes(PRGs) in patients with multiple myeloma, further providing new ideas for the development of MM. We combined the clinical assessment of platelet activation in MM patients with functional co-culture experiments using MM cell lines (RPMI8226, MM.1 S) to investigate platelet-driven tumor progression. Additionally, an integrated analysis of bulk (GS

Association of Polygenic Risk Score for Suicidal Behavior With Ventral Prefrontal Morphology in Bipolar Disorder.

BACKGROUND: Bipolar disorder (BD) studies consistently implicate the ventral prefrontal cortex (VPFC) in association with suicidal behaviors. A suicidal behavior polygenic risk score (PRS) can now be computed to test whether elevated genetic risk for suicidal behavior is associated with variation in VPFC gray matter and/or suicidal behavior history. METHODS: We explored relationships among a PRS previously associated with suicidal behavior, VPFC morphology, and suicidal behavior in 376 individuals of European ancestry (mean age 30y; 57% female): 225 with BD (76 suicide attempters (SAs), 149 n

Familial hypercholesterolaemia in children and adolescents: a European Atherosclerosis Society consensus statement.

Familial hypercholesterolaemia (FH) is a common genetic disorder characterized by lifelong elevated LDL cholesterol (LDL-C) concentrations. FH exists in two forms: heterozygous FH (HeFH), which affects around 1 in 300 people worldwide, and homozygous FH (HoFH), which affects around 1 in 300 000. Individuals with FH are at increased risk of premature atherosclerotic cardiovascular disease (ASCVD) and death, and those with HoFH are, if untreated, at extreme risk of ASCVD manifestations even before adulthood. Early diagnosis and treatment in childhood can extend or normalize life expectancy, but

Immune System Alterations in Treatment-Resistant Schizophrenia: A Systematic Review of the Current Evidence and Future Directions.

Treatment-resistant schizophrenia (TRS) remains a significant clinical challenge due to limited therapeutic options and a poor understanding of its underlying biology. Recent findings suggest that immune system dysregulation may play a critical role in the pathophysiology of TRS. This systematic review aimed to synthesize current evidence on immunological abnormalities associated with TRS, with a focus on inflammatory markers, immune cell profiles, and the role of autoantibodies, and to explore their potential utility in diagnosis and treatment. A systematic review of the literature was conduc

Infant Nephrogenic Diabetes Insipidus: Challenges Leading to Delayed Management.

BACKGROUND: Nephrogenic diabetes insipidus (NDI) is a rare, potentially life-threatening renal tubular disorder characterized by a decreased ability to concentrate urine due to antidiuretic hormone (ADH) resistance, leading to significant free water loss. The congenital form, most often caused by X-linked mutations in the AVPR2 gene, typically presents in early infancy. Due to its rarity and nonspecific clinical features in young children, such as failure to thrive, vomiting, and excessive fluid intake, diagnostic delays are common, especially in centers with limited prior experience in managi
