INTRODUCTION: This study aimed to compare the efficacy of trabeculectomy (TE) in different glaucoma types.
METHODS: A single-center retrospective analysis was conducted on eyes undergoing TE for primary open-angle glaucoma (POAG), pseudoexfoliation glaucoma (PEX), or angle-closure glaucoma (ACG) with a postoperative follow-up of 5 years. Parameters including intraocular pressure (IOP), number of IOP-lowering medications, mean defect (MD) of standard automated perimetry, mean peripapillary retinal nerve fiber layer (RNFL) thickness, and surgical success rates were analyzed and compared.

BACKGROUND AND OBJECTIVES: Glaucoma is one of the leading causes of irreversible blindness worldwide and is increasingly recognized as a potential adverse effect of various pharmacological agents. It has been suggested that psychotropic medications influence glaucoma risk, but findings across studies have remained inconsistent. We aimed to clarify the association between psychotropic drug use and glaucoma through a Bayesian meta-analysis.
METHODS: We conducted a systematic literature search up to December 2024. Studies that examined the relationship between psychotropic medications and glauco

This is a study of 59 patients, 42 with glaucoma and 17 with neurological disease, to compare the Octopus automated perimeter with the conventional Goldmann and Tubinger perimeters for detection and assessment of visual field defects. We believe the Octopus automated perimeter is an excellent method for screening patients suspected of glaucoma and can be expected to identify over 90% of the patients actually having visual field loss due to glaucoma. In patients with neurological disease, the Goldmann perimeter was superior to either the Octopus or Tubinger perimeters in providing clinically us

Glaucomatous changes have been known to occur in the inner retinal layer all over the fundus and in the deep retinal layer and retinal pigment epithelium close at the optic disc border. This study was conducted to address the question as to whether the middle retinal layers are altered by the glaucomatous process. The study included histological slides of 23 eyes enucleated due to painful secondary traumatic angle-closure glaucoma and 14 nonglaucomatous eyes removed because of a malignant choroidal melanoma. We evaluated the cell count and thickness of the inner nuclear layer via histomorphome

Central anterior chamber depth (cACD), defined as the shortest distance between the posterior corneal surface and the anterior lens surface, is a key anatomical parameter for assessing the risk of primary angle-closure glaucoma (PACG). Traditional views often regard a shallow anterior chamber as a static anatomical consequence of a short axial length or a thickened lens; however, this perspective fails to fully explain the physiological fluctuations of cACD or the variability in angle-closure progression among individuals. Integrating recent high-resolution in vivo imaging and fluid dynamic ev

BACKGROUND: Klinefelter syndrome (47,XXY) commonly associated with hypogonadism, infertility, and neurocognitive deficits, has rarely reported ocular anomalies. This case highlights a novel ocular presentation of Klinefelter syndrome, emphasizing the role of ophthalmic findings in early diagnosis of genetic syndromes. CASE PRESENTATION: We describe a case of a 14-year-old male presented with acute angle-closure glaucoma secondary to spherophakia, accompanied by vitreoretinal abnormalities. Chromosomal analysis revealed a 47,XXY karyotype, consistent with Klinefelter syndrome. He underwent trab

Nanophthalmos is characterised by shorter posterior and anterior segments of the eye, with a predisposition towards high hyperopia and primary angle-closure glaucoma. Variants in TMEM98 have been associated with autosomal dominant nanophthalmos in multiple kindreds, but definitive evidence for causation has been limited. Here we used CRISPR/Cas9 mutagenesis to recreate the human nanophthalmos-associated TMEM98 p.(Ala193Pro) variant in mice. The p.(Ala193Pro) variant was associated with ocular phenotypes in both mice and humans, with dominant inheritance in humans and recessive inheritance in m

BACKGROUND: To explore the efficacy and safety of laser peripheral iridoplasty (LPIp) with different energy levels and locations in the treatment of primary angle closure disease (PACD) assessed by swept-source anterior segment optical coherence tomography (AS-OCT).
METHODS: We enrolled patients with PACD following best-corrected visual acuity (BCVA), intraocular pressure (IOP), anterior chamber gonioscopy, ultrasound biomicroscopy(UBM), optic disc OCT, and visual field examinations. After Pentacam and AS-OCT measurements, the patients were randomly divided into four treatment groups for LPIp

PURPOSE OF REVIEW: Primary angle closure typically causes acute intraocular pressure rise in the phakic elderly. Alternative diagnoses, however, for which iridotomy is usually ineffective, occur commonly in younger, nonhyperopic, and pseudophakic patients.
RECENT FINDINGS: High-resolution ultrasonography has advanced our understanding of these entities. Management of platueau iris, present in over half of angle closures with patent iridotomy, may depend on disease stage. Early postoperative pseudophakic patients with myopic shift and narrow angle should be treated with laser capsulotomy for c

Ultrasound biomicroscopy is a new technique that uses high-frequency ultrasound (50 to 100 MHz) to produce images of the entire anterior segment at high resolution (20 to 50 microns). The iridocorneal angle, iris, ciliary body, and posterior chamber can be imaged in detail and the dimensions and anatomic relationships of these structures determined. Plateau iris syndrome is a condition in which the angle remains appositionally closed or occludable after iridectomy for angle-closure glaucoma. How the iris remains in a position that allows it to occlude the angle has been uncertain. We performed

Primary angle closure glaucoma is a leading cause of irreversible blindness, particularly in Asia. Its pathophysiology is based in the closure of the anterior chamber angle (ACA). In addition to gonioscopy (current reference standard), in the past decade, anterior segment optical coherence tomography (AS-OCT) has been incorporated in routine ophthalmic practice to help assess the configuration of the ACA. Especially in nonspecialist ophthalmology practice, gonioscopy may be less frequently performed and AS-OCT may not be available, leading to the need of other anterior segment evaluation metho

INTRODUCTION: Pupillary block, a concerning complication of cataract surgery, is heightened when a single-piece acrylic (SPA) intraocular lens (IOL) is implanted in the ciliary sulcus. We report an unusual occurrence of relative pupillary block and chronic angle-closure glaucoma (ACG) identified in the late postoperative period due to unintentional SPA IOL implantation in the sulcus.
CASE PRESENTATION: An 82-year-old woman presented with a history of chronic ACG 5 years after bilateral cataract extraction. Postoperatively, she experienced anterior chamber shallowing, elevated intraocular pres

The therapeutic effects of carteolol hydrochloride were evaluated in the treatment of 44 uveitis patients (51 eyes) with intraocular pressure elevation or secondary glaucoma. Carteolol ophthalmic solutions, either 1% or 2%, were given twice day for more than 4 weeks in glaucomatocyclitic crisis and for more than 8 weeks in other forms of uveitis. Intraocular pressure significantly decreased from week 1 of treatment and persisted within normal limits until week 8 in glaucomatocyclitic crisis. Similarly, intraocular pressure decreased significantly from week 2 in other forms of uveitis. Intraocu

We prospectively surveyed the incidence of retinal vein occlusion (RVO) at the glaucoma clinic of Hiroshima University between 1986 and 1991. Among 433 glaucoma patients, 18 (4.2%) subsequently presented with RVO, 9 had central retinal vein occlusion (CRVO) and 9 exhibited branch retinal vein occlusion (BRVO). Seven of 87 (8.1%) patients with primary angle closure glaucoma exhibited RVO, showing the highest incidence among glaucoma types. The incidence of RVO detected by the general outpatient clinic was 0.59% during the same period. The BRVO/CRVO ratio in the glaucoma clinic was 1.0, while it

BACKGROUND: Secondary angle-closure glaucoma associated with iridocorneal endothelial (ICE) syndrome can be challenging to manage. This report presents a novel case of sulcus placement of a PreserFlo Microshunt in a patient with ICE syndrome following the failure of previous trabeculectomy and medical therapy.
CASE PRESENTATION: A 61-year-old patient with secondary angle-closure glaucoma due to ICE syndrome had uncontrolled intraocular pressures despite maximal medical therapy. They had a previous trabeculectomy, which failed following cataract surgery. Because of extensive peripheral anterio

Previous population studies have demonstrated a shallow anterior chamber as an inherited characteristic of Eskimos, explaining their high prevalence of primary angle-closure glaucoma. In order to evaluate the stability of the anterior chamber depth (ACD) distribution and its inherent morbidity risk in a different environment, immigrant Greenland Eskimo women in Copenhagen, aged 40 years or more were sampled. Those who had been at least 25 years in Denmark were examined (N = 64), using Haag Streit pachymetry, limbal chamber depth (LCD) estimation and gonioscopy. The ACD level of the immigrants
