Polylactic acid bioabsorbable implants of the hand: A review.

In the treatment of hand fractures, metal implants are often used to allow early range of motion and a stable fixation. Although they provide adequate bone union, metal implants have been known to cause joint stiffness, painful or prominent hardware, and adhesions, often leading to another surgery for hardware removal. Bioabsorbable implants have been shown to offer comparable results for fracture fixation in the hand while removing the complications seen with retained hardware. In this article, we review biomechanical and clinical studies focused on bioabsorbable implants made of polylactic a

Osteomyelitis variolosa: a case report.

Osteomyelitis variolosa is an infection of bone and joints by smallpox virus variola major, most commonly in the elbows, wrists, ankles, hands, and feet. We report one such case in a 70-year-old woman who presented with deformities of the right knee, both elbows and ankles, and the left hand, and a history of childhood fever with rashes. Her lateral femoral condyle of the right knee was hypoplastic with patella baja. Her right elbow was ankylosed and her left elbow was dislocated with multidirectional instability. Her third and fourth metacarpals on the left hand were shortened. Both ankles we

External fixation of complex hand and wrist fractures.

From 1977 to 1980 fixation with an external fixateur was performed in 33 injuries of the upper extremity: ten fractures of the hand, 22 Colles' fractures, and one osteotomy for Madelung's deformity. Application was primary in 19 patients and after another type of fixation (cast or Kirschner wire) had failed to hold reduction in 14 patients. All ten hand fractures healed in 4 to 12 weeks, and the distal radius fractures united in 6 to 10 weeks. The external fixateur for unstable fractures of the hand and wrist with bone or soft-tissue loss provides rigid skeletal stabilization, accessibility fo

Detection of degenerative and inflammatory osseous changes on an magnetic resonance imaging (MRI) of the hands and wrists: T2-weighted Dixon imaging can replace pre-contrast T1-weighted images.

AIM: To evaluate whether pre-contrast T1-weighted magnetic resonance imaging (MRI) sequences can be omitted when using T2-weighted Dixon imaging to detect inflammatory and degenerative osseous changes in the hands and wrists. MATERIALS AND METHODS: Two radiologists retrospectively reviewed bilateral hand and wrist MRI examinations from 48 patients with hand pain. A standard protocol (coronal and axial T1-weighted images) was compared with a simplified protocol including coronal and axial fat-only, in-phase, and water-only T2-weighted Dixon images. Readers independently scored osteophytes, joi

[Does additive spongiosaplasty improve outcome after surgical therapy for solitary enchondroma in the hand?].

INTRODUCTION: The necessity of spongiosaplasty in the treatment of solitary enchondroma in the hand has been a subject of controversial discussions for several years. Over a period of 10 years the authors performed single curettage without spongiosaplasty. The aim of this study was to investigate our results and to compare these findings with those of other studies. PATIENTS AND METHOD: Over the last 10 years we have treated 106 patients with solitary enchondroma of the hand by single curettage without bone grafting. All patients underwent postoperative radiological examination. The mean foll

[Comparison between direct radiographic enlargement and mammography technique in detection of osseous changes of the hand skeleton in hyperparathyroidism].

PURPOSE: To compare direct magnification radiography and mammographic film-screen systems in osseous changes of the hand in hyperparathyroidism. MATERIALS AND METHODS: The hands of 60 patients who were suspected to have primary (5), secondary or tertiary (both 55) hyperparathyroidism were evaluated with direct magnification radiography and a mammographic film-screen system (S12.5). Posterior-anterior views of each hand were obtained with both techniques. The magnification radiographs were made with a focal spot size of 40 or 60 microns, digital luminescence radiography (S600/1200), and a x 6

Heterotopic ossification in the hand of a patient with spinal cord injury.

Heterotopic ossification (HO), reported to occur in 20% to 40% of spinal cord injured patients, has been described in the hips, knees, shoulders and elbows, but not in the hands or around the phalangeal joints. This report is believed to be the first of HO of the hand after spinal cord injury. Nine weeks after injury, a diving accident, a 27-year-old man having C6 quadriplegia, motor complete, sensory incomplete, developed acute swelling and loss of range of motion in both hands. Serum alkaline phosphatase showed no significant rise. Bone scan demonstrated increased uptake in many joints of th

Diagnostic Value of Mid-Thigh and Mid-Calf Bone, Muscle, and Fat Mass in Osteosarcopenia: A Pilot Study.

Osteosarcopenia is defined as the concomitant occurrence of osteoporosis and sarcopenia. Current lack of consensus on sarcopenia definitions, combined with the low sensitivity and specificity of screening methodologies, has resulted in varying prevalences of sarcopenia, and consequently osteosarcopenia diagnosis. Previous research indicates that mid-thigh is a potential surrogate region for the assessment of bone, muscle, and fat mass in a single, efficient and low-radiation dual x-ray absorptiometry (DXA) scan. We hypothesized that muscle and bone mass measurements in the mid-thigh region can

Six years of follow-up after bilateral hand replantation.

Replantation is the gold standard surgical treatment of amputations of the upper limb; however, this demanding procedure is not always preformed in bilateral limb amputation. The objective of this study was to analyze, six years after surgery, the sensorimotor recovery of both replanted hands. A 21-year-old patient with bilateral hand amputation was benefited from limb replantation. Surgery included debridement of the amputated hands and recipient's stumps, bone fixation, arterial and venous anastomoses, nerve sutures, tendon sutures and skin closure. Rehabilitation program included physiother

Morphometric indices and carpal tunnel syndrome: a radiological study in female patients.

INTRODUCTION: Carpal tunnel syndrome (CTS) is the most common entrapment neuropathy syndrome of the upper extremities. The carpal tunnel is an osteofibrous canal that is medially bordered by hamulus ossis hamati and pisiform bone, and laterally by scaphoid and trapezoid bones. In this retrospective case-control study, we investigated the relationship between radiologically measured morphometric indices and CTS in female patients. METHODS: Clinical, radiological, and demographic data were collected for 55 hands of 40 female patients diagnosed with CTS and 58 hands of control subjects. Radiolog

Small carpal bone surface area, a characteristic of Turner's syndrome.

An abnormality which has received little attention but may be easily recognized on radiographs of the hand of patients with Turner's syndrome is described. Eleven of thirty-one patients (35.5%) with Turner's syndrome were shown on radiographs of the hand to have a visually detectable smallness on the bone surface area of the carpus when compared to the area of the second through fifth metacarpals. Values for the "C/M" ratio (the area of the carpals divided by the area of the second through fifth metacarpals) were calculated for films of 31 individuals with gonadal dysgenesis and compared with

[Mono-arthritis of the hand: case report of osteoid osteoma].

Osteoid osteoma is a rare occurrence at the hand. There is often a long delay before the diagnosis because the clinical signs mimic other frequent affections of the hand. We report the case of a 24-year old female patient suffering from an osteoid osteoma in the first phalanx of a finger. The diagnosis was made four years after the first symptoms. We started by a curettage-biopsy, followed in a second operative step by cauterization and filling up the defect by a bone autograft. Six months after the surgery, the patient was no longer experiencing any symptom. This rare case demonstrates that o

The effects of trabecular-bone loading variables on the surface signaling potential for bone remodeling and adaptation.

It is widely believed that mechanical forces affect trabecular bone structure and orientation. The cellular mechanisms involved in this relationship, however, are poorly understood. In earlier work we developed a theoretical, computational framework, coupling bone-cell metabolic expressions to the local mechanical effects of external bone loading. This theory is based on the assumption that osteocytes within the bone tissue control the recruitment of bone-resorbing osteoclasts and bone-forming osteoblasts, by sending strain-energy-density (SED) related signals to trabecular surfaces through th

"Distraction" of grafted alveolar bone in cleft case using endosseous implant.

OBJECTIVE: Presentation of a specific surgical technique of vertical alveolar ridge distraction performed on an alveolar bone graft 1 year after the primary grafting procedure designed to correct a labioalveolar cleft. The graft had not obtained the results desired and presented heavily scarred mucosal tissue. TECHNIQUE: A dental implant placed within the graft once it has been entirely freed by osteotomies functions as an endless screw. To avoid vascular risk, the alveolar bone graft is detached as a whole along its total height. Consequently, no bone remains above it to provide support for

Cytokine-mediated immunomodulation of osteoclastogenesis.

Cytokines are an important set of proteins regulating bone homeostasis. In inflammation induced bone resorption, cytokines, such as RANKL, TNF-α, M-CSF, are indispensable for the differentiation and activation of resorption-driving osteoclasts, the process we know as osteoclastogenesis. On the other hand, immune system produces a number of regulatory cytokines, including IL-4, IL-10 and IFNs, and limits excessive activation of osteoclastogenesis and bone loss during inflammation. These unique properties make cytokines powerful targets as rheostat to maintain bone homeostasis and for potential

Eosinophilic granuloma of bone.

Forty-three patients with histologically proven Histiocytosis X localized to bone at presentation have been reviewed. Thirty-six patients presented with solitary bone lesions and 31 have healed without complication with a mean follow-up of 10 years. The remaining five patients developed polyostotic disease, three exhibiting either pulmonary lesions or diabetes insipidus. Seven patients presented with multiple bone lesions and six of them developed other bone lesions. Four of these patients also showed some of the features of Hand-Schüller-Christian disease. An increase in size of the presentin

The association between bone mineral density, metacarpal morphometry, and upper limb fractures in children: a population-based case-control study.

The aim of this population-based case-control study was to examine the association between bone mass and upper limb fractures in children aged 9-16 yr. Areal bone mineral density and bone mineral apparent density (BMAD) were measured by both dual energy absorptiometry (DXA) and metacarpal index (MI) by hand radiograph. A total of 321 fracture cases and 321 randomly selected individually matched controls were studied. For all fractures, cases had lower DXA measures at all sites (1.1-3.3%; all P < 0.05). A larger reduction was observed for those with wrist and forearm fractures (1.2-4.5%; all P

[Evaluation of the Benefits of Administering Hand Therapy to Patients with Chemotherapy-Induced Peripheral Neuropathy].

Taxanes, which are used to treat breast cancer, damage the microtubules of normal nerve cells, causing numbness of the fingers related to chemotherapy-induced peripheral neuropathy(CIPN); therefore, effective methods for reducing numbness are needed. In 2017, it was reported that physical stimuli related to massage improved finger blood flow volumes, contributing to the regeneration of damaged nerves. We developed a method of hand therapy for breast cancer patients complaining of numbness related to anticancer drug administration, and examined its effects on numbness. Hand therapy was performe
