[Experimental hip dysplasia in the rabbit].

Neglected cases of congenital dislocation of the hip (CDH) always present with profoundly interesting X rays and pathoanatomical changes. Because of progressive dysplastic change, it is difficult to determine clinically at what stage a patient can not have reconstructive surgery. We used an animal model to induce hip dysplasia in order to understand the mechanism of dislocation and to demonstrate the dysplastic changes at various ages of dislocation. We used a plastic tube to immobilize the left knee of one week old New Zealand white rabbits in the extension position. Fifty-five rabbits in a n…

Transient synovitis of the hip joint.

A review of 286 children who presented with transient synovitis of the hip over a 6-year period shows that the condition remains a benign disease of the paediatric hip which settles quickly with simple symptomatic treatment. We have not found any obvious causal agent for the condition despite intensive laboratory investigation. There was only one subsequent case of Perthes' disease in association with transient synovitis, causing doubt on any relationship between the two conditions. The need for extensive investigation and long-term follow-up seems to be unnecessary in transient synovitis of t…

Anaesthesia for hip surgery in the elderly.

Surgical repair of hip fracture and total hip arthroplasty are primarily performed on elderly patients. Patients presenting for hip fracture surgery have a high prevalence of preoperative medical problems and may require medical stabilization before surgery. Regional anaesthesia for hip fracture repair may be contraindicated due to perioperative pharmacologic prophylaxis for deep venous thrombosis. The use of regional anaesthesia increases the magnitude and frequency of hypotensive episodes when compared with general anaesthesia. Intraoperative blood losses, averaging 250-300 ml, are not affec…

[Studies of hip dislocation in twins].

An arbitrary series of twins is used to demonstrate that there is no increased morbidity from congenital dislocated hip joint in twins. A physiologic delay in ossification of the cartilaginously preformed parts of articulations is caught up during the first year of life. In addition, connection with pregnancy and birth is discussed and r recommendations for diagnosis and therapy using sonography are given.

Osteoarthritis of the hip and Heberden's nodes.

One hundred consecutive patients with proved osteoarthritis were assessed for the presence of terminal interphalangeal joint disease. There was a significant association found between the presence of Heberden's nodes and primary (axillary) arthritis. Secondary osteoarthritis was relatively free of nodal involvement. It is suggested that Heberden's nodes are a helpful clinical marker in differentiating between the two major groups of osteoarthritis.

[Traumatic hip dislocation in the child].

We report on 20 own cases of the past 20 years. Traumatic hip dislocation in children, which is a rare event, should be primarily reduced under anaesthesia as quickly as possible. An open reduction is only necessary with the luxatio obturatoria if a sole conservative reduction attempt is unsuccessful. A possible ipsilateral femur shaft fracture must be treated operatively at the same time.

Synovial chondromatosis of the hip joint.

Synovial chondromatosis of the hip joint is uncommon entity. Two cases are presented, and the spectrum of the disease is discussed. Partial synovectomy with loose body removal is recommended as the treatment of choice. Joint replacement is performed only in patients with severe secondary osteoarthritis, and used only in selected cases.

[Tuberculous osteoarthritis of the hip in adults].

We have carried out a retrospective study of 49 cases of tuberculosis of the hip in adults, all treated by one of the authors. All patients presented with destructive lesions in the hip. Abscess formation and sinuses were infrequent. A scientific diagnosis was obtained in 46 cases. All the patients were treated with chemotherapy for 6 to 12 months. The hip was immobilised for 3 months, either by skeletal traction followed by a plaster cast, or by a cast alone, depending on the need for correction of deformity. If deformity was fixed or absent, immobilisation was not undertaken. A favourable re…

[Principles of sonographic examination of the hip].

The advantages of ultrasound in the diagnosis of congenital dislocation and dysplasia of the hip, compared with other imaging diagnostical methods are obvious, since an exact and early diagnosis of hip immaturity can be made. Using realtime sonography, it is possible to directly visualise the motion of the femoral head in the acetabulum and verify any instability of the joint. Especially hips which are classified as being in the critical range should always be examined dynamically to judge their grade of instability. This is necessary because these hips often show no pathology when examined cl…

Hip arthroscopy by the lateral approach.

This report describes a new technique by which arthroscopy of the hip provides complete visualization of the joint space. The arthroscope and operative instruments are inserted by a direct lateral approach over the greater trochanter, with the patient in the lateral decubitus position. The involved leg is held in an abducted and flexed position with traction by pulleys hung overhead. Hip abduction and flexion relaxes the capsule and traction separates the joint so that insertion of the arthroscope is facilitated. Arthroscopic examination of the hip joint was performed in 11 patients. The indic…

Late effects of JRA on the hip.

Osseous changes at the hips were seen in the radiographs of 36 children with long-standing juvenile rheumatoid arthritis (JRA) and no significant steroid therapy. Seven boys and one girl had progressive primary inflammatory destruction of the hip with erosive arthritis and progressive joint space narrowing. Twenty-one girls and seven boys revealed adaptive developmental changes in the growing skeleton without erosion. Both courses resulted in secondary osteoarthritis and functional disability. Early recognition of adaptive, developmental changes may identify a group of children who may benefit…

Sonography in congenital dislocation of the hip.

A new technique for ultrasonic examination of the hip joint was evaluated in neonatals and infants. An anterior approach was used with the sound sector centered over the femoral head and parallel to the femoral neck. The ultrasonograms corresponded to lateral radiographs of the joint with the leg in Lorenz' first position. It was possible to evaluate the size and depth of the acetabulum and the size and position of the femoral head. The projection also permitted a dynamic examination for determination of hip instability. Thus, the technique provided a method for an objective diagnosis in conge…

The hip in arthrogryposis multiplex congenita.

The hip is involved in up to 80% of individuals with a diagnosis of arthrogryposis multiplex congenita. The hip deformity consists of contracture with or without dislocation. Isolated contracture can usually be treated conservatively by manipulation and splinting, only occasionally requiring operative intervention. Dislocation is as frequently bilateral as unilateral. Bilateral hip dislocations are best left unreduced; only accompanying contractures should be treated. The unilateral dislocation should be treated aggressively, because persistent dislocation will give rise to pelvic obliquity an…
