Hemobilia from a right hepatic artery pseudoaneurysm due to chronic cholecystitis.

Splanchnic pseudoaneurysms are rare causes of hemobilia. Specifically, hepatic artery pseudoaneurysms from infectious or inflammatory etiology are even more rare. In this article, we describe our encounter with a 72-year-old female presenting with obstructive jaundice and acute blood loss anemia. Upper endoscopy indicated hemobilia and endoscopic retrograde cholangiopancreatography was completed with stent in place. Post endoscopic retrograde cholangiopancreatography, computed tomography angiogram indicated a right hepatic artery pseudoaneurysm which was the cause of her hemobilia. The patient

Bilothorax as a complication of percutaneous transhepatic biliary drainage.

We report two cases of bilothorax that occurred as a complication of percutaneous transhepatic biliary drainage. In an 86-year-old woman who had undergone percutaneous transhepatic biliary drainage for obstructive jaundice, bilothorax occurred after accidental removal of the tube. She recovered with chest drainage only. An 83-year-old man who had undergone percutaneous transhepatic biliary drainage for cholecystitis developed bilothorax with infection. He recovered with thoracoscopic curettage. Although bilothorax is a rare complication of percutaneous transhepatic biliary drainage, appropriat

[Laparoscopy in the treatment of complicated forms of acute cholecystitis].

The results of treatment 157 patients 19-88 years with complicated forms of acute cholecystitis. In the first stage 34 patients with destructive cholecystitis and obstructive jaundice, and 4 patients with pancreatitis underwent endoscopic retrograde cholangiopancreatography, and papillosphincterotomy litoekstraktsiya. In 33 cases, percutaneous transhepatic drainage is carried out of the gallbladder. All patients were operated laparoscopically. In 34 observations during the operation was found loose in the 37 observations--dense perivesical infiltration. The conclusion about the priority of min

[A case of IgG4-related sclerosing cholangitis with pancreas divisum].

We report an extremely rare case of IgG4-related sclerosing cholangitis with pancreas divisum. A 62-year-old man presented to our hospital with obstructive jaundice and hilar bile duct stenosis. An inflammatory tumor was suspected due to elevated of blood IgG4 levels. Endoscopic retrograde cholangiopancreatography revealed hilar bile duct stenosis and pancreas divisum. The biopsy specimens obtained by endoscopic ultrasonography-guided fine-needle aspiration biopsy (EUS-FNAB) revealed inflammatory findings. Steroid hormone therapy at an initial dose of 30mg/day resulted in dramatic improvement

[A case of primary hepato-biliary malignant lymphoma effectively treated with R-CHOP chemotherapy].

A 65-year-old man was admitted to our hospital because of obstructive jaundice caused by a mass extending in the porta hepatis, neck of gall bladder and extrahepatic bile duct. The specimens obtained with ultrasound-guided needle biopsy showed malignant lymphoma (diffuse large B-cell lymphoma: DLBCL). CHOP with Rituximab therapy (R-CHOP therapy) was performed. The treatment resulted in remarkable reduction of the tumor size and improvement of the biliary stenosis. We should take into consideration malignant lymphoma when we see a patient with a tumor of the hepato-biliary system.

A patient with persistent pruritus.

We present a patient with an initial and acute presentation of jaundice and marked persistent pruritus. Laboratory and radiology test results eliminated the possibility of acute hepatitis A/B/C viral infections, primary biliary cirrhosis, autoimmune hepatitis, Wilson disease, paraneoplastic cholestasis, and obstructive biliary disease. Centrilobular cholestasis was prominent in a liver biopsy specimen. Benign recurrent intrahepatic cholestasis (BRIC) was diagnosed through a review of the clinical history, available data, and the subsequent exclusion of other possible etiologies. The patient's

Choledochal cyst: diagnosis and surgical treatment.

A 20-year old woman with a 2-month history of epigastric pain, progressive jaundice and obstructive liver biochemistry underwent imaging of the biliary tract, carried out using ultrasonography, CT-scan and MRI. Abdominal ultrasound showed a dilation of the choledochal duct. CT-scan and MRI revealed a cystic dilation of the choledochal duct. Because of possible malignant degeneration, en-bloc cholecystectomy and resection of the cyst were performed, with Roux-en-Y hepaticojejunostomy reconstruction. This case demonstrates the diagnostic value of MRI and MRCP in a pathology that is rather rare i

Vanishing bile duct syndrome in a patient with advanced AIDS.

A 39-year-old HIV-infected woman developed signs and symptoms of obstructive jaundice and cholestasis. Serological tests were positive for cytomegalovirus (CMV) infection. There was no evidence of AIDS cholangiopathy in ultrasonography or magnetic resonance cholangiopancreatography (MRCP). A liver biopsy revealed marked ductopenia and the patient was diagnosed with vanishing bile duct syndrome, thought to be secondary to CMV infection as a result of profound immunosuppression. To the best of our knowledge, this is the first reported case of vanishing bile duct syndrome diagnosed in a patient w

Prolonged anuria complicating primary sclerosing cholangitis: successful outcome following orthotopic liver transplantation.

A 49-year-old man with progressive obstructive jaundice secondary to primary sclerosing cholangitis developed acute, anuric renal failure requiring prolonged hemodialysis. Renal biopsy showed evidence of tubular epithelial toxicity, pigmented casts, interstitial fibrosis, and ischemic glomerular changes. After orthotopic liver transplantation (OLT), there was an immediate return of urine output. The introduction of tacrolimus (FK506) was delayed for 3 weeks after transplantation, during which time a gradual improvement in renal function occurred. Renal function remained stable 1 year later at

[Favorable effect of splenectomy on anemia in 3 siblings with type II congenital dyserythropoietic anemia (HEMPAS). (Ultrastructural changes in erythrocytes after splenectomy)].

Congenital dyserythropoietic anemia type II (CDA-type II) (HEMPAS) was reported in three siblings. CDA-type II was associated with marked hepatosplenomegaly and siderosis of both organs. All three sibling developed cholelithiasis with choledocholithiasis and obstructive jaundice in two of them. Anemia showing hemolytic component with sequestration of erythrocytes in the spleen was corrected after splenectomy without the need of blood transfusions during the follow up period of 20, 11 and 11 years. Ultrastructural investigation of the erythrocytes after splenectomy revealed increased number of

Rhabdomyosarcoma of the biliary tree.

Rhabdomyosarcoma (RMS) of the biliary tree is a rare tumor in children that has a very poor prognosis. Preoperatively, it is often mistaken for a choledochal cyst. We report a case of RMS of the biliary tree in a 4-year-old girl who presented with abdominal pain and obstructive jaundice. The RMS was diagnosed at laparotomy; excision was not possible due to its size and localization. Chemotherapy achieved complete regression of the tumor observed at second-look surgery. Preoperative chemotherapy can now avoid mutilating surgical procedures and improve survival.

[Pancreatic pseudo-tumor caused by bilio-pancreatic tuberculous lymphadenitis. 2 cases].

Peripancreatic tuberculous lymphadenitis is rare and of difficult diagnosis. The two cases described illustrate two clinical aspects according to the proximity of the biliary tract: either anterior lymph nodes responsible for obstructive jaundice, or posterior lymph nodes responsible for chronic epigastric pain. Endoscopic retrograde cholangiopancreatography is essential to exclude a pancreatic lesion and facilitate the interpretation of the CT images. Tuberculosis must then be suspected. In the absence of another tuberculous localisation, laparotomy is necessary to assert the diagnosis. Antit

Relapsing acute pancreatitis as the presenting manifestation of an ampullary neoplasm in a patient with familial polyposis coli.

The incidence of periampullary carcinoma is increased in patients with familial polyposis coli or the Gardner syndrome. Patients with familial polyposis coli and ampullary tumors usually present with obstructive jaundice or abdominal pain. We report the case of a 41-year-old woman with the Gardner syndrome in whom relapsing acute pancreatitis was the presenting manifestation of an ampullary neoplasm. A diagnosis of ampullary neoplasm should be considered in any patient with familial polyposis coli or the Gardner syndrome and pancreatitis, even in cases of relapsing acute pancreatitis.

Streptococcal bacteremia in hepatobiliary operations.

The incidence of fecal streptococci in bile is between 5 and 15 per cent of all positive culture findings. Fecal streptococci are not susceptible to the aminoglycoside or cephalosporin antibiotics, although these drugs are widely favored for prophylaxis during surgical treatment of the biliary tract. Over a three year period, 64 episodes of bacteremia have been identified in patients undergoing treatment for obstructive jaundice. Fifteen (23 per cent) of these episodes were due to fecal streptococci. The implications of this finding are discussed and the role of acylureidopenicillin antibiotic

Choledochal cyst in adults. A clinical and radiological study in ten cases.

Choledochal cysts are being recognized with an increasing frequency among the adult population. In this series a correct preoperative diagnosis was established in eight of the ten patients. The clinical tirad of obstructive jaundice, right upper abdominal pain, and a palpable flactuant mass was noted in only two cases but at least one of these symptoms was present in all patients. Acute pancreatitis, cholecystitis, and biliary calculi were the commonly associated findings. New etiologic and clinical concepts of choledochal cysts are reviewed and the spectrum of radiographic features in adults

Surgical jaundice in infancy.

The surgery of congenital malformations of the biliary system has undergone a revolutionary change during the past decade. Although Kasai's hepatic portoenterostomy operation for biliary atresia is the most dramatic example, radical departures from conventional surgical treatment also have been advocated for choledochal cyst, Caroli's disease, congenital perforation of the bile ducts and biliary hypoplasia. This communication describes the lesions responsible for obstructive jaundice in the early months of life, briefly reviews standard treatment and consolidates the available information abou

Cephalexin levels in human bile in presence of biliary tract disease.

Cephalexin was given to 24 patients before and after operation on the bile ducts and gall bladder. Two patients had obstructive jaundice. Samples of the bile were taken either directly from the gall bladder at operation or via the T-tube. Cephalexin was excreted in the bile, peak levels being obtained after two to three hours. These levels could be raised if probenecid was given concurrently. Higher levels were found in patients with functioning gall-bladders. A trial of cephalexin seems justified for the treatment of typhoid carriers.
