Intracellular distribution of a speech/language disorder associated FOXP2 mutant.

Although a mutation (R553H) in the forkhead box (FOX)P2 gene is associated with speech/language disorder, little is known about the function of FOXP2 or its relevance to this disorder. In the present study, we identify the forkhead nuclear localization domains that contribute to the cellular distribution of FOXP2. Nuclear localization of FOXP2 depended on two distally separated nuclear localization signals in the forkhead domain. A truncated version of FOXP2 lacking the leu-zip, Zn2+ finger, and forkhead domains that was observed in another patient with speech abnormalities demonstrated an agg…

Comorbidity in severe developmental language disorders: neuropediatric and psychological considerations.

AIM: To explore possible patterns of comorbidity in children with severe developmental language disorders (DLD). METHODS: A retrospective investigation of the clinical records of 28 children relating to oral motor and language problems, psychological profiles, medical history and EEG findings. RESULTS: 36% of all the children had pure expressive language problems, 64% had combined expressive-receptive language problems and 57% had additional oral motor problems. Girls predominated in expressive-receptive problems, while boys predominated in oral motor problems. Children with expressive-recep…

Genetic influences on language impairment and phonological short-term memory.

It has been known for some years that specific language impairment (SLI), an unexpected failure to acquire age-appropriate language skills, is highly heritable. However, molecular genetic studies have been hampered by the heterogeneity of the disorder and the predominant lack of clear genotype-phenotype relationships. We review recent studies suggesting that a better understanding of the genetics of SLI might emerge if we move away from clinical criteria for diagnosis to look instead at a theoretically based quantitative and cognitive measure of the phenotype: a test of phonological short-term…

An interpreter's interpretation: sign language interpreters' view of musculoskeletal disorders.

INTRODUCTION: Sign language interpreters are at increased risk for musculoskeletal disorders associated with work. Previous studies have used survey techniques to identify potential risk factors and approaches to their medical management. Little is known about risk factors and management of symptoms in this group from the perspective of the interpreter. Such qualitative information should help inform future research related to this professional group. METHOD: One thousand ninety-two sign language interpreters recruited from the Registry of Interpreters for the Deaf completed an open-ended que…

[Specific language disorders: their diagnosis, classification and study using magnetoencephalography].

INTRODUCTION: Specific language disorder (SLD) is defined as a disorder in which language is slow and retarded with respect to the patient's chronological age, and which is not related to a sensory-auditory or motor deficit or to a pervasive development disorder; in other words, structural and functional neurological developmental disorders must be excluded. After many studies and attempts to reach an agreement, specific language disorders have been broadly classified into two groups: specific expressive language disorder and specific receptive-expressive language disorder. AIMS: Our objectiv…

Dyslexia and dysgraphia: more than written language difficulties in common.

A dual-task paradigm involving concurrent finger tapping and line orientation judgment was used to investigate brain processing differences … slexia and dysgraphia share a left-hemisphere processing limitation that is not confined to written language. In light of other relevant research findings, I suggest that this limitation is due to the absence of a disembedding scanning mechanism for converting spatial arrays (e.g., spelling patterns) to temporal form-an impairment putatively caused by atte …

Language development of a multiply handicapped child after cochlear implantation.

The presence of additional handicaps in hearing-impaired children makes the prediction of language ability after cochlear implantation unreliable. Only limited follow-up data on developmental improvement after implantation among multiply handicapped children is available. The present study reports the course of development (audiological and linguistic) after cochlear implantation in one subject with moderate mental retardation. Preoperatively, his language development showed 34 months delay when compared to chronological age. The difference had shortened to 23 months by 2 years post-surgery. T…

Auditory processing efficiency deficits in children with developmental language impairments.

The "temporal processing hypothesis" suggests that individuals with specific language impairments (SLIs) and dyslexia have severe deficits in processing rapidly presented or brief sensory information, both within the auditory and visual domains. This hypothesis has been supported through evidence that language-impaired individuals have excess auditory backward masking. This paper presents an analysis of masking results from several studies in terms of a model of temporal resolution. Results from this modeling suggest that the masking results can be better explained by an "auditory efficiency" …

Functional plasticity of language-related brain areas after cochlear implantation.

Using PET, the cerebral network engaged by heard language processing in normal hearing subjects was compared with that in patients who received a cochlear implant after a period of profound deafness. The experimental conditions were words, syllables and environmental sounds, each controlled by a noise baseline. Four categories of effect were observed: (i) regions that were recruited by patients and controls under identical task conditions: the left and right superior temporal cortices and the left insula were activated in both groups in all conditions; (ii) new regions, which were recruited by…

fMRI auditory language differences between dyslexic and able reading children.

During fMRI, dyslexic and control boys completed auditory language tasks (judging whether pairs of real and/or pseudo words rhymed or were real words) in 30 s 'on' conditions alternating with a 30 s 'off' condition (judging whether tone pairs were same). During phonological judgment, dyslexics had more activity than controls in right than left inferior temporal gyrus and in left precentral gyrus. During lexical judgment, dyslexics were less active than controls in bilateral middle frontal gyrus and more active than controls in left orbital frontal cortex. Individual dyslexics were reliably les…

Language deficits in dyslexic children: speech perception, phonology, and morphology.

We investigated the relationship between dyslexia and three aspects of language: speech perception, phonology, and morphology. Reading and language tasks were administered to dyslexics aged 8-9 years and to two normal reader groups (age-matched and reading-level matched). Three dyslexic groups were identified: phonological dyslexics (PD), developmentally language impaired (LI), and globally delayed (delay-type dyslexics). The LI and PD groups exhibited similar patterns of reading impairment, attributed to low phonological skills. However, only the LI group showed clear speech perception defici…

The asleep-awake-asleep anesthetic technique for intraoperative language mapping.

OBJECTIVE: We evaluated a combined technique designed for procedures requiring intraoperative language mapping. We planned to induce general anesthesia with endotracheal intubation and hyperventilation and then to awaken and extubate the patient for speech testing. After the latter, endotracheal reintubation and general anesthesia were planned. METHODS: With the patient under intravenously induced sedation, we topically anesthetized the airway with lidocaine that was delivered through a spraying catheter. Fiberoptic endotracheal intubation was then performed on the awake patient, using a modi…

Language guiding therapy: the case of dehydration versus volume depletion.

Indiscriminate use of the terms dehydration and volume depletion, so carefully crafted by our predecessors, risks confusion and therapeutic errors. These two conditions should be distinguished at the bedside and in how we speak to one another. Dehydration largely refers to intracellular water deficits stemming from hypertonicity and a disturbance in water metabolism. The diagnosis of dehydration cannot be established without laboratory analysis of p[Na +] or calculation of serum tonicity. In contrast, volume depletion describes the net loss of total body sodium and a reduction in intravascular

Adults with language-learning disabilities: new challenges and changing perspectives.

As children and adolescents mature into adult life, their language-learning disabilities continue. Consequently, the view that language-learning disabilities are limited to the school years must be changed. This article reviews our understanding of adults with language-learning disabilities. Ecological and context-specific perspectives are presented to provide a framework for understanding the chronic and persistent nature of these disabilities across the lifespan. Descriptors of language-learning disorders in adults are provided that focus the clinician on observable behaviors to guide the pr…

Profiling agrammatic spoken language: towards a government and binding framework.

An analytical procedure for describing agrammatic spoken language based on government and binding theory is used to characterise the spoken grammar of an aphasic adult (JR) with severe agrammatism. Analysis of JR's spoken language revealed that he produced a greater number of single-word utterances (single lexical items) in comparison with lexical and/or functional projections. both morphological and syntactic evidence suggested that JR retained a differentiated set of lexical categories. No violations of word order were found. Target determiners and target inflection phrases were not always c…

Teacher-identified oral language difficulties among boys with attention problems.

Teachers evaluated the language functioning of 95 boys, aged 6.5 to 13.8 years, identified as having attention problems (AP, n = 30), learning disability (LD, n = 33), or average achievement (AA, n = 32). The three groups did not differ significantly (p > .05) in their frequency of articulation problems. Significantly (p < .05) more AP boys were rated as having pragmatics problems than both of the other groups. AP boys were also viewed as having a higher frequency of receptive/expressive language problems than were AA boys, but not compared with LD boys. LD and AA boys did not differ in their …

Language acquisition after mutism: a longitudinal case study of autism.

This longitudinal case study challenges the assumption that individuals with autism who have severely restricted speech and language skills have a poor prognosis for further development of expressive oral language. The study follows the development of a woman with autism from mutism at age 10 to acquisition of a range of spoken and written language skills at age 26. The intervention in which the woman participated and her skills pre- and post-intervention and at two follow-up assessments are documented. The results support the hypotheses that speech and language development may proceed after m…
