Reliability of the active and passive knee extension test in acute hamstring injuries.

BACKGROUND: Hamstring flexibility measurements are of clinical relevance for the prognosis of hamstring injury and for monitoring recovery after such injury. The active knee extension test (AKET) and passive knee extension test (PKET) are proven to be reliable in healthy subjects. Reliability has not been determined in patients with acute hamstring injuries. PURPOSE: The purpose of this study was to determine intertester reliability of the AKET and the PKET in patients with acute hamstring injuries. STUDY DESIGN: Cohort study (diagnosis); Level of evidence, 2. METHODS: Fifty consecutive ath

Unsupervised children in vehicles: a risk for pediatric trauma.

In this study, a series of instances of children injured by a motor vehicle set in motion by an unsupervised child are reviewed. During a 24-month period, nine such children were identified through a multihospital and coroner's office monitoring system in a single urban county. Injuries ranged from multiple abrasions and contusions to serious leg and head injuries. Three children died. The typical circumstance involved a child releasing the brake or placing the vehicle in gear in a private driveway which resulted in the vehicle striking or rolling over the victim. In four of the nine cases, th

Relationship of Physical Factors to the Occurrence of Injuries in Young Gymnasts.

BACKGROUND: There is a large population of young athletes who participate in gymnastics, and the prevention of injury in junior athletes is considered important. However, few studies have prospectively investigated the relationship between physical factors and the occurrence of injury. PURPOSE: To investigate the physical characteristics that are factors in the injury occurrence in elementary and junior high school gymnasts. STUDY DESIGN: Prospective observational study. METHODS: A total of 36 healthy young gymnasts (at national competition level) were enrolled in the study. Once a week for

Differences in injury risk and characteristics of injuries between novice and experienced runners over a 4-year period.

OBJECTIVES: Running is increasingly popular, but also carries a high injury risk, especially in novice runners. This study investigates differences in incidence and characteristics of running-related injuries (RRIs) between novice and experienced runners in the open population, in order to adapt to more specific needs for future prevention programs. METHODS: Data were obtained from Injuries and Physical Activity in the Netherlands (IPAN), a Dutch questionnaire on injuries, physical activity, and sports. A representative sample of runners (n = 4621) from IPAN, including 1405 novice runners, wa

Prevalence of and risk factors for hock and knee injuries on dairy cows in tiestall housing in Canada.

Leg injuries on dairy cows are a common and highly visible welfare concern on commercial dairy farms. With greater attention being placed on food animal welfare and limited research being conducted on tiestall farms, this study aimed to identify prevalence and risk factors for hock and knee injuries on dairy cows housed in tiestall barns in Ontario (n=40) and Quebec (n=60). A sample of 40 cows was purposively selected per farm and several animal- and farm-based measures were taken. Both hocks and both knees on each cow were scored as injured (presence of lesions or swelling) or not injured (no…

Pure closed posteromedial dislocation of the tibiotalar joint without fracture.

Pure tibiotalar dislocation without an associated fracture is an extremely rare injury. We present three cases of closed posteromedial tibiotalar dislocation without any associated fractures to the foot, ankle, or leg. All patients were treated conservatively with immediate closed reduction under general or local anaesthesia and immobilised in a short leg cast for six weeks without weight-bearing resulting in a satisfactory outcome at the final follow-up. A review of the literature is also presented in this paper.

Congenital aplasia of the cruciate ligaments. A report of six cases.

Instability of the knee is frequently found in association with congenital leg-length discrepancy. We have studied six such patients clinically, radiologically and arthroscopically. Clinical signs of knee instability and significant radiological changes were present in all, and at arthroscopy the anterior cruciate ligament was completely absent in four patients and functionless in the other two. This deficiency appears to be a congenital condition which may predispose to meniscus injury or retropatellar pain; it may also lead to subluxation or dislocation of the knee during leg-lengthening pro

The brain tumor of George Gershwin and the legs of Cole Porter.

George Gershwin died in 1937 of a glioblastoma of the right temporal lobe. He had been in psychoanalytical care for some time and was hospitalized a few weeks before his death, when he was thought to have a functional illness. The controversies about George Gershwin's death, duration of neurologic symptoms, and problems in diagnosis are discussed. Cole Porter fell off a horse he was riding in 1937 and sustained multiple open fractures of both legs. There probably was some nerve injury in the right leg, at least, from this fall. Despite intensive pain, many hospitalizations, and 33 operations o

Playground slide-related injuries in preschool children: increased risk of lower extremity injuries when riding on laps.

BACKGROUND: The purpose of this study was to better understand the factors associated with playground slide-related injuries in preschool children and to test the hypothesis that riding on laps increases the likelihood of lower extremity injuries. METHODS: Playground slide-related injuries (product code 1242) in children ≤5 years of age treated in emergency departments from 2002 to 2015 were identified (N = 12,686) using the U.S. Consumer Product Safety Commission's National Electronic Injury Surveillance System (NEISS). Descriptive and comparative analyses, including chi-square testing and b

Reconstruction of neglected Achilles' tendon defect with peroneus brevis tendon allograft: a case report.

This case report describes the use of a peroneus brevis allograft to reconstruct a neglected Achilles' tendon injury in a 75-year-old woman. She had difficulty walking, had stiffness, and was unable to perform a heel raise from a single-leg standing position. Magnetic resonance imaging confirmed a 6.8-cm wide defect 13 months after the initial injury. Surgical repair of the neglected rupture was performed using 4 strands of peroneus brevis allograft to bridge the defect. Early weight bearing and rehabilitation was allowed. At follow-up clinical examination 14 months postoperation, the patient

Management of a rare intra-articular fracture of the lateral femoral condyle: case report.

Unicondylar fractures of the femur are uncommon injuries that can occur in the sagittal or, less frequently, in the coronal plane (Hoffa fractures).  Distal femoral fractures are usually described following the AO/OTA Classification system which includes extra-articular, partial articular and intra-articular injuries, further divided in three types based on the pattern and comminution.  Accurate reduction and stable fixation are needed especially in articular injuries in order to allow early mobilization and reduce complications such as knee stiffness, malunion or secondary osteoarthritis. Foot-Ankle Fractures and Injury Probability Curves from Post-mortem Human Surrogate Tests.

This purpose of this study was to replicate foot-ankle injuries seen in the military and derive human injury probability curves using the human cadaver model. Lower legs were isolated below knee from seventeen unembalmed human cadavers and they were aligned in a 90-90 posture (plantar surface orthogonal to leg). The specimens were loaded along the tibia axis by applying short-time duration pulses, using a repeated testing protocol. Injuries were documented using pre- and post-test X-rays, computed tomography scans, and dissection. Peak force-based risk curves were derived using survival analys

Tourniquet cuff pressure and nerve injury in knee arthroplasty in a bloodless field: a neurophysiological study.

BACKGROUND AND PURPOSE: Tourniquet-related nerve injuries remain a concern in orthopedic surgery. The cuff pressures used today are generally lower, and therefore a decreasing incidence of peripheral nerve injuries might also be expected. However, there have been few neurophysiological studies describing the outcome after bloodless field surgery. We describe the results of neurophysiological examinations and report the incidence of nerve injuries after total knee arthroplasty (TKA) in a bloodless field. PATIENTS AND METHODS: This study was part of a prospective, randomized controlled clinical

Association between body mass index and acute traumatic workplace injury in hourly manufacturing employees.

In this study, the authors examined the distribution and odds of occupational injury among hourly employees of a US aluminum manufacturing company by body mass index (weight (kg)/height (m)(2)). In 2002, height and weight data on 7,690 workers at eight plants were extracted from medical records from annual physicals, and body mass index was categorized. Information on traumatic injuries recorded between January 1, 2002, and December 31, 2004, was obtained from a company injury surveillance system. Twenty-nine percent of the employees (n = 2,221) sustained at least one injury. Approximately 85

Adolescent Morel-Lavallée Lesions: A Case Report of Challenges in Diagnosis and Management in Atypical Locations.

BACKGROUND Morel-Lavallee lesions (MLLs) are uncommon, closed soft-tissue degloving injuries caused by high-energy trauma that are often missed due to their rarity and delayed presentation, resulting in serious complications. MLLs are particularly missed and underreported in pediatric and adolescent patients. We describe the case of an adolescent MLL occurring in an atypical lesion site at the calf to increase awareness of this diagnosis and associated presentation in this patient group, which can differ from adult presentation and contribute to diagnostic uncertainty that consequently impacts

Lower Extremity Sandblast Injury: A Rarely Seen Injury Mechanism in the Civilian Population.

Blast injuries are both complex and rare in the civilian population. This combination can often lead to missed opportunities for early, effective intervention. This is a case report of a 31-year-old male who suffered a lower extremity blast injury while using an industrial sandblaster. This blast injury presented as a closed degloving, or Morel-Lavallee lesion, which can easily be mistreated and lead to infection and further disability. Following assessment, identification, and confirmation of the Morel-Lavallee lesion via radiographic imaging, this patient underwent debridement surgery, wound

Hamstring muscle injuries in athletics.

Hamstring muscle injuries (HMI) are a common and recurrent issue in the sport of athletics, particularly in sprinting and jumping disciplines. This review summarizes the latest literature on hamstring muscle injuries in athletics from a clinical perspective. The considerable heterogeneity in injury definitions and reporting methodologies among studies still needs to be addressed for greater clarity. Expert teams have recently developed evidence-based muscle injury classification systems whose application could guide clinical decision-making; however, no system has been adopted universally in c
