Cognitive dysfunction has been widely studied in multiple sclerosis (MS), however decision-making has been less investigated. The current study examined the decision-making processes of patients with multiple sclerosis (MS) using a computerized version of the Iowa Gambling Task (IGT). This task was applied to 18 patients with clinically diagnosed MS and 18 healthy control subjects matched for age, sex, and years of education. The results showed that IGT scores differ significantly between patients with MS and the healthy control subjects: the patients with MS made significantly less advantageo…

BACKGROUND: The purpose of this study was to assess decision making in patients with multiple sclerosis (MS) at the earliest clinically detectable time point of the disease.
METHODS: Patients with definite MS (n = 109) or with clinically isolated syndrome (CIS, n = 56), a disease duration of 3 months to 5 years, and no or only minor neurological impairment (Expanded Disability Status Scale [EDSS] score 0-2.5) were compared to 50 healthy controls using the Iowa Gambling Task (IGT).
RESULTS: The performance of definite MS, CIS patients, and controls was comparable for the two main outcomes of …

OBJECTIVE: To describe temporal profile of chronic inflammatory demyelinating polyradiculoneuropathy (CIDP) in patients with definite, relapsing multiple sclerosis (MS).
BACKGROUND: Peripheral demyelinating neuropathy has been rarely reported in association with central nervous system demyelinating disorder indistinguishable from MS.
METHODS: In addition to usual diagnostic studies for CIDP and MS in all 5 patients, we studied proximal segments of nerves using deep tendon reflex latency measurements of biceps reflex, patellar reflex, and ankle reflex.
RESULTS: All patients with MS subsequen…

(1) In relapsing-remitting multiple sclerosis, the standard therapy (other than symptomatic treatment) is interferon beta. It prevents about one exacerbation every 2.5 years but has no demonstrated effect on the progression of disability. However, interferon beta can cause serious adverse effects. (2) Natalizumab, an immunosuppressant, has been approved for first-line treatment of patients with "aggressive" multiple sclerosis (with frequent exacerbations) and for second-line treatment after failure of interferon beta. (3) In first-line treatment, natalizumab has not been compared with interfer…

Sixty percent of the CSF samples from 21 multiple sclerosis (MS) patients in remission inhibited cellular proliferation of a variety of neuronal and glial cell lines. No inhibition was observed with CSF from relapsing or progressive MS patients (0/19). Two of 82 samples from stationary MS patients were positive. No inhibition was found in MS sera. The CSF samples had no effect on myelination of mouse organotypic cerebellar cultures. Like MS cellular proliferation and subsequent lesion formation, the 'deactivation' of the lesion also is an active defined process, rather than just the cessation …

The case of a 30-year-old woman who had two episodes of photopsia along with sudden-onset monocular visual field defects, developing into bilateral tunnel vision within 4 years, is reported. She also had episodes of a right hemiparesis and right-sided hypoaesthesia, accompanied by severe fatigue. This patient fulfilled the criteria for both clinically definite multiple sclerosis and acute zonal occult outer retinopathy (AZOOR). AZOOR can have an onset with monocular visual field loss, and can be distinguished from optic neuritis. In addition, some observations suggest common neuropathological …

Bladder dysfunction is a common problem for patients with multiple sclerosis. The severity of symptoms often correlate with the degree of spinal cord involvement and, hence, the patient's general level of disability. The emphasis of management is now mainly medical and is increasingly offered by nonurologists. Treatments can be highly effective, relieving patients of what are otherwise very troublesome symptoms that would compound their neurological disability. This article gives an overview of the neural control of the bladder, followed by an explanation of the pathophysiology of detrusor ove…

OBJECTIVE: To ascertain if infectious mononucleosis is a risk factor for the development of multiple sclerosis (MS); and, if it is, whether its effect is close to or remote in time from the onset of MS.
DESIGN: Analysis of database of linked abstracts of records of hospital admission and death.
SETTING: Health region in central southern England.
MAIN OUTCOME MEASURE: Ratio of rate of MS in a cohort of people admitted to hospital with infectious mononucleosis to the rate in a comparison cohort.
RESULTS: Considering all time intervals from admission with infection to admission with MS, there…

Multiple sclerosis (MS) is a fairly common condition that affects approximately 350,000 people in the United States. It is associated with various neuropsychiatric symptoms including cognitive and behavioral symptoms. However, visual hallucinations are rare in multiple sclerosis without the presence of cognitive deficits. We are describing the case of a 40-year-old married white female with isolated complex visual hallucinations compatible with the Charles Bonnet syndrome (CBS). The patient was successfully treated with the atypical antipsychotic olanzapine.

Multiple sclerosis (MS) is the most common demyelinating inflammatory disease of the central nervous system (CNS), presenting with multifocal, disseminated inflammatory lesions referred to as plaques. Magnetic resonance imaging (MRI) typically depicts multiple, round to oval, circumscript lesions predominantly involving periventricular and subcortical white matter, brainstem and cerebellum. More recent investigations have demonstrated that the macroscopically visible plaques only present the tip of the iceberg: Already early in its course, MS causes neuroaxonal damage and diffusely involves th…

A comparative study of 5 patients with definite multiple sclerosis who have abnormally high titer of anticardiolipin IgG-antibodies [MS aCL(+)] and 9 patients with antiphospholipid syndrome (APS) aged 22-43 was carried out. The results of this study are reported. The investigation has shown that some clinical and magnetic resonance imaging (MRI) sings in patients with APS imitate the course of MS. This situation makes differentiation of two conditions quite difficult. The peculiarities of course of MS aCL(+) such as progressive myelitic pattern, nontypical for MS articular, cutaneous (livedo, …

Sexual dysfunction affects a large part of patients suffering from multiple sclerosis, but some aspects of its clinical presentation and aetiology are not clearly defined yet. In an unselected sample of 108 patients with definite multiple sclerosis we investigated the relationship between symptoms of sexual dysfunctioning and sphincteric dysfunction, patients' and disease characteristics, disability and neurological impairment, psychological and cognitive functioning. Sexual dysfunction directly correlated with presence of physical disorders (r=0.37, P=0.0004), low educational level (r=0.32, P…

This case illustrates the occurrence of a sudden hearing sensitivity loss that, in all likelihood, was the result of brainstem disorder resulting from multiple sclerosis (MS). Subject LD is a young woman who developed a sudden hearing loss while hospitalized for exacerbation of symptoms related to multiple sclerosis. By her own report, she had normal hearing in her left ear at the time of hospitalization. Four days after admission, she developed a hearing loss in her left ear, accompanied by roaring tinnitus and fullness. An audiologic evaluation revealed a substantial high-frequency sensitivi…

Frozen brain specimens from patients with multiple sclerosis (MS) and other neurologic diseases were analyzed using immunocytochemical techniques for the presence of TNF. In brain lesions in MS, and subacute sclerosing panencephalitis, TNF+ cells were demonstrated. At the lesion site in MS, TNF+ staining is associated with both astrocytes and macrophages. These observations were not made in Alzheimer's disease or normal brain tissue. The presence of TNF in MS lesions suggests a significant role for cytokines and the immune response in disease progression.

A patient is presented with clinical signs and symptoms of neuromyelitis optica (Devic's syndrome). The clinical findings, the cerebrospinal fluid findings and the Magnetic Resonance Imaging findings in our patient demonstrate that neuromyelitis optica is not always a form of Multiple Sclerosis. It can also be a manifestation of other demyelinating diseases, for example Disseminated Encephalomyelitis.

Among 56 established cases of multiple sclerosis in our clinic from April 1978 to March 1985, optic neuritis was found in 38; 21 cases were bilateral. We studied pattern-evoked potentials detected with 16 electrodes placed on both the scalp and the facial area. Pattern-evoked potentials (PERGs) were obtainable at the facial area in multiple sclerosis patients with optic neuritis, except in long-standing cases. However, a significant reduction of amplitude was found even in cases without optic neuritis. No correlation was found between visual acuity and amplitude or peak latency of PERGs in MS …
