The pro-angiogenic cytokine vascular endothelial growth factor (VEGF) has been implicated in periprosthetic osteolysis and subsequent aseptic loosening of implants following total hip arthroplasty (THA). The goal of this study was to investigate whether increased VEGF at the bone-implant interface is secondary to a greater number of VEGF-producing cells or to increased VEGF production by individual cells. Real time polymerase chain reaction (RT-PCR) techniques were used to assess the expression of VEGF mRNA (isoforms 121, 165, 189) in periprosthetic tissues from revision THAs. Immunofluorescen

This study describes the results of a procedure for removing the infected aortofemoral graft using a two-stage procedure with a delay between the stages. The objective was to lessen the morbidity and mortality associated with removing an infected graft through a single operation. Eight consecutive patients were treated in this manner over a 6-year span. The indications for surgery were infected groin false aneurysms in three, chronic draining sinuses involving the prosthetic graft in four, and an open groin infection involving graft in one. There were five males and three females, with ages ra

INTRODUCTION: Extraoral craniofacial implants are often used for retention of auricular prostheses. In this case report, the rare complication of sigmoid and transverse sinus thrombosis after implant placement in the mastoid is described.
CASE REPORT: A 36-year-old man with congenital hypoplasia of the right ear was treated with an implant-based auricular prosthesis. One of the three implants in the mastoid area had to be removed because of an infection, whereas the other two implants were removed at the patient's request. Twenty-four days after implant removal, he was seen with sigmoid and t

Prosthetic valve endocarditis is still a very serious complication, carrying an incidence of death between 30 and 70% in some series. Therefore early and accurate diagnosis is crucial. Early (less than 60 days post surgery) endocarditis is usually a fulminant disease, where staphylococcal infection is most common. Late prosthetic endocarditis resembles more closely other forms of the disease. Conventional echocardiography is useful in the evaluation of prosthetic valve function, but it is very limited in the demonstration of infective lesions, primarily because of acoustic shadowing. INTRODUCTION: The aim was to study the effect of incisional negative pressure wound therapy (iNPWT) in wound healing compared with standard sterile gauze dressings after major lower extremity amputation in patients with peripheral arterial disease (PAD).
METHODS: This prospective, randomised controlled trial included 50 patients undergoing major lower extremity amputations for PAD. Patients were randomised into iNPWT and standard dressing groups. The patency of blood vessels at the level of the stump was ensured with or without revascularisation. The primary outcome was wound-related complica

INTRODUCTION: This study aims to evaluate functional and quality-of-life (QoL) outcomes in patients with periprosthetic joint infection (PJI) undergoing implantation of a metal-on-polyethylene articulating knee spacer as part of a planned two-stage revision, and to explore preoperative predictors of meaningful postoperative improvement. METHODS: This was a retrospective observational study including 108 consecutive patients with chronic PJI treated with a metal-on-polyethylene articulating spacer between 2016 and 2020 and followed prospectively for clinical outcomes. QoL and joint-specific fun

BACKGROUND: Although the gold standard for treatment of periprosthetic joint infection (PJI) is a two-stage exchange, patients may elect to retain a well-functioning spacer. The aim of this retrospective review was to evaluate the survivorship, complications, and patient-reported outcome measures (PROMs) of retained articulating total knee arthroplasty (TKA) spacers with patellar component replacement.
METHODS: All PJI patients from a tertiary care academic center who received a balanced, articulating metal-on-polyethylene TKA spacer with patellar component replacement were reviewed. After th

BACKGROUND Aorto-aortic extra-anatomical bypass is a specialized surgical intervention primarily used to manage complex vascular complications, including stent graft infections after endovascular aortic repair. Stent graft infection is a rare but severe complication associated with high morbidity and mortality, which requires prompt treatment to prevent life-threatening sequelae such as pseudoaneurysm formation, aortic rupture, or fistula formation. CASE REPORT We describe 2 young male patients who developed infected thoracic aortic stent grafts after aortic arch debranching and endovascular r

INTRODUCTION: Historically, contra-indications for immediate salvage of an infected penile prosthesis have included purulence on the device, purulent infections in diabetic patients, and severe diabetes. These clinical principles are from the 1990s to early 2000s and based on limited retrospective series or expert opinion. Since the publication of these contra-indications there have been improvements in device design, surgical technique, corporal washout protocols, and perioperative antibiotic prophylaxis that allow us to re-evaluate whether we can broaden the criteria for immediate salvage of

AIMS: The Intraosseous Transcutaneous Amputation Prosthesis (ITAP) may improve quality of life for amputees by avoiding soft-tissue complications associated with socket prostheses and by improving sensory feedback and function. It relies on the formation of a seal between the soft tissues and the implant and currently has a flange with drilled holes to promote dermal attachment. Despite this, infection remains a significant risk. This study explored alternative strategies to enhance soft-tissue integration.
MATERIALS AND METHODS: The effect of ITAP pins with a fully porous titanium alloy flan

Mucopolysaccharidosis (MPS) are rare genetic disorders, caused by enzymatic defects that lead to abnormal glycosaminoglycan metabolism and its accumulation. Hurler-Scheie syndrome (MPS I) is associated with a deficiency of the lysosomal enzyme α-L-iduronidase. Enzymatic replacement with intravenous laronidase is a frequently utilized therapeutic option. In patients with MPS I, progressive glycosaminoglycan storage in the heart can lead to valvular abnormalities; however, few surgical heart valve interventions have been reported in MPS I patients. We present an unusual case of a double-valve re

Penile implants are surgically inserted devices used for the treatment of erectile disfunction. Improvements in surgical techniques and in the manufacturing of these implants have reduced the risk of intra- and postoperative complications and increased the popularity and use of this therapeutic option. Routine imaging is not recommended before penile prosthesis surgery. Malleable penile prostheses use noninflatable shafts inserted into each corpus cavernosum, which appear hypointense on T1- and T2-weighted MR images. Inflatable penile prostheses (IPPs) may consist of two or three parts and are

The results of 52 Lotus unicompartmental knee implants with a follow up time exceeding 5 years are analysed. Forty-one prostheses with a mean follow-up of 6.5 years have given very good or good relief of pain. In almost all cases, flexion range exceeds 90 degrees and the preoperative motion has been most often clearly improved. Except for those due to prosthesis infection, all the poor and fair results in terms of pain, are explained by mechanical causes i.e. persistence of a frontal instability, loosening of the tibial component (2 cases) or narrowing of the nonprosthetic femorotibial space (

BACKGROUND: A 2-stage approach is most commonly used to treat periprosthetic joint infection (PJI). Some successful studies of the 1-stage approach were underpowered, lacked a 2-stage comparative group, and excluded patients with draining sinuses, comorbidities, and/or antibiotic-resistant organisms. Given the morbidity and expense associated with 2-stage treatment, we conducted a prospective, multicenter, randomized trial to compare the results of 1- and 2-stage PJI treatment, specifically including patients with draining sinuses, comorbidities, and resistant organisms.

BACKGROUND: Reoperations following total hip arthroplasty (THA) remain a major clinical challenge, with their incidence and socioeconomic burden rising despite advances in surgical techniques and prosthesis design. This study aimed to evaluate the predominant causes of reoperations following THA over two decades at a tertiary referral hospital, comparing trends between 2004 to 2013 and 2014 to 2023.
METHODS: We analyzed all reoperations performed at a tertiary referral hospital between January 2004 and December 2023. A total of 515 hips (483 patients) were included after excluding multiple re

INTRODUCTION: There is continued debate regarding the survivorship and revision rate of cementless versus cemented total knee arthroplasty (TKA) prostheses. This includes the assessment of early revision surgery due to aseptic loosenings and periprosthetic joint infections (PJIs). Studies have not always taken into account the impact of comorbidities, such as diabetes, obesity, and tobacco. Therefore, we compared revisions in a large population of patients undergoing cemented or cementless TKAs at 90 days, 1 year, and 2 years.
METHODS: A review of an administrative claims database was used to
